WHEN I TRY TO BRAKE, OCCASIONALLY, THE CAR JUMPS FORWARD, INSTEAD OF STOPPING. IT MOVES LIKE 6 INCHES TO 12 INCHES. IT HAPPENS MORE OFTEN WHEN I TRY TO BRAKE AT A STOP SIGN, WHEN THE ROAD IS ROUGH, OR THERE IS A POTHOLE. IT GIVES A FEELING OF THAT CAR ACTUALLY ACCELERATED INSTEAD OF SLOWING, AND FEELS UNSAFE. *TR

WHILE ON VACATION ON AUG 3, 2013, WE WERE DRIVING IN THE SMOKY MOUNTAINS IN GATLINBURG, TN, VIA THE SCENIC LOOP DOWN FROM CADES COVE. I WAS DRIVING AND MY HUSBAND, OUR 13 YEAR OLD SON (ONLY CHILD) AND HIS 13 YR. OLD BUDDY (MY FRIEND'S ONLY CHILD) WERE THE PASSENGERS. THE ROUTE WAS A STEEP MOUNTAINSIDE WITH TYPICAL WINDING MOUNTAIN ROADS, PITCH DARK AT ~8:30 PM. SUDDENLY, BOTH HEADLIGHTS WENT OUT, TOGETHER, AT THE SAME TIME. THE DASHBOARD SHOWED THAT THE HEADLIGHTS WERE ON, NO WARNING SIGNS CAME ON. I HAD DRIVEN THROUGH A TUNNEL A BIT EARLIER AND THE HEADLIGHTS WERE BOTH WORKING. PANICKED, I PULLED OVER AT THE NEXT LOOKOUT LEDGE, TURNED THE CAR'S POWER OFF AND ON SEVERAL TIMES, TURNED THE LIGHTS OFF AND ON SEVERAL TIMES WHILE THE CAR WAS RUNNING - NOTHING. NO CELLPHONE SERVICE WAS AVAILABLE IN THE AREA. AS I WAS HYSTERICAL AND TERRIFIED BEYOND MY WITS, MY HUSBAND TOOK OVER DRIVING DUTY AND DROVE WITH THE EMERGENCY FLASHERS ON. AFTER A FEW MILES OF SHEER PANIC AND INCOHERENCE, IT OCCURRED TO US TO TRY THE HIGH BEAM, WHICH FORTUNATELY WORKED AND WE ARE ALIVE TO TELL THE STORY TODAY. AS SOON AS WE GOT CELLPHONE SIGNAL WE LEFT MESSAGES FOR OUR FRIENDS IN CASE WE DIED THEY WOULD KNOW WHY. GET A GRIP, TOYOTA! I LOVED MY PRIUS - THIS IS MY 2ND PRIUS, BUT I AM NOW PETRIFIED TO STEP IN THIS CAR AGAIN. I FOUND THOUSANDS OF SIMILAR COMPLAINTS ONLINE REGARDING BOTH THE PRIUS HEADLIGHTS FAILING AT ONCE, SO TOYOTA YOU KNOW I AM NOT AN ISOLATED CASE. HORRIBLE, DANGEROUS PROBLEM WITH THE 2010 PRIUS, YOU ARE RISKING LIVES! *TR

AT ABOUT 125,000 MILES, THE ENGINE WARNED ME OF LOW OIL. IT WAS THE FIRST SUCH WARNING EVER WITH THIS VEHICLE. AT THAT TIME I COULD SECURE ONLY 2 QUARTS OF THE REQUIRED SYNTHETIC OIL AND IT WAS STILL OVER 1 QUART LOW. ABOUT 500 MILES LATER I TOOK THE VEHICLE TO TOYOTA CHULA VISTA AND EXPLAINED. THEY ADDED 3 MORE QUARTS OF OIL AND SUGGESTED A SERIES OF OIL CONSUMPTION TESTS. I CALLED TOYOTA MOTOR USA CONSUMER RELATIONS AND OPENED A CASE INDICATING THAT I WAS CONSUMING (BURNING NOT DRIPPING) 1 TO 2 QUARTS PER 1,000 MILES AND MY MPG HAD DECREASED FROM 46 AVG TO 35 AVG. THE OIL CONSUMPTION OVER THE NEXT 3 MONTHS AVERAGED 1 QT/600 MILES AND MY AVG MPG DETERIORATED FURTHER TO 29 MPG. IN JULY TOYOTA CHULA VISTA PERFORMED A TEARDOWN AND ENDED UP REPLACING THE LOWER ENGINE, CYLINDERS RINGS, AND VALVES. I WAS INFORMED THAT OTHER 2010 TOYOTA PRIUS ENGINES EXHIBITED FAILURE/WEAR OF THE CYLINDER WALLS AT ABOUT THE SAME MILEAGE. FIELD TECHS WERE CONTACTED AGAIN BY THE CHULA VISTA SERVICE MGR AND I CONTACTED TOYOTA NATIONAL SEEKING SOME ASSISTANCE IN THE $6,400 REPAIR BILL. BOTH LOCAL FIELD REP AND NATIONAL DECLINED THOSE REQUESTS. WHAT GOOD IS A 10 YEAR HYBRID BATTERY WITH A 3 YEAR ENGINE? *TR

THIS HAS HAPPENED ATLEAST 4 TIMES IN THE LAST YEAR, WHERE THE CAR HAS LOST COMPLETE BRAKING POWER AND AT THE TIME THIS HAPPENS, ALL THE LIGHTS ON THE DASHBOARD LIGHT UP FOR A FEW SECONDS AND ON SUBSEQUENT PUMPING OF THE BRAKE PEDAL, THE BRAKE POWER RETURNS TO NORMAL. ON ONE OCCASION I ALMOST ENDED UP IN THE MIDDLE OF AN INTERSECTION, THANKFULLY THERE WAS NO ONCOMING TRAFFIC AND WAS ABLE TO RECOVER WITHOUT GETTING CRASHED INTO. I SEE A LOT OF SIMILAR COMPLAINTS HERE AND ON TOYOTA'S OWN COMMUNITY PORTAL DESCRIBING THE EXACT SAME BEHAVIOR. IN ALL CASES, THE DEALERSHIP HAS ENDED UP REPLACING THE BRAKE BOOSTER ASSEMBLY. IN MY CASE THOUGH, THE DEALER CONFIRMED THAT MY CAR WAS ON THE RECALL LIST FOR THE BRAKE BOOSTER ASSEMBLY, BUT WHEN THEY RAN SOME DIAGNOSTICS TESTS ADVISED BY TOYOTA WHICH DID NOT REPORT ANY ISSUES AND THEREFORE REFUSED TO CHANGE THE BRAKE BOOSTER ASSEMBLY. I ALSO CONTACTED TOYOTA (CASE# [XXX]) ONLY TO BE TOLD A WISHY WASHY ANSWER. I WAS TOLD THAT TOYOTA CANNOT DO ANYTHING ABOUT IT SINCE IT HAS GIVEN GUIDELINES TO THE DEALERSHIP AND IF THE DEALERSHIP SAYS "NO" THEN SO DOES "TOYOTA". NOT TO MENTION HAVING SPENT HOURS ON THE PHONE WITH TOYOTA ONLY TO BE TOSSED SEVERAL TIMES BETWEEN DIFFERENT SUPPORTS REPS AND THE DEALERSHIP. ADDITIONALLY, I WAS TOLD BY THE DEALERSHIP TO KEEP DRIVING THE CAR AND SEE IF IT WAS TO HAPPEN MORE OFTEN. ONCE IT GETS TO A POINT WHERE I CAN FIND A PATTERN AND REPRODUCE THE ERROR WITH THE TOYOTA REP THAT'S THEN THEY WILL LOOK INTO THE ISSUE. FOR SOME REASON THEY FAIL TO UNDERSTAND THAT I AM NOT COMPLAINING ABOUT MY WIPER BLADES NOT WORKING BUT THE BRAKES, WHICH COULD PUT THE DIVER IN A LIFE THREATENING SITUATION. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6). *TR

On September 19, 2019, the Office of Defects Investigation (ODI) received a defect petition from Mr. Roger Hogan requesting that the Agency investigate and recall certain Toyota Prius, Prius PHV, Camry Hybrid and Avalon Hybrid vehicles for a brake actuator valve wear condition covered under Toyota Customer Support Programs ZJB and ZKK (subject CSP?s). In support of his request, the petitioner identified 117 NHTSA complaints allegedly related to the subject CSP condition, including 60 reporting crashes. On September 30, 2019, ODI opened Defect Petition DP19-004 to evaluate the petitioner?s request for an investigation. In August 2020, the petitioner notified the Agency that he was withdrawing his petition. Because the petitioner has withdrawn the petition, we are denying the petition as moot and no further analysis of this petition is necessary.NHTSA is authorized to issue an order requiring notification and remedy of a defect if the Agency?s investigation shows a defect in the design, construction, or performance of a motor vehicle that presents an unreasonable risk to safety. 49 U.S.C. ?? 30102(a)(9), 30118. Because the petitioner has with withdrawn the petition, the petition is denied as moot. This action does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency notes that it is not required to receive a defect petition prior to opening a defect investigation and will take further action if warranted by future circumstances.

The subject vehicles are the first model year of a new design generation of the Prius model line. The vehicles are powered by a hybrid drive system that uses both a regenerative braking system, which converts vehicle momentum into electrical energy to be stored in onboard batteries, and a hydraulic brake system more typical of the brake system used on non-hybrid vehicles. As is the case for other similarly designed hybrid vehicles the interaction and management of the two braking systems during ABS braking is complex and requires a sophisticated control system. In a February 9, 2010 Defect Information Report, Toyota advised the agency of its plans to conduct a safety recall to address inconsistent brake feel and increased stopping distances that may occur during braking events requiring ABS operation, such as would occur when braking on rough or low friction road surfaces.Toyota will reprogram the ABS control unit with improved software that addresses this condition.Based on Toyota's action the investigation is closed. ODI will continue to monitor the subject vehicles' performance and will take further action as future circumstances dictate.
