With about 4000 miles on the van, I was driving down the freeway at 75 mph when the vehicle turned off, the check engine light illuminated, and all forward motive power was lost. Once stopped and off the road, the car was completely shut off. After a few minutes of waiting, the van was able to restart and be driven. This same loss of power occurred multiple times more in an effort to get the van to the dealership. The Power Invertor Module was replaced but the vehicle still experienced the loss of power. The next step was to completely replace the transmission. After 4000 more miles, the same issue occurred and the dealership once again replaced the transmission. After 7000 more miles, the loss of power occurred again and this time a wiring harness on the transmission was replaced. The very day we received the car back, it had the same loss of power, so it was decided by the dealership to completely replace the transmission again. The vehicle is on its fourth transmission and the van only has 17,000 miles on it. Every time this occurs, the check engine light comes on and gives an error code of "faulty Drive Motor A position sensor". The safety of me and my family is threatened every time there is a complete loss of power at highway speeds. The problem is reproducible and fully acknowledged and confirmed by the service staff. Over the span of 10 months this van has been in for service 5 times for a minimum of 7 days to over a month sometimes. Chrysler has not offered to buy back or replace the vehicle and has not offered a loaner until they can figure out a true repair.

Loss of power, vehicle slows to a dangerous speed and nearly hit by a pickup truck and trailer in the rear, pulled to side of road and limped to next rest stop. Went to a charging station at NY State rest stop and the electric charger does not have a plug for this veh. I shut car off and restarted, no tools to reset 12v system, restarted car and it ran but lights still on, felt like power was working so we chanced it and made it home with nearly normal power. This dangerous loss of power can cause someone else to mismatch speed and cause a rear end collision. This unsafe condition and defects need to be handled by Chrysler asap and recalled. This vehicle is a 12/2022 model purchased in June 2023. N HTSA need to recall - many comlpaints of same and similar issue. Also noticed a burning smell

Three (3) incidents between 36 miles and 110 miles on brand new minivan - beginning 6/10, 6/11 and today 6/14. The first time the vehicle was plugged in charging 120v l1 charger, my kids came to me and said the van is making noise all the time. The key was inside the house about 75 feet away. It sounded like the cooling fans were running, varying in speed every minute or so up to high speed, the dash was lit up with battery error, transmission error, and charging error messages, the red wrench with electric bolt was lit, maybe other messages. I unplugged everything, tried to shut off restart car from keyfob, the doors were left open, the start button did nothing, the shift knob was stuck in P and locked. Nothing would unlock the knob. It appeared to be a computer glitch, I removed the neg battery cable and reattached it and the car started and the messages cleared. The second time while driving the car lost power and stopped, same issue, luckily I had a pliers with me and I again rebooted the 12v system and it ran fine again. During both these events the dash always said "regen engine oil and fuel in progress" This time i tried to reset the oil life monitor, it made no diff to the regen message and I noticed the new hybrid car was not using any battery so it seems the engine was running all the time. The regen message remained for days until I decided to get gas, I read adding 4 gal might clear the message. I went to the gas station and again today 6/14 as I parked at the gas pump the car dies before I even got out to pump gas blocking the pumps, I quickly reboot, but the doors would not open, I crawled to the back and rebooted the 12v system and added 4 gals. When I restarted the car it ran normally again, then the regen message cleared and the electric motor began propelling the car as intended.

Additionally on the dashboard, the BRAKE (red warning light), the yellow engine light, the red turtle light displayed. The car quickly lost power and speed was quickly reduced to about 30 mph. The car continued to slow down and I was able to coast to the shoulder. I could have easily been injured or killed trying to get to the shoulder without any power from the engine. The transmission recall had been fixed the day prior. Car was towed to the dealership and needed a new transmission. Have been waiting for a new transmission for over 2 months.

While driving on highway on4/22/23 the vehicle displayed a message stating “stop safely, vehicle will shut off soon”. I lost the ability to accelerate and had to coast to the nearest shoulder. This happened during a high traffic period and I had children in the car. At the time there was no shoulder near by so I had to drive on the busy highway at very slow speeds until I could pull over. This was a very dangerous incident and I’m lucky I was not in an accident.

My 2022 Pacific Hybrid Limited stalled today while heading towards the Monterey Bay Aquarium. Luckily, I had enough momentum to make it to the shoulder. I've had it since late December and it has 3900 miles. It started with this whine for 10 miles prior to the stall, similar to the fake whine EV cars make to warn pedestrians. Then I hit a patch of traffic that dropped my speed to less than 10 mph. When traffic started to go again, the car shook and grumbled until it stopped accelerating all together. The check engine light blinked on. At that point I was maybe going 20mph and quickly started to go over two lanes (had to cut off a guy) but made it to the shoulder safely. Once we stopped, we all noticed the smell of something like burnt electronics. The car wouldn't move at the point. I tried turning the car off and on for the next 15 mins and still no go. The car also had <1% battery power (which we used up the battery maybe 20 minutes prior to the stall).

During the month of March, 2023, my vehicle completely shut down on two occasions, both of which occurred while traveling at highway speeds. There were dash icons that were lit, including a check engine light, a turtle icon and a message to Pull Over. The shutdown caused me to move quickly to the shoulder of the road, with limited ability to maneuver in a safe manner. These shutdowns occurred after the vehicle had its software updated based on Recall 03A (Loss of Motive Power). Apparently, the update only served to provide a warning on the dash that the engine was about to shut down (which occurred immediately). The dealer has had the vehicle for some time, and the latest update is that they are replacing the vehicle's transmission. It would seem unacceptable if, in fact, a (known?) issue that causes a shutdown was addressed by merely providing a millisecond's notice that the vehicle is going to completely die. This is an extremely unsafe situation, placing the motorist, their passengers and nearby vehicles in danger.

On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV). On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010. On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances.
