Component Failed & Availability: The engine on my 2015 Toyota Sienna (99,000 miles) catastrophically failed and is completely seized. The vehicle and engine are in my possession and are fully available for physical inspection. Safety Risk: The vehicle shut down completely while in motion, causing an immediate loss of motive power, power steering, and power braking assistance. Had this un-warned seizure occurred at highway speeds rather than a parking lot, the sudden loss of vehicle control would have posed a severe crash hazard. Reproduced/Confirmed: Yes. A certified mechanic removed the serpentine belt, attempted to turn the engine with a lock bar, and confirmed the engine block is completely seized. The local dealership, Fred Haas ToyotaWorld, also confirmed the vehicle does not run. Inspections: The vehicle was inspected by my independent mechanic and the local dealership. However, Toyota Corporate formally refused my request to dispatch a field engineer to inspect the defective engine. Prior Warnings: There were absolutely no warning lamps, check engine lights, temperature spikes, or abnormal noises prior to the failure. The engine seized instantly and without warning.

I am submitting this complaint to request that the National Highway Traffic Safety Administration (NHTSA) document and evaluate what may be a broader defect affecting certain Toyota Sienna vehicles and determine whether it presents a potential safety-related defect trend. My 2015 Toyota Sienna was diagnosed by an authorized Toyota dealership with an external rear-bank head gasket leak. A head gasket is a critical engine component, and failure can result in coolant loss, engine overheating, reduced engine performance, stalling, or complete vehicle disablement. These conditions may create a safety risk, particularly if they occur while driving in traffic or at highway speeds. An authorized Toyota dealership confirmed the diagnosis and submitted a goodwill assistance request to Toyota. The dealership noted my history as a repeat Toyota customer and recommended that assistance be provided. Toyota denied the goodwill request based on the vehicle's age and mileage. I subsequently appealed through Toyota's customer relations process, and the denial was upheld. I understand that NHTSA does not resolve individual warranty or customer-assistance disputes. My purpose in submitting this complaint is to request that NHTSA determine whether similar reports exist involving premature head gasket failures in this model year or related vehicles and, if warranted, evaluate whether a broader defect trend exists that could present a safety concern. Potential safety consequences include engine overheating, loss of power, stalling, and unexpected vehicle disablement. The vehicle has been maintained in accordance with manufacturer recommendations and has not experienced any overheating event that would reasonably explain a premature head gasket failure. The contact owns a 2015 Toyota Sienna. The contact stated while starting the vehicle he noticed that the vehicle was shuddering and shaking uncontrollably. The check engine warning light was illuminated. The contact attempted to restart the vehicle however, the failure persisted. The contact took the vehicle to an independent mechanic to be diagnosed. The contact was informed that the failure was due to low cylinder pressure on cylinder #6. The mechanic was not able to do the repair on the vehicle, so the contact took the vehicle to the local dealer who diagnosed and confirmed the failure as a misfire and low-pressure cylinder #6 and that the spark plug was damaged. The vehicle was repaired however, the failure persisted. The dealer recommended that the engine be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 87,000.

Vantage Mobility International, LLC (Vantage) is recalling certain 2012-2018 AMS Genesis wheelchair vans, built on a Toyota Sienna vehicles. The rubber brake hoses that attach to the rear wheel assembly may be too short, causing them to rub against the rear axle trailing arm.

Consequence & remedy

Consequence: The contact may cause failure of the rear brake hoses, resulting in a loss of vehicle control and reduced braking ability, increasing the risk of a crash.

Remedy: Vantage will notify owners, and dealers will install a set of rear brake hoses that will provide the proper clearance, free of charge. The recall began December 16, 2019. Owners may contact Vantage customer service at 1-800-488-9082.

Braun Corporation (Braun) is recalling certain 2014-2019 Toyota Sienna mobility van conversions. The brake lines may have incorrect tube nut fittings installed, which may cause a pressure drop within the braking system.

Consequence & remedy

Consequence: The reduction in the braking system pressure may cause a loss of braking power in the rear braking system, increasing the risk of crash.

Remedy: Braun has notified owners, and dealers will install an adaptor to correct the issue, free of charge. The recall began June 25, 2019. Owners may contact Braun customer service at 1-800-488-0359.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2011-2016 Toyota Sienna minivans manufactured January 4, 2010, to August 12, 2016. If the power sliding door is unable to be opened when commanded, such as if the door is frozen shut, the door may subsequently open unexpectedly, possibly while the vehicle is moving.

Consequence & remedy

Consequence: If door opens while the vehicle is moving, there would be an increased risk of injury to the vehicle occupants.

Remedy: Toyota will notify owners, and dealers will replace the junction block, rear sliding door wire harnesses, and install two sub wire harnesses, free of charge. The recall began on July 28, 2017. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is G04.

(GST) is recalling certain model year 2015 Toyota Sienna vehicles manufactured January 23, 2015, to April 13, 2015, and equipped with non-Toyota-brand overhead entertainment accessories installed by GST. During reassembly of the interior after the overhead entertainment system installation, a trim panel securing clip may have been reused instead of being replaced.

Consequence & remedy

Consequence: If the clip was damaged during the entertainment system installation, in the event of a side curtain air bag deployment, the trim panel may detach and strike one of the vehicle occupants, increasing the risk of injury.

Remedy: GST will notify owners, and dealers will replace the trim panel securing clip, free of charge. The recall began June 5, 2015. Owners may contact GST customer service at 1-800-444-1074. GST's number for this recall is 15R1.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2015 Sienna vehicles manufactured September 2, 2014, to November 25, 2014. In the event of a crash that necessitates the deployment of the curtain shield airbag (CSA), the second row overhead assist grips may detach.

Consequence & remedy

Consequence: If the assist grip detaches due to CSA deployment, the grip may strike a vehicle occupant, increasing the risk of injury.

Remedy: Toyota will notify owners, and dealers will modify the headliner near the second row overhead assist grip, free of charge. The recall began on January 15, 2015. Owners may contact Toyota customer service at 1-800-331-4331.
