We are reporting a suspected thermal runaway incident involving our recently purchased used 2023 Hyundai Ioniq 5 and have significant concerns regarding the vehicle's safety. **Vehicle Information** * Purchased XXX, 36,662 miles * Clean inspection and no accidents per Carfax **Incident Timeline – XXX** Between 6:00 and 6:30 PM, while traveling northbound on [XXX] just past the [XXX] exit in [XXX], the vehicle struck a white piece of road debris that appeared flat, similar to Styrofoam (video available). There was no immediate loss of vehicle function. [XXX] The vehicle was then moved to the driveway for inspection after a low coolant warning appeared. Damage to the underside near the driver's side and observed blue coolant leaking from the area. The coolant reservoir under the hood was empty. Based on online guidance advising against driving a vehicle with a coolant leak, we decided to leave the vehicle parked in our garage overnight and arrange a flatbed tow to a Hyundai dealership the following day. The garage door was closed, and the vehicle was **not charging**. At approximately 8:10 PM, a burning smell was detected inside the home. Upon opening the door between the house and garage, smoke was observed. The family immediately evacuated, and XXX was called at 8:12 PM. The fire department arrived at approximately 8:18 PM and found the garage filled with smoke. At 8:30 PM, firefighters removed the vehicle from the garage. At 8:35 PM, flames were observed beneath the vehicle. Firefighters applied water directly to the fire until 11:00 PM, after which the vehicle was removed. [XXX] [XXX] [XXX] Manufacturer was contacted; denies any liability due to debris. However, we believe inadequate safety design contributed, including soft protection shield and poor location of coolant. The vehicle repeatedly displays a "Stop Vehicle and Check Brake System" warning together with an ABS warning, accompanied by a loss of normal braking response. On XXX, while driving at approximately 60 mph on I-85 northbound near Exit XXX in [XXX], the warnings appeared and the brakes did not respond normally. I was maintaining a substantial following distance, and the braking system restored itself before I reached the point of needing to brake hard, so no crash or injury occurred. Had hard braking been required in that window, I do not believe the vehicle would have stopped as commanded. This is a recurring fault. The vehicle, purchased certified pre-owned and still under the Hyundai factory warranty, has been taken to the dealer (Ed Voyles Hyundai, [XXX]) three times for the same condition. On the first visit the Integrated Electronic Brake (IEB) unit was replaced; on the second, the Smart Junction Box was replaced; on the third, the dealer removed an aftermarket tracking device wired to the data link connector and referenced Hyundai TSB 24-GI-011H, representing the problem as corrected. The braking-loss event on XXX occurred after that repair. The vehicle has been out of service approximately 30 days total. Because the failure is an intermittent loss of braking response at highway speed, I consider it a serious safety hazard and no longer drive the vehicle. The "Smart Smooth Cruise Control Conditions Not Met" warning message was displayed on the instrument panel. The contact restarted the vehicle as a temporary fix; however, the failure recurred. The vehicle was taken to a local dealer where it was diagnosed with an unknown electrical failure. The vehicle was repaired; however, the failure recurred. The vehicle was taken back to the same local dealer; however, the cause of the failure could not be determined. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 47,000.
Rear hatch window spontaneously shattered for no apparent reason. Car was parked in a Florida parking lot, no use of rear hatch and no use of rear defroster. I understand there is a potential class action lawsuit for the Ioniq 5 model years 2022-2025 for this problem.

The ioniq 5 highway driving assist, which steers the car and keeps it centered in the lane, will disengage without clear warning - the small green steering wheel icon (which is at the top of the dash cluster) simply becomes grey and blinks, but there's no audible chime to indicate that the car has disengaged the steering assist. I have had multiple times on the highway where it disengaged and I don't notice (because I am looking at the road, not the dash) and one time on the highway it disengaged during a turn and hit the rumble strip, causing me to swerve back onto the road. My hands were on the steering wheel and I was paying full attention, but I was expecting the car to handle the turn, and it just disengaged without me noticing. Had there been another car in the middle lane or I reacted slower, it's likely a crash would have occurred. I have a 2023 Hyundai Ioniq 5 Limited and had the ICCU "popped" (failed) which took about two weeks to replace. We regularly drive from from [XXX] to [XXX] were there are large section of [XXX] with no cell service. Meaning potentially walking for miles in extreme heat or cold. Hyundai right now seems only interested in continuously replacing the part rather than actually fixing the route cause, making it so the car is not trustworthy or safe to drive long distances. Hyundai Motor America (Hyundai) is recalling certain 2023-2024 Ioniq 5 vehicles. The high voltage battery cells may contain misaligned electrodes, which can lead to a fire while parked or driving.
Consequence & remedy
Consequence: A fire increases the risk of injury.
Remedy: Owners are advised to park outside and away from structures and limit their charge to a maximum of 80% until the recall repair is complete. Dealers will replace the high voltage battery system assembly, free of charge. Owner notification letters are expected to be mailed August 31, 2026. Owners may contact Hyundai's customer service at 855-371-9460. Hyundai's number for this recall is 305. The Integrated Charging Control Unit (ICCU) may become damaged and stop charging the 12-volt battery, which can result in a loss of drive power.
Consequence & remedy
Consequence: A loss of drive power increases the risk of a crash.
Remedy: Dealers will inspect and replace the ICCU and its fuse, as necessary. In addition, dealers will update the ICCU software. All repairs will be performed free of charge. Owner notification letters were mailed December 20, 2024. Owners may contact Hyundai customer service at 1-855-371-9460 or Genesis customer service at 1-844-340-9741. Hyundai's numbers for this recall are 272 (Hyundai) and 025G (Genesis). This recall expands and replaces previous recall number 24V-204. Hyundai Motor America (Hyundai) is recalling certain 2023 IONIQ 5, IONIQ 6, and Genesis GV60 vehicles. The rear inner driveshaft may have been improperly heat-treated, allowing it to break under load and resulting in a loss of drive power.
Consequence & remedy
Consequence: A loss of drive power increases the risk of a crash.
Remedy: Dealers will replace the rear inner driveshaft, free of charge. Owner notification letters were mailed March 29, 2024. Owners may contact Hyundai customer service at 1-855-371-9460. Hyundai's numbers for this recall are 253(H), 017G(G).

On June 8, 2023, the Office of Defect Investigation (ODI) opened PE23011 to investigate allegations of loss of motor power on model year (MY) 2022 Hyundai Ioniq 5 vehicles. Some consumers reported loss of motor power immediately after a warning lamp illumination or reduction of motive power while the vehicle was in motion. This investigation found that the Integrated Charging Control Unit (ICCU) may be subject to various conditions such as overcurrent, overvoltage induced at the end of charging, and certain thermal loading during operation that can damage its internal components and open the ICCU fuse, resulting in an inability to charge the 12-volt battery. The vehicle could eventually lose all motive power, increasing the risk of a crash. In July 2023, Hyundai issued a service campaign to address certain MY 2022-2023 Ioniq 5 and MY 2023 Ioniq 6 vehicles that may have low 12V auxiliary battery charging occurrences due to an ICCU fault and may set the DTC P1A9096 “DC/DC Converter Input Voltage Sensor Fault”. On March 15, 2024, Hyundai filed Safety Recall 24V-204 for 98,878 MY 2022-2024 Hyundai Ioniq 5 vehicles and MY 2023-2024 Hyundai Ioniq 6, Genesis GV60, Genesis GV70 “Electrified” and Genesis G80 “Electrified” vehicles. The recall remedy provides an inspection, and if necessary, a replacement of the ICCU and the associated fuse. The ICCU software reduces the risk of overcurrent and implements voltage peak reduction at the end of EV battery charging. The recall also includes a software update that revises the electric water pump operational threshold to reduce thermal loading during charging and driving. In view of the recall action being taken by Hyundai, ODI is closing this Preliminary Evaluation. The agency reserves the right to take additional action if warranted by future circumstances.
