Within the 1st year I purchased it new, I would get a warning that the RSA is malfunctioning. It seems to happen when I park in the sun and the windshield is very hot. The warning in my dash prevents me from seeing my other readings; mph, fuel, and I can't use my other safety features; LDA, PCS or cruise control. I've taken it in but then it does a reset and the technicians say there's nothing wrong. The warnings happen every month, weekly. I just got tired of taking it in to hear they don't see anything wrong!

Our 3 yrs old + Toyota RAV4 Hybrid SUV with barely over 30,000 miles on it had 2 safety issues within 2 months of each other! First was the driver's side mirror- it was rattling, feeling like it might fall off! Seems common issue, perhaps due to cheap plastic parts inside. We had it fixed- Toyota Brand Engagement Center gave a 'Goodwill' gesture to cover us replacing it at Dealership, though it should never have happened on such a newer vehicle in the first place! Then not even 2 months later, a Warning flashed- "Pre Collision System Malfunction!" Also "Headlights" with same warning- "SEE DEALER NOW!" Toyota Dealer. They said Forward Recognition Camera had 'INTERNAL MALFUNCTION' and needed to be replaced. It was going to be quite expensive, but the camera part is not even available in foreseeable future as there's back order for hundreds and they're still in production! It seems this too is a common safety problem~ We can't understand why there's not a RECALL on this. Toyota took no responsibility for this issue, which is APPALLING as, like the first 'side driver's side mirror failure' was also a safety issue! The whole reason we bought this vehicle was for its safety features! Without the front facing camera that comes with the car in same compartment as rear view mirror resides, we now have NO Pre-Collision Warnings, NO Headlight safety feature, NO RSA (Road Sign Assist System). It's infuriating that Toyota, who obviously is making inferior safety products, does not offer to help out in any way. We're both [XXX] and entitled to these safety features as we bought the car both new and paid top dollar! Our service advisor at the Toyota Dealership agrees this is 'not normal', the car is too young to have this happen! He will attest to this & we have paperwork. It stated 'No immediate remedy'. (replacement camera not available) I believe this is happening to hundreds of other RAV4 owners. It's outrageous! Please help! These electronics for safety should not fail! I am filing this urgent complaint regarding a life-threatening defect in my 2022 Toyota RAV4 SUV that gravely endangers my family and the public. This is a desperate plea for intervention. For over a year, my vehicle has repeatedly displayed a terrifying, unreadable red alert and piercing buzzer, occurring 3-4 times without warning and not real collision threat. These incidents are terrifying precursors to catastrophic failure. The Toyota dealership dismissed my concerns, stating they "cannot do anything" without replicating the intermittent problem. This dangerous negligence compounds my frustration, especially after prior unaddressed issues like radio malfunctions and door water infiltration. Crucially, the dealer's own CAR DTC logs confirmed C14FE14: "steering angle sensor supply voltage circuit short to ground or open." This documented electrical failure directly impacts the steering system. Toyota's own webpage [XXX] ) also details similar warnings, confirming my fear of brake system activation. My paramount fear is a false Pre-Collision System activation. Imagine driving at high speed when the brakes violently engage or steering falters due to this electrical short. The unthinkable consequences could be a devastating collision, causing severe injury or death to my family and others. This is a documented defect, met with manufacturer and dealer refusal to act. I cannot feel secure driving this car, nor can I responsibly sell this ticking time bomb to another family. I implore the NHTSA to launch an immediate, comprehensive investigation into this critical safety defect. This is a public safety matter demanding urgent attention. Apart from that My Toyota RAV4 Moon Roof shield couldn't be able to withstand for the wind during raining when the vehicle speed was at 75mph on Highway i-35 in Oklahoma. The glass busted completely, and those glass particles causes inconvenience to the other vehicle on the high way. I reported this incident to the car dealer but they were so rude to me, and said that car is not designed for the 75mph speed. Ended up convincing myself that RAV4 is not safe to drive anymore.

The Cruise Control has issues. On 3 long rides so far, I have experienced that it suddenly loses speed and gives the message radar ready now. Furtunately i was maintaining very safe driving distance with other cars on the highway, which has helped me accelerate in time to continue driving and avoid collision. I dont know of any pattern which triggers this. This could lead to a serious problem if it fails in a tricky spot on highway. I am afraid to use it as its not 100 percent reliable.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022 Rav4, Rav4 Hybrid, and Rav4 Prime vehicles. The front passenger seat may have been assembled with interference between internal parts that may cause the Occupant Classification System (OCS) sensor to incorrectly detect the occupant. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."

Consequence & remedy

Consequence: Incorrect detection of an occupant may result in improper air bag deployment during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and repair the front passenger seat, as necessary, free of charge. Owner notification letters were mailed September 2, 2022. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 22TA08.
