This report is an addendum to report 11762283 that was made yesterday. The vehicle allows driving with the rear liftgate ajar, however under some circumstances it engages the EPB when the vehicle comes to a stop while the gear state is still Drive. The gear state of the car remains in Drive when the EPB engages. This presents the driver with the choice, in the middle of the road and possibly in the middle of an intersection, of either exiting the vehicle and rearranging contents of the vehicle to allow the rear liftgate to close, or activating the accelerator to continue driving with the traction motor overpowering clamped EPB. Neither option is safe. I have attempted to reproduce this unsafe state of the vehicle and was successful in one case and unsuccessful in another. My hypothesis for the differentiating factor is the top speed reached prior to decelerating to a stop: in the first case, 13mph, and in the second, 15mph. I have attached a single frame from one of two videos, of the successful case. The instrument display shows gear state is Drive, the car is moving, high torque is being applied by the traction motors (but typically the screen center shows ADAS state, and there is no indication of instantaneous traction torque), and the EPB is engaged. The car is in the middle of an intersection at the time.

After coming to a stop at a stop sign, and while the gear state was in Drive, the vehicle auto-applied the electronic parking brake because the tailgate was ajar. Gear state remained in Drive. Upon accelerator input, the parking brake did not release, and the vehicle delivered propulsion torque overcoming the parking brake. As a result, the vehicle accelerated and drove several blocks with the rear brakes fully clamped, which was only effectively indicated by a smell and smoke emitted from the rear wheel wells. An irritating repetitive audio and large flashing visual alert in the display cluster indicating the tailgate state distracted from any additional indications that may have been displayed. That distracting indication was continuously active from when gear state was first switched to Drive and did not correlate with EPB engagement.

I was coasting into a library parking space with my foot off the accelerator. The car accelerated even with my foot on the brake, jumped a curb and was stopped with above ground tree roots. The dashboard should have been lighted up with collision warning indicators and the automatic braking should have engaged. It did none of that. The car was totaled. Nothing was ever checked to see if it was a mechanical error. My insurer totaled the car.

This almost resulted in a car rear ending me due to the sudden deceleration. The following day, with clear weather, the same issue happened again despite nothing appearing to have caused this issue while I was driving. This is a very serious concern to suddenly stop on a highway.

On the morning of February 3rd, 2025, while driving, I heard a pop. A few seconds later, as I approached an intersection, a yellow warning appeared that instructed me to "Check the Electric Vehicle System". It minimized to a small caution light. I attempted to continue driving, pulling through the intersection; however, the vehicle had an abrupt loss of power; I was unable to proceed over 20-25 mph. In total no more than 2-3 mins passed from hearing the pop to loss of power. I pulled into a neighborhood, stopped the car, and consulted the owner's manual for direction on the warning light. While parked, and an estimated 8-10 minutes after hearing the first pop, I received a second warning, this time in red. It said "Turn power off immediately. Check power supply." I turned car off/contacted roadside, as advised in my manual, for a tow. After review at the dealership, I was originally told I needed a new 12V that it had died while I was driving. I authorized the replacement, though I also indicated to the service department that if that CAN happen, it really does not make sense AND there should be a different warning indicator for a failed 12v that could be replaced roadside versus instructing for a tow.However, after replacing my 12v the service advisor indicated that the car pulled codes and that the ICCU and its fuse seemed to have failed, which likely caused the damage to the 12v battery. After a few days of testing, my car was cleared for replacement of the failed ICCU by Hyundai's techline on February 10th. The dealer then provided a vehicle for my use while I await repairs. I was told a part could arrive the 18th or the 25th and the repair could take 3-4 days once the parts are received. It is still in shop with no updates. On 2/13/25 I filed a complaint to Hyundai; I have not received a response to date. All recalls for these issues had been performed on my vehicle in November 2024; we can no longer, in good conscience, use this vehicle. It is unsafe.

While driving, the 'check EV system' light comes on, intermittently what will happen is that when stopped the car will put itself in park, when it is in park I can not get it out of park, thus unable to move forward, it has also happened while reversing (and not stopped). I need to cycle the car on and off a bunch of times in order for it to finally turn back on. This is very dangerous while on the road stopped in a busy intersection and has potential for a crash. I was driving 75mph on the freeway and I heard a pop and then the electrical warning system on the car lit up. I lost power to the car and was only able to accelerate minimally. There was a warning of low battery and the vehicle was going to lose power. I turned on my hazard lights and was able to get to the exit and was able to exit the freeway safely. Once the vehicle lost power I was unable to open the charger door, start the vehicle, use any of the key fob functions and had to jump the 12V battery, only to die immediately after. There is no pre-warning to this happening, it just shows the alert and the car loses all power. There are also problems with the vehicle after being plugged in to charge and it says "Charging Unsuccessful", or stops charging after a short period of time. It also will not charge to full capacity at home or the charging station. I receive no warning lights or messages about why the charge is not working. It took weeks to get the vehicle into the dealer, making it extremely unsafe to drive and the error has almost caused a major crash. My vehicle has been at the dealer and has not been diagnosed with any issues yet. The dealer has provided me with a rental vehicle...in which now I'm paying for gas. Reading through these other comments...who knows how long it will be to get my vehicle back, yet I'm still making my payments...this isn't

The Integrated Charging Control Unit (ICCU) may become damaged and stop charging the 12-volt battery, which can result in a loss of drive power.
Consequence & remedy
Consequence: A loss of drive power increases the risk of a crash.
Remedy: Dealers will inspect and replace the ICCU and its fuse, as necessary. In addition, dealers will update the ICCU software. All repairs will be performed free of charge. Owner notification letters were mailed December 20, 2024. Owners may contact Hyundai customer service at 1-855-371-9460 or Genesis customer service at 1-844-340-9741. Hyundai's numbers for this recall are 272 (Hyundai) and 025G (Genesis). This recall expands and replaces previous recall number 24V-204. Hyundai Motor America (Hyundai) is recalling certain 2022 Ioniq 5 vehicles. A software error in the Shifter Control Unit (SCU) may disengage the parking mechanism, which can allow the vehicle to rollaway.
Consequence & remedy
Consequence: Vehicle rollaway can increase the risk of a crash or injury.
Remedy: Drivers are recommended to use the Electronic Parking Brake (EPB) when parking their vehicles until the remedy has been performed. Dealers will update the Shifter Control Unit software, free of charge. Owner notification letters were mailed July 14, 2022. Owners may contact Hyundai customer service at 1-855-371-9460. Hyundai's number for this recall is 228.

On October 3, 2024, the Office of Defects Investigations (ODI) opened PE24028 to investigate alleged failures of Enel X Way's JuiceBox Level 2 residential EV chargers. The allegations were described as thermal incidents and electrical malfunctions, which could trip the circuit breakers in Juicebox owners’ homes and disrupt electric vehicle charge cycles. On October 2, 2024, Enel X Way posted on its website that Enel X Way North America would cease operations in the United States and Canada on October 11, 2024. The closure would potentially impact the charging app and supporting software. ODI sent Enel X Way an Information Request (IR) letter on October 21, 2024. Enel X Way provided an incomplete response to the IR on December 9, 2024. Enel X Way later provided a complete response to the IR on January 24, 2025. ODI reviewed Enel X Way’s IR submissions to determine whether further action is required. Based on ODI’s analysis, there was no clear trend in the cause of the failures described in customer complaints and warranty reports. Additionally, there have been no recent reports alleging safety issues. The available information does not warrant further action at this time. The closing of this investigation does not constitute a finding by NHTSA that a safety-related defect does not exist. The Agency will continue to monitor for any safety trends and reserves the right to take additional action if warranted by future circumstances. On June 8, 2023, the Office of Defect Investigation (ODI) opened PE23011 to investigate allegations of loss of motor power on model year (MY) 2022 Hyundai Ioniq 5 vehicles. Some consumers reported loss of motor power immediately after a warning lamp illumination or reduction of motive power while the vehicle was in motion. This investigation found that the Integrated Charging Control Unit (ICCU) may be subject to various conditions such as overcurrent, overvoltage induced at the end of charging, and certain thermal loading during operation that can damage its internal components and open the ICCU fuse, resulting in an inability to charge the 12-volt battery. The vehicle could eventually lose all motive power, increasing the risk of a crash. In July 2023, Hyundai issued a service campaign to address certain MY 2022-2023 Ioniq 5 and MY 2023 Ioniq 6 vehicles that may have low 12V auxiliary battery charging occurrences due to an ICCU fault and may set the DTC P1A9096 “DC/DC Converter Input Voltage Sensor Fault”. On March 15, 2024, Hyundai filed Safety Recall 24V-204 for 98,878 MY 2022-2024 Hyundai Ioniq 5 vehicles and MY 2023-2024 Hyundai Ioniq 6, Genesis GV60, Genesis GV70 “Electrified” and Genesis G80 “Electrified” vehicles. The recall remedy provides an inspection, and if necessary, a replacement of the ICCU and the associated fuse. The ICCU software reduces the risk of overcurrent and implements voltage peak reduction at the end of EV battery charging. The recall also includes a software update that revises the electric water pump operational threshold to reduce thermal loading during charging and driving. In view of the recall action being taken by Hyundai, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
