During the failure the "brake" waring message had also displayed. The contact indicated that the failure was intermittent and had only occurred while towing the boat. The local dealer was notified of the problem but no assistance was offered. The manufacturer was not yet contacted. The failure mileage was 1,000.

[XXX], [XXX] PM. 2024 Toyota Tundra Hybrid (i-FORCE MAX, V35A engine), 36k miles, original owner. Clear, dry pavement, evening daylight. Exiting [XXX] westbound at [XXX] , I merged into the far right lane at the second light for a right turn. A vehicle was stopped ahead at the light. Traveling 15-20 mph, I applied the brake for controlled deceleration. Right foot squarely on brake, not touching accelerator. Vehicle did not decelerate as expected. For 3-4 seconds the brake felt unresponsive — pedal felt soft and loose, lacking normal firmness. I pressed the brake fully to the floor. A dashboard warning appeared: “gas and brake pedals are being pressed simultaneously.” I was not, and had not been, touching the accelerator. I then heard engine RPM increase audibly, consistent with an acceleration command, while my foot remained fully on the brake. I felt forward thrust working against my braking. The warning cleared and brakes engaged. Vehicle stopped approximately half a car length from the vehicle ahead, narrowly avoiding rear-end collision. Automatic Emergency Braking did NOT engage despite conditions clearly meeting its criteria — stationary vehicle directly ahead, my vehicle approaching at 15-20 mph with no observable deceleration for 3-4 seconds. No collision warning sounded; no auto-braking intervened. Notably, this same AEB has previously engaged falsely in benign situations (e.g., backing near low vegetation), suggesting reliability/calibration issues. Prior manufacturer notice: On [XXX] at 15,412 miles, I reported to Camelback Toyota, Phoenix (RO #[XXX], on file): “vehicle lurches forward going from reverse to drive (now more consistent)” — dispositioned by tech 10210 as “vehicle is operating to spec,” no diagnosis performed. Same RO documented “rattling in neutral/at stop” — dispositioned “unable to duplicate.” Toyota was on notice of throttle-response symptoms 21 months before this safety event. I have had an issue several times which nearly resulted in a traffic accident. The truck has a sudden loss of acceleration where the gas pedal is completely dead. It last a few seconds and then goes back to normal. This has nearly caused several accidents where I could not properly accelerate while traffic was heading in my direction. After talking with other Tundra owners, I am not the only one with this issue. This needs immediate attention. If it is not fixed as soon as possible, I fear that someone will get seriously injured or killed. Please take this matter seriously. I suspect it may have something to do with the pre collision sensor system on the truck. It seems that it happens when there is a slight grade or dip in the road. Perhaps the sensors are at a lower angle due to the dip, making it think there is an object it will hit. The Toyota service centers don’t appear to know why this is happening and cannot reproduce the issue in a short visit.

The lens on the forward facing camera in the front grill cracked making the camera useless and in need of replacement. I first noticed the problem about a year ago. It is available for inspection. I can't see any damage around the camera that would have been caused by an object hitting the front of the truck which leads me to believe that the camera lens cracked because of a defect. The camera has not been inspected by the dealer however I have gotten a replacement estimate at the dealer.

I have taken my vehicle in more than 3 times, regarding braking issues, the vehicle would brake automatically even when there is nothing in front of it, the pre collision sensor comes on in rain when there is no debris in front of it.Vwhicle was having throttle lag when come to a complete stop at a light.Now the panoramic roof is leaking, took it to the dealer more than 3 rimes for some these issues, regarding the roof leak, they said they were unable to duplicate it, I have videos showing it was leaking & pictures of all the malfunction from the dashboard.

The contact owns a 2024 Toyota Tundra. The contact stated while driving 25 MPH at night, the low beam and high beam headlights were extremely dim. The vehicle was taken to the dealer, where the high beam headlights were adjusted; however, the failure persisted while driving. The contact's friend inspected the vehicle and informed him that the LED fog lights were not installed on the vehicle; however, the vehicle specifications indicated that the vehicle was designed with LED fog lights. The vehicle was taken to the dealer who informed the contact that they were unaware that the LED fog lights were not installed on the vehicle at the factory. The dealer advised the contact that the manufacturer had informed them that the LED fog lights were not installed on the vehicle because they could cause a glare to other drivers when driving with the suspension lifted. The dealer installed the LED fog lights on the vehicle. Additionally, the contact stated that while driving 30 MPH with the Adaptive Cruise Control engaged, the Lane Keep Assist caused the vehicle to drift outside the lane to the left and to the right continuously and failed to keep the vehicle in the lane. The dealer was notified of the failure and informed the contact that the failure occurred due to a crack on the front grill. The contact also stated that the hood molding was causing a sun reflection on his eyes while driving, which affected the visibility of the roadway even while wearing sunglasses. The contact stated that the vehicle design caused the water on the road to splash on the windshield while driving instead of parting the water to the sides, which temporarily reduced the visibility of the roadway. The contact stated that the trims on the windows were inconsistently sealed. The dealer was notified of the failures and informed the contact that they would need the vehicle and User data to repair the vehicle. The contact declined to provide the data to prevent a breach. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 Tundra vehicles equipped with a V35A engine. Debris from the manufacturing process may contaminate the engine and cause a main bearing to fail, which can result in an engine stall and loss of drive power.

Consequence & remedy

Consequence: A loss of drive power increases the risk of a crash.

Remedy: Dealers will perform an inspection on the #1 main bearing using vehicle inspection software and drive data, and if necessary, replace the engine. Repairs will be performed free of charge. Interim letters notifying owners of the safety risk were mailed July 23, 2026. Additional letters will be sent once the final remedy is available. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB14 and 25TA14. This recall expands previous NHTSA recall numbers 24V381 and 25V767.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist ECU software, free of charge. Owner notification letters were mailed March 23, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26TB02 and 26TA02.

Consequence & remedy

Consequence: Reverse light failure may reduce visibility, and not alert pedestrians and other drivers that the vehicle is backing up, increasing the risk of a crash.

Remedy: Dealers will replace both reverse light assemblies and repair the wire harnesses as necessary, free of charge. Owner letters were mailed May 23, 2026. This is a phased recall. Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 Sequoia Hybrid, Tundra and Tundra Hybrid vehicles. The passenger-side front and rear tires may have inner sidewall damage, which can cause a sudden loss of tire pressure.

Consequence & remedy

Consequence: A sudden loss of tire pressure can result in a loss of vehicle control, increasing the risk of a crash.

Consequence: Unexpected vehicle movement can increase the risk of a crash.

The load carrying capacity modification label states the incorrect load carrying capacity, which can result in an overloaded vehicle. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."

Consequence & remedy

Consequence: An overloaded vehicle increases the risk of a crash.
