ON JULY 19, 2020 ~3:30P, I WAS TRAVELING ON CA 134 W TRANSITIONING TO CA 101 N. AS I APPLIED THE BRAKES THE PEDAL WENT STRAIGHT TO THE FLOOR WITH NO BRAKING EFFECT BEING INITIATED. VIGOROUSLY PUMPING THE BRAKES RESULTED IN VIRTUALLY NO BRAKING EFFECT. TURNING THE EMERGENCY FLASHERS ON, I THEN ALLOWED THE VEHICLE TO COAST TO A STOP ON THE SHOULDER OF THE CA 101 NORTH AND AWAITED FOR ROADSIDE ASSISTANCE. THERE WAS NO PREVIOUS BRAKING PERFORMANCE ISSUE, ANNUNCIATOR WARNING, OR ALARM OF ANY FORM TO SUGGEST A PENDING ISSUE. ONLY UPON THE ACTUAL FAILURE ITSELF DID A DISPLAY MESSAGE SHOW UP ON THE DASH. THAT MESSAGE READ SOMETHING LIKE "BRAKE FLUID LOW". AS I WAITED FOR ROADSIDE ASSISTANCE, I TOOK A MOMENT TO INSPECT THE AREA UNDER THE VEHICLE LOOKING FOR EVIDENCE OF ANY DRIPPING OR LEAKING FLUIDS. THERE WAS NONE AND THE VEHICLE WAS OTHERWISE WELL MAINTAINED WITH APPROXIMATELY 60,000 MILES ON IT. I CONTACTED AAA FOR AN EMERGENCY TOW. WHEN THE AAA FLATBED TRUCK ARRIVED, THE DRIVER, BILL, ASKED THE NATURE OF THE PROBLEM AND I INFORMED HIM IT WAS A VIRTUAL 100% BRAKE FAILURE. AFTER HE LOADED THE TRUCK ONTO THE FLAT BED, HE NOTED WITH SURPRISE THAT THE VEHICLE HAD INDEED LOST 100% OF ITS BRAKES. AAA TOWED THE VEHICLE TO THEIR NEAREST SERVICE CENTER LOCATED AT 5451 TUJUNGA AVE, NORTH HOLLYWOOD (LOS ANGELES), CA. WHEN THE THERE, DAN, THE TECHNICIAN/OWNER IN CHARGE, SPOKE WITH ME AFTER HIS INITIAL INSPECTION, HE WAS SURPRISED TO FIND THE VEHICLE HAD 100% BRAKE FAILURE WITH NO EVIDENCE OF ANY EXTERNAL LEAKS. HE DID NOTE THAT THE BRAKE FLUID RESERVOIR WAS EMPTY BUT THERE WAS NO LEAKS. HE SURMISED THAT THE FLUID MUST HAVE LEAKED "INTERNALLY" AND THEREAFTER INDICATED HE WOULD DO NO WORK ON THE VEHICLE, BELIEVING THAT FORD NEEDED TO ANALYZE THE FAILURE. MY 91 YEAR OLD PASSENGER AND I WERE LUCKY TO HAVE NOT BEEN THE CAUSE OF A MAJOR HIGHWAY ACCIDENT.

AFTER STOPPING TO PICK UP MY GRANDSON WHEN I WENT TO BACK-UP I NOTICED THE RIGHT SIDE REAR WINDOW HAD SHATTERED, HAVE NO IDEA WHY OTHER THAN MAYBE THE ELECTRIC DEFROST DID SOMETHING. LOOKED FOR SIGNS OF SOMETHING HITTING IT BUT COULDN'T FIND ANYTHING OR ANY SIGN THAT AN OBJECT HIT THE WINDOW. WHEN I WENT ONLINE TO SEE WHAT IT COST TO REPLACE I NOTICED THIS HAS BEEN HAPPENING ON FORD TRUCKS SO I FIGURED I NEED TO PUT MINE ON FILE ALSO.

VEHICLE SHUT DOWN AND WENT INTO "LIMP MODE" 1ST TIME IT HAPPENED WAS 10/16/19 WITH 44K MILES. DEALER SCANNED WITH NO CODES RETURNED SO THEY JUST REPROGRAMMED STEERING MODULE AND VEHICLE SEEMED FINE. IT STARTED HAPPENING AGAIN ON 06/25/20 WITH 47.5K MILES WHILE PULLING OUT ONTO MAIN ROAD ALMOST CAUSING ACCIDENT AND SEVERAL TIMES ON THE HIGHWAY AT 70 MPH ALMOST CAUSING ACCIDENTS. TOOK TO DEALER AND SCAN RETURNED ELECTRONIC THROTTLE BODY ERROR, SCAN CODE P2112. THROUGH ADVICE WE CONTACTED FORD CUSTOMER SERVICE AND THEY WERE UNABLE TO ASSIST IN THE REPAIRS EVEN THOUGH THEY HAD KNOWLEDGE AND ADMITTED THAT THIS IS A COMMON PROBLEM. FORD APPLIED AN EXTENDED WARRANTY FOR THE SAME PROBLEM TO SEVERAL MODELS UP TO 2013 VEHICLES, OUR IS A 2014. WE DECLINED REPAIRS AT THIS TIME SINCE WE DON'T DRIVE VEHICLE VERY OFTEN. FORD REP SAID TO SAVE OUR INVOICE FOR POSSIBLE REIMBURSEMENT IN THE FUTURE IF A RECALL WAS ISSUED. IF WE ELECTED TO HAVE THE REPAIRS PERFORMED AT A FORD DEALER, FORD CUSTOMER SERVICE OFFERED TO APPLY SOME SORT OF EXTENDED WARRANTY FOR US IN CASE OF FUTURE PROBLEMS. AFTER DRIVING THIS WEEKEND THE PROBLEM HAS NOT ONLY PERSISTED, BUT OCCURRED MORE FREQUENTLY THAN USUAL, MAINLY AT HIGH SPEEDS MAKING THIS SAFETY ISSUE EVEN MORE DANGEROUS. AT THIS POINT, WE HAVE NO OPTION BUT TO RETURN TO THE DEALER TO HAVE THE WORK DONE. WE STILL FEEL THAT THIS IN NOT A NORMAL WEAR AND TEAR ITEM. IT IS IN FACT A SAFETY ITEM THAT NEEDS TO BE ADDRESSED IMMEDIATELY.

MY TRUCK WOULD RANDOMLY HESITATE DURING THE ACCELERATION PHASE. EVENTUALLY IT LED TO ME BEING STRANDED DUE TO THE INABILITY OF THE VEHICLE TO START. TRUCKED WAS TOWED TO THE DEALERSHIP AND I WAS INFORMED THAT THIS WAS A REOCCURRING DESIGN PROBLEM IN THE ELECTRICAL SYSTEM THAT FORD HAS BEEN AWARE OF FOR QUITE SOMETIME. I URGE YOUR AGENCY TO STRONGLY CONSIDER A RECALL FOR NUMEROUS REASONS. 1. IT'S A DESIGN FLAW 2. WHILE I WASN'T AN ISSUE FOR ME NORMAL ACCELERATION CAN BE JUST AS IMPORTANT AS NORMAL BRAKING IN CERTAIN SITUATIONS DUE TO SAFETY CONSIDERATIONS.(MERGING, TURNING LEFT ACROSS TRAFFIC, ETC.) 3. IT WASN'T AN ISSUE FOR ME BUT MY SERVICE INVOICE INDICATED A SLIGHT BURN ON THE FUSE TERMINAL. OBVIOUSLY A POSSIBLE FIRE HAZARD DUE TO POOR DESIGN. THANK YOU FOR UPON CONSIDERATION.

RQ24005 · Unintended Transmission Downshift

Opened Mar 27, 2024 · Closed Jul 29, 2024

Status: closed (inferred from source dates) · Power Train:automatic Transmission:control Module (tcm/pcm/tecm)

In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24-005). The agency reserves the right to take additional action if warranted by future circumstances.
