THE CONTACT MENTIONED THAT THE FAILURE RECURRED AFTER TRAVELING 15 MPH AND THE VEHICLE CRASHED INTO THE REAR END OF ANOTHER VEHICLE. A POLICE REPORT WAS NOT FILED AND NO INJURIES WERE REPORTED.
