ON MY WAY TO TUFTS MEDICAL IN BOSTON MASS. I PAID THE LAST TOLL IN AND NOT 200 FT FROM THE TOLL BOOTH, MY CAR STOPPED. MY RAV4 SHUT DOWN AND ROLLED BACK TOWARDS THE TOLL BOOTH (TOWARDS ONCOMING TRAFFIC). THE CAR WOULD NOT STOP DESPITE LIFTING THE EMERGENCY BRAKE. AFTER TURNING THE CAR OFF, IT SLOWLY STOPPED BUT, DID CONTINUE TO ROLL. MASS STATE TROOPERS ARRIVED IN ABOUT 5-7 MINUTES AND PLACED HIS VEHICLE SO THAT IT WOULD NOT ROLL INTO ANYONE. MY CAR WAS TOWED BACK TO THE DEALER WHILE I RENTED A CAR TO GET BACK TO WESTERN MASS. THE DEALER HAS LOOKED AT THE CAR 3 DIFFERENT TIMES AND FOUND NOTHING. I CAN NO LONGER TRANSPORT MY ELDERLY MOTHER TO HER APPOINTMENTS IN BOSTON WITHOUT RENTING A CAR. *TR

MY FIANCEE AND I WERE DRIVING, 2008 RAV4/LIMITED/V6/4WD. WE WERE AT A STOP LIGHT. I WAS DRIVING, I HAD MY FOOT ON THE BRAKE FOR ABOUT 30 SECONDS WAITING FOR LIGHT TO TURN GREEN. IT WAS A BUSY INTERSECTION HERE IN DENVER AND THERE WAS A CAR IN FRONT OF US. ALL OF A SUDDEN THE ACCELERATOR TOOK OFF AND I COULD FEEL THE CAR TRYING TO MOVE FORWARD. LUCKILY, I ALREADY HAD MY FOOT ON THE BRAKE AND I PRESSED DOWN EVEN HARDER ON THE BRAKE AS I IMMEDIATELY PUT THE CAR IN NEUTRAL. WHEN I PUT IT IN NEUTRAL THE ACCELERATOR CONTINUED TO REV HIGHER THAN IT WAS BEFORE. I SLAMMED THE GEAR SHIFT INTO PARK AND SHUT THE CAR OFF. WE WERE OBVIOUSLY SHAKEN FROM THIS AND JUST SAT THERE FOR A MINUTE AS WE REALIZED WHAT HAD JUST HAPPENED. MY FIANCEE WANTED TO GET OUT OF THE CAR AND CALL A TOW TRUCK. I WANTED TO TRY AND START THE CAR AGAIN BUT LEAVE IT IN PARK AND TEST IT OUT. I STARTED THE CAR WITHOUT TOUCHING THE BRAKE AND IT DID IT AGAIN. EVERY LIGHT ON THE DASH BOARD LIT UP LIKE A CHRISTMAS TREE. WE WERE IN PARK SO THE CAR COULD NOT MOVE BUT THE RPM'S WERE ON THE WAY TO REDLINING AND I TURNED OFF THE CAR AGAIN. THE LIGHTS AROUND THE SPEEDOMETER HAD NEVER LIT UP LIKE THAT BEFORE. SERIOUSLY EVERY DUMMY LIGHT ON THE CAR WAS LIT AND FLASHING. WE WERE COMPLETELY SCARED AT THIS POINT AND WE JUST TURNED ON THE EMERGENCY LIGHTS AND JUST SAT THERE IN SHOCK. WE SAT THERE FOR TEN MINUTES AT THIS BUSY INTERSECTION TRYING TO FIGURE OUT WHAT TO DO. I DECIDED TO TRY IT AGAIN AND THIS TIME THE CAR STARTED NORMALLY. MY FIANCEE WAS AFRAID TO DRIVE IT BUT I ASSURED HER IF IT HAPPENED AGAIN I COULD JUST PUT IT IN NEUTRAL AND WE COULD PULL OVER TO THE SIDE OF THE ROAD. WE DROVE DIRECTLY TO THE DEALERSHIP. WE EXPLAINED WHAT HAPPENED AND THEY SAID THE WOULD CHECK IT OUT. THEY CALLED LATER THAT DAY AND SAID ALL THE DIAGNOSTIC TESTS WERE NORMAL. THEY TOLD ME IT MUST HAVE BEEN THE FLOOR MATS. THAT IS IMPOSSIBLE SINCE THE FLOOR MATS WEREN'T EVEN TOUCHING ACCELERATOR. UPDATED 05/12/10 *BF UPDATED 07/16/10.*JB *TR

I WAS DRIVING MY 2007 TOYOTA RAV 4 AND I WAS STOPPED AT A TRAFFIC LIGHT. WHEN THE LIGHT TURNED GREEN, I TURNED LEFT AND THE CAR STARTED TO RACE AND ACCELERATED TO 40 MPH AND STUCK THERE. I TRIED PRESSING ON THE BRAKE PEDAL AND NOTHING HAPPENED. I SWERVED TO MISS TWO CARS AND THE CAR KEPT GOING. I TRIED PUTTING IT INTO NEUTRAL AND NOTHING HAPPENED AGAIN. I THEN CALLED MY FATHER WHO IS FAMILIAR WITH CARS AND TOLD HIM THE STORY AND HE TOLD ME TO TRY THE EMERGENCY BRAKE AND THAT DIDN'T WORK EITHER. HE INSTRUCTED ME TO TRY IT AGAIN AND THIS TIME THE CAR JOLTED FORWARD AND SLOWED DOWN. I PULLED OVER AND TURNED THE IGNITION FF AND WAS VERY SHAKEN AND QUITE UPSET. I HAD THE CAR TOWED TO LIA NORTHAMPTON TOYOTA AND INFORMED THEM I WOULD NOT BE DRIVING THIS LEASED CAR AGAIN UNLESS THEY LOOKED AT IT THOROUGHLY AND REPAIRED IT. THE TECH VERIFIED A HIGH IDLE . THE TECH FOUND A THROTTLE PLATE WAS MECHANICALLY STICKING. THEY REPLACED THE THROTTLE BODY. I ALSO HAD THEM DOCUMENT IT AND GIVE ME PAPERWORK. I DID CONTACT THEM WHEN THE RECALL WAS ANNOUNCED AND THEY TOLD ME THAT 2007 WAS NOT PART AND THEY WOULD NOT LOOK AT MY CAR AGAIN. *TR

I WAS ATTEMPTING TO PASS A SEMI ON AN UPHILL IN MY NEW 2008 RAV4 WHEN THE ACCELERATOR BECAME STUCK. I HAD FLOORED THE GAS PEDAL SINCE I WAS GOING UPHILL AND PASSING, BUT WHEN I EASED MY FOOT OFF THE PEDAL REMAINED PRESSED TO THE FLOOR. I HAD ENTERED A SINGLE LANE CONSTRUCTION ZONE AND WAS ACCELERATING RAPIDLY. I STOMPED ON THE ACCELERATOR HOPING TO SPRING IT LOOSE AND TRIED THE BRAKES, TO NO AVAIL. AS I WAS REACHING SPEEDS WELL INTO THE 90'S AND WITHIN SECONDS OF PLOWING INTO THE TRAFFIC AHEAD OF ME I REACHED DOWN AND GRABBED THE GAS PEDAL AND PULLED UP ON IT. IT DID COME UP THEN AND I WAS ABLE TO SLOW THE CAR DOWN IN TIME TO AVOID CRASHING. I WAS TERRIFIED AND THOUGHT I WOULD DIE. I TOOK IT TO THE DEALER WE HAD PURCHASED IT FROM AND THEY INSISTED IT WAS CAUSED BY MY FLOOR MAT AND THAT IT WAS MY FAULT SINCE IT WAS NOT A TOYOTA FLOOR MAT. I TOLD THEM THERE WAS NOTHING OBSTRUCTING THE PEDAL WHEN I REACHED DOWN FOR IT, IT WAS SIMPLY STUCK. THEY CONTINUE TO BLAME THE FLOOR MAT, MY CAR HAS NOT BEEN PUT ON THE RECALL LIST AND IT SCARES ME EVERY DAY TO DRIVE IT. I NEVER, EVER FLOOR IT AND NOW KNOW TO PUT IT IN NEUTRAL SHOULD IT HAPPEN AGAIN---ALTHOUGH I HAVE HEARD SOME PEOPLE CRASHED BECAUSE THEY COULD NOT GET THE CAR IN NEUTRAL. I AM VERY UPSET THAT TOYOTA BLAMESHIFTS AND DODGES THE ISSUE BY BLAMING THE MATS AND REFUSES TO DO ANYTHING ABOUT IT. THEIR RECALL HOTLINE TOLD ME IT IS MY PROBLEM SINCE MY CAR IS NOT ON THE RECALL LIST. I DID BUY THEIR MAT AFTER THE INCIDENT, ONLY TO BE TOLD LATER THAT I SHOULD REMOVE IT SINCE IT COULD CAUSE ACCELERATOR ENTRAPMENT. MY DEALER STILL BLAMES THE WHOLE PROBLEM ON MY NON COMPLIANT MAT EVEN THOUGH THEY RECALLED THEIR OWN MATS, TOO. IT IS VERY DISTURBING TO FIND OUT THAT THIS HAS BEEN HAPPENING TO SO MANY OTHERS AND GOING ON FOR A LONG TIME. SO MANY DEATHS AND ACCIDENTS, AND I EASILY COULD HAVE BEEN ONE. WE HAVE BOUGHT 5 TOYOTAS FROM THIS DEALER AND HAVE HAD NO SUPPORT AND HAVE BEEN TREATED RUDELY. *TR

Aug 12, 2016

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2011 Toyota RAV4 vehicles manufactured October 31, 2005, to September 7, 2010, and 2010 Lexus HS250h vehicles manufactured July 6, 2009, to August 26, 2010. The affected vehicles have rear suspension arms (rear tie rods) with adjusting lock nuts that may have been improperly tightened after servicing. As a result of being loose, the arm may have thread damage and may rust, possibly leading to the failure of the arm and an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners and dealers will replace both rear suspension arms and encapsulate the locknuts, free of charge. The recall began on September 30, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are G0V for Toyota vehicles and GLK for Lexus vehicles. Note: This recall supersedes recall 13V-383, in which some vehicles were only inspected, sealed, and did not have both arms replaced. The affected vehicles were previously recalled under 13V-383, but did not have both rear suspension arms replaced at that time. Those vehicles need to have both arms replaced as part of this recall remedy.

14V168000 · Air Bags:frontal:driver Side:inflator Module

Apr 9, 2014

Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.

Consequence & remedy

Consequence: If the FFC is damaged, connectivity to the driver's air bag module could be lost and the air bag deactivated. The failure of the driver's air bag to deploy in the event of a crash that typically necessitates deployment increases the risk of injury to the driver.

Remedy: Toyota will notify the Toyota vehicle owners, and General Motors will notify the Pontiac Vibe owners. Toyota sent owners an interim notification letter in May 2014 to advise owners of the recall. Toyota will mail owners a second letter when remedy parts are available. General Motors sent Pontiac Vibe owners an Interim letter on June 20, 2014 notifying them that parts are currently unavailable. General Motors sent notices to the Pontiac Vibe owners on September 15, 2014, that a remedy is available. The recall began on October 27, 2014. Owners may contact Toyota at 1-800-331-4331. Owners may contact General Motors at 1-800-762-2737.

Aug 28, 2013

Toyota is recalling certain model year 2006 through 2011 RAV4 and 2010 Lexus HS250h passenger vehicles manufactured from October 2005 through September 2010. If the nuts for adjusting the rear wheel alignment are improperly tightened during service, the rear suspension arm (rear tie rod) may develop unwanted movement and rust leading to thread damage and eventual failure. Failure of the rear tie rod will cause an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners, and dealers will inspect the tie rods and replace any that have rust or corrosion on the alignment adjustment threads. All of the rear tie rods will then be encased in an epoxy to prevent future environmental damage. Due to limitations in parts, the recall remedy will be launched in phases from September 2013 through mid-April 2014. All owners will receive notification of the recall, with most receiving second notifications when parts are available in their area. IMPORTANT: This recall supersedes Toyota and Lexus recall 12V-373. All vehicles that have been inspected and remedied under the previous recall need to be re-inspected and have the appropriate remedy applied.

Oct 10, 2012

Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

In a letter dated August 1, 2012 Toyota Motor Engineering & Manufacturing North America, Inc. (Toyota) submitted a Defect Information Report (NHTSA Recall No. 12V-373) to the NHTSA identifying a safety defect regarding possible separation of the Rear Suspension Arm No. 1 Assembly (arm) at the threaded portion (shaft and turn buckle) of the component in MY 2006 through MY 2011 Toyota RAV4 vehicles built from October 2005 through September 2010 and in MY 2010 Lexus HS250h vehicles manufactured from July 2009 through August 2010.According to Toyota, if the adjustment nuts for the rear wheel alignment are improperly tightened when alignment is performed in service, backlash may develop at the threaded portion of the arm, followed by rust formation. The threads may then wear causing the arm to separate, which may result in a loss of vehicle control. In analysis of both Vehicle Owner Questionnaire (VOQ) data submitted to the Office of Defects Investigation (ODI) and consumer complaint data submitted to Toyota, ODI identified 131 non-duplicative reports alleging failure of the subject rear suspension arm (Note:some of the complaints and related service records refer to the arm as a rear tie rod).Six of the complainants allege the failure resulted in some loss of vehicle control. For purposes of this analysis, loss of control events involve alleged complete or partial departure from the intended lane of travel. An additional four complaints reported difficulty maintaining control of the vehicle when the arm failed.Recalled vehicles will be inspected for proper torque of the adjustment nuts and replaced as necessary. In addition, Toyota will take several actions to reduce the potential for improper torque in future alignment procedures. This includes:(1) providing proper tightening sequence and torque specification information to two major wheel alignment equipment manufacturers, who would disseminate the information to alignment shops using their equipment; (2) installation of a clip on each arm with an instructional label showing proper tightening sequence and torque specification (the clip must be removed to perform a wheel alignment); (3) installation of additional labels on the front and back vertical surfaces of the Rear Suspension Arm No. 2 assemblies; and (4) providing an instruction booklet to owners to place in the glove box. This investigation is closed.
