NHTSA Vehicle Safety Complain To whom it may concern, My Name is [XXX] I am submitting this complaint regarding my 2019 Ford F-250 Super Duty equipped with the 6.7L Power Stroke diesel engine (VIN: [XXX]). At approximately 137,900 miles, my vehicle suddenly lost engine power while driving and stalled without warning, creating a serious safety hazard. After stalling, the truck would not restart and had to be towed to a repair facility. The repair shop diagnosed a catastrophic failure of the diesel high-pressure fuel injection pump. According to the repair invoice, fine metallic flakes were found throughout the entire fuel system, contaminating the fuel lines, fuel tank, injectors, and related components. Because of this contamination, the shop determined that the entire fuel system required replacement, including replacement of all eight fuel injectors, cleaning of the fuel tank and fuel lines, replacement of the fuel filter, and installation of an upgraded diesel injection pump. The estimated repair cost exceeded $11,000, resulting in a significant financial burden. I understand that Ford Safety Recall 24S78 addresses a high-pressure fuel pump issue on certain 2020–2022 Ford Super Duty trucks equipped with the same 6.7L Power Stroke diesel engine. Although my 2019 model is not included in the recall, my vehicle experienced what appears to be a very similar failure that resulted in sudden loss of engine power while driving and complete contamination of the fuel system. I am not asserting that my vehicle is covered by the current recall. Rather, I respectfully request that NHTSA investigate whether the same or a substantially similar defect may also affect certain 2019 Ford F-250 Super Duty vehicles equipped with the 6.7L Power Stroke diesel engine. If the investigation determines that these vehicles are affected by the same safety defect, I ask that the recall be expanded accordingly. The contact owns a 2019 Ford F-250. The contact stated while driving 30 MPH and depressing the accelerator pedal, the vehicle failed to respond as intended and lost motive power. The contact reversed over to the right side of the roadway and exited the vehicle. The contact inspected under the hood but found no cause for the failure. The contact re-entered the vehicle, shifted into reverse, and reversed back into the driveway. The contact stated the failure had occurred on two separate occasions. The vehicle was towed the dealer where it was diagnosed and determined that rodents had chewed on the wiring harness, and the wiring harness needed to be replaced. The vehicle was repaired. The manufacturer was notified of the failure and opened a case. The contact was referred to the NHTSA Hotline to report the failure. The approximate failure mileage was 82,000.

I was driving my truck with no prior issues nor check engine light warnings, suddenly my truck began to lose speed, warning light came on instrument panel stating something to the effect of reduced power, I heard a loud noise (similar to the sound of a tire blown), as I looked out of the drivers side window I observed flames coming from the left front fender well. I was able to pull out of traffic coasting off into the median. Once in the median My wife and myself had to evacuate the truck immediately for our own safety. the engine compartment was rapidly becoming fully involved with fire. Attempted to extinguish fire with three fire extinguishers, called 911 and waited on fire department to arrive. My truck is now a total lose, I have read that your department has received similar issues that I have described.I felt it necessary to report my issue to hopefully help resolve this issue with Ford trucks.

The rearview camera may display a distorted, inverted, or blank image when the vehicle is in reverse.

Consequence & remedy

Consequence: A rearview camera that displays an inverted, distorted, or blank image can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will inspect and replace the rearview camera as necessary, free of charge. Owner notification letters were mailed January 13, 2026. The owner's manual does not include instructions for adjusting or removing certain head restraints. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 202, "Head Restraints."

Consequence & remedy

Consequence: Without instructions, the head restraints may be improperly adjusted, increasing the risk of injury in the event of a crash.

Remedy: Ford will mail the Owner's Manual addendums that describe how to adjust and remove the head restraints for the center seating positions, free of charge. Consequence & remedy

Consequence: Lack of a clear rearview camera image can reduce the driver's visibility of what is behind the vehicle, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2017-2019 Super Duty F-250, F-350, and F-450 trucks equipped with an electric tailgate latch release switch mounted in the tailgate handle. Water may enter the electrical wiring system and cause a short circuit, activating the switch and releasing the tailgate latches.

Consequence & remedy

Consequence: Unintended tailgate opening may allow unrestrained cargo to fall out of the truck bed and become a road hazard, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will modify the tailgate/frame wiring harnesses by adding jumper pigtails to isolate the tailgate release control circuits and will install a new tailgate handle release switch, free of charge. The Daytime Running Lights (DRL) may not dim when the headlight switch is moved into the "On" position. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 108, "Lamps, Reflective Devices, and Associated Equipment."

Consequence & remedy

Consequence: DRL that cannot dim may reduce the visibility of other drivers, increasing the risk of a crash.

The Office of Defect Investigations (ODI) has received 27 complaints alleging diesel fuel leaks due to a fracture in the secondary fuel filter housing in model year (MY) 2015-2021 Ford Motor Company (Ford) F-250, 350, 450, and 550 Super Duty vehicles equipped with 6.7L diesel engines. Of these complaints, 12 describe an engine compartment fire. The proximity of the secondary fuel filter to high heat sources creates a potential for autoignition thermal events and fires. By letter dated December 4, 2019, Ford Motor Company (Ford) notified the Office of Defects Investigation (ODI) of a safety defect that may exist in model years (MY) 2017 through 2019 Ford Super Duty F-250, F-350, F-450 vehicles equipped with an electric tailgate. Following the receipt of five consumer complaints alleging unintended tailgate opening without vehicle operator knowledge, while the vehicles were in motion, NHTSA opened this investigation to assess the manufacturer's data including warranty claims, field reports, design changes, etc.In vehicles equipped with an electric tailgate, water can enter the electrical wiring system and may cause a short circuit resulting in the unintended activation and release of the tailgate latch. This could allow unintended opening of the tailgate while the vehicle is in motion.Analysis of the data provided by Ford found that the frequency of a tailgate opening unintended was high in vehicles equipped with electric tailgates. Unintended tailgate opening without vehicle operator knowledge may result in loss of unrestrained cargo, increasing the risk of a crash. In response to this investigation, Ford decided to conduct recall 19V-864.Owners will be notified by mail and instructed to take their vehicle to a Ford dealer to modify the tailgate/frame wiring harnesses by adding jumper pigtails to isolate the tailgate release control circuits, and will install a new tailgate handle release switch. With recall action 19V-864 taken by Ford Motor Company this investigation is closed.
