A message will pop up on the dash "STOP SAFELY Vehicle Will Shut Off Soon" then the vehicle powers off. It will happen at different times: with cruise control on while on an interstate, on a highway or driving in town at speeds under 35mph. After researching online apparently this has been an issue with the Hybrid Pacifica for several years. Chrysler has not been any help to our local Chrysler dealer for a fix. With about 4000 miles on the van, I was driving down the freeway at 75 mph when the vehicle turned off, the check engine light illuminated, and all forward motive power was lost. Once stopped and off the road, the car was completely shut off. After a few minutes of waiting, the van was able to restart and be driven. This same loss of power occurred multiple times more in an effort to get the van to the dealership. The Power Invertor Module was replaced but the vehicle still experienced the loss of power. The next step was to completely replace the transmission. After 4000 more miles, the same issue occurred and the dealership once again replaced the transmission. After 7000 more miles, the loss of power occurred again and this time a wiring harness on the transmission was replaced. The very day we received the car back, it had the same loss of power, so it was decided by the dealership to completely replace the transmission again. The vehicle is on its fourth transmission and the van only has 17,000 miles on it. Every time this occurs, the check engine light comes on and gives an error code of "faulty Drive Motor A position sensor". The safety of me and my family is threatened every time there is a complete loss of power at highway speeds. The problem is reproducible and fully acknowledged and confirmed by the service staff. Over the span of 10 months this van has been in for service 5 times for a minimum of 7 days to over a month sometimes. Chrysler has not offered to buy back or replace the vehicle and has not offered a loaner until they can figure out a true repair.

Was driving on freeway with my family, including my one year old baby and four year old daughter in the back, when a warning message came on dashboard stating "Stop Safely Vehicle Will Shut Off Soon" and van suddenly died in the middle of the freeway. Had to immediately cross multiple lanes and was terrified I was going to get hit by a big rig or other vehicle passing by. The safety of my family was absolutely put at risk. Contacted Chrysler dealership immediately but told they did not have any available appointments and could not examine the van until Thursday, 07/21/2023. Took the van to local dealership on Thursday, 07/21/23. Next day received a call and was told that the transmission needs to be replaced. The van was new when we bought it in February 2022 and had less than 15K miles when this incident happened. Was told by dealership that it could be months before the van can be repaired and that no loaner cars were available but that I would be placed on a wait list for a loaner car. Instructed by dealer to contact Chrysler Cares, which I did. Was assigned a case number by Chrysler Cares and told that someone would get back to me within 48-72 business hours, but that in the meantime there was nothing that could be done about the fact that the one vehicle we rely on to transport our kids is in the shop.

I have submitted a report on this same vehicle following the initial incident on 5/12/23. I have not heard anything after I submitted this and would like to have some type of feedback. This report is an update on the vehicle with how it was handled at the dealership and company level. At this time our van is at a Chrysler dealership that is 3 hours away from where we live, as that is where the van broke down, and has been there for over two months. During this process we have verbally been told by the dealership that this is not the dealership's problem and it is a Chrysler problem. Chrysler has told us that they needed their engineers to investigate initially. After several weeks of us calling, they told us they were not able to fix it and were considering other options. Then, after having the van about one month they decided they needed to replace the hybrid generator/transimission. On 7/12/23 we became very pushy demanding information or we were going to get legal advice. At that time customer service told us that our vehicles information was confidential. On Friday 7/15/23 we were left a message that the van was ready and the dealership would contact us. On 7/18/23 we were left a message that stated they wanted to ensure we got the van and they wanted to close the case in two days. I am working on trying to get the dealership to ship the vehicle to us, as I do not have a vehicle to go get it. They have still given us no real information or assurances, paid for anything (rental vehicle for one week, hotel for one night, towing, etc). I am pretty sure that what happened is rare, but not knowing what it was and assuring they replaced a dysfunctional new part with another new part is not reassuring to us. I am not sure who reads this, what it does or whatever. I would like feedback, direction, advice, assistance of any kind. The whole incident was very scary and I would be surprised if it does not happen again, hopefully nothing bad happens.

Anohter issue on the same new 2022 minivan with 350 miles. on highway while driving at 68mph the CEL turns on and engine makes high revving sound, turtle light on above D, engine oil temp rises to 246 degrees, coolant rises to 220 deg, engine makes noises and sounds like revving high. Loss of power, vehicle slows to a dangerous speed and nearly hit by a pickup truck and trailer in the rear, pulled to side of road and limped to next rest stop. Went to a charging station at NY State rest stop and the electric charger does not have a plug for this veh. I shut car off and restarted, no tools to reset 12v system, restarted car and it ran but lights still on, felt like power was working so we chanced it and made it home with nearly normal power. This dangerous loss of power can cause someone else to mismatch speed and cause a rear end collision. This unsafe condition and defects need to be handled by Chrysler asap and recalled. This vehicle is a 12/2022 model purchased in June 2023. N HTSA need to recall - many comlpaints of same and similar issue. Also noticed a burning smell

RQ24004 · Loss Of Motive Power Due To An Internal Wiring Connector Short.

Opened Feb 27, 2024 · No close date supplied

Status: open (inferred from source dates) · Electrical System:wiring

On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV).  On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown.  An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.  In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008.  The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode.  The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010.  These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010.  The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down.  When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced.  Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010.  To review the ODI reports cited in the opening resume ODI report identification number document, go to NHTSA.gov.

PE22008 · Transmission Fault Leading To Loss Of Motive Power

Opened Jul 22, 2022 · Closed Mar 27, 2024

Status: closed (inferred from source dates) · Power Train

On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances. To review the ODI reports cited in the Closing Resume ODI Report Identification Number document, go to NHTSA.gov.
