MY 2008 TOYOTA RAV4 ACCELERATED WHEN THE BRAKE WAS APPLIED ON TWO SEPARATE OCCASIONS RECENTLY. ONE INSTANCE TOOK PLACE ON OR ABOUT JUNE 30, 2011, THE SECOND TIME HAPPENED ON OR ABOUT MONDAY, AUGUST 1, 2011. THE FIRST TIME, TOYOTA CLAIMED THAT THE NON-BRAKING/UNEXPECTED ACCELERATION PROBLEM WAS CAUSED BY FLOOR MATS I PUT IN THE CAR. I BOUGHT THESE FOR THE CAR BECAUSE I FELT THE ONES TOYOTA ORIGINALLY PROVIDED WERE NOT SAFE (HEELS ON MY SHOES, THOUGH FLAT, KEPT GETTING CAUGHT IN THE DEEP RIDGES OF THEIR MATS, MAKING IT DIFFICULT TO MANEUVER). AFTER SPEAKING WITH THE TOYOTA 'S MECHANICS, I THREW OUT MY MATS AND REPLACED THEM WITH NEW TOYOTA MATS RECOMMENDED BY THE DEALERSHIP. ABOUT ONE MONTH LATER THE SAME THING HAPPENED AGAIN. WHEN I APPLIED MY FOOT TO THE BRAKE, INSTEAD OF THIS ACTION STOPPING THE CAR, THE CAR ACCELERATED AND MOVED FORWARD. THE FIRST TIME THIS HAPPENED, MY CAR WAS APPROACHING A STOP SIGN AT THE BOTTOM OF A HILL LEADING TO MY COMPLEX'S EXIT. AS I NEARED THE STOP SIGN IN FRONT OF A VERY BUSY ROADWAY, I STARTED APPLYING MY BRAKES. THE CAR DID NOT SLOW DOWN. ALTHOUGH I PUSHED DOWN HARDER & HARDER ON THE BRAKE IN A DESPERATE EFFORT TO STOP THE CAR, IT CONTINUED TO MOVE FORWARD. MY RAV4 WENT ONTO THE ROADWAY, NARROWLY MISSING A TRUCK IN ONCOMING TRAFFIC. THE SECOND TIME I WAS IN A PARKING LOT. I THINK THERE IS SOMETHING SIGNIFICANTLY WRONG WITH THIS CAR AND DO NOT BELIEVE IT TO BE SAFE. THE MECHANIC AT TOYOTA, BEFORE EVEN EXAMINING THE CAR, CLAIMED IT WAS "HUMAN ERROR." I HAVE DRIVEN MANY CARS IN MY LIFETIME AND HAVE NEVER HAD THIS HAPPEN TO ME BEFORE. IT IS MY SINCERE HOPE THAT THE NHTSA & DOT WILL LOOK INTO THIS AND OTHER RELATED MATTERS. IT APPEARS THERE ARE MANY PEOPLE HAVING BRAKE PROBLEMS WITH THESE CARS. I TRULY BELIEVE PEOPLE'S LIVES ARE IN DANGER. I'M AFRAID TO HAVE MY GRANDDAUGHTERS TRAVEL IN MY CAR! *KB

I DID NOT HAVE ANY BRAKES, SO I TRIED PUMPING BRAKES BEFORE IMPACT . BRAKE PEDAL WAS COMPLETELY TO THE FLOOR, SO I USED EMERGENCY BRAKE TO STOP CAR BEFORE IT CAUSED FURTHER DAMAGE TO CAR AND FENCE. I DID NOT REPORT INCIDENT, BUT TOOK CAR TO LACEY COLLISION, AND THEY ESTIMATED DAMAGE AT $1500. THIS IS NOT THE FIRST TIME THIS HAS HAPPENED, I HAVE REPORTED THIS TO TWO DIFFERENT TOYOTA DEALERSHIPS..NOTHING WAS EVER DONE. THEY ONLY ADMITTED THAT YES, MY BRAKES WERE A BIT 'SOFT'. BUT CAR WAS NOT ON THEIR RECALL LIST.

Aug 12, 2016

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2006-2011 Toyota RAV4 vehicles manufactured October 31, 2005, to September 7, 2010, and 2010 Lexus HS250h vehicles manufactured July 6, 2009, to August 26, 2010. The affected vehicles have rear suspension arms (rear tie rods) with adjusting lock nuts that may have been improperly tightened after servicing. As a result of being loose, the arm may have thread damage and may rust, possibly leading to the failure of the arm and an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners and dealers will replace both rear suspension arms and encapsulate the locknuts, free of charge. The recall began on September 30, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are G0V for Toyota vehicles and GLK for Lexus vehicles. Note: This recall supersedes recall 13V-383, in which some vehicles were only inspected, sealed, and did not have both arms replaced. The affected vehicles were previously recalled under 13V-383, but did not have both rear suspension arms replaced at that time. Those vehicles need to have both arms replaced as part of this recall remedy.

Apr 9, 2014

Toyota is recalling certain model year 2006-2008 RAV4, 2006-2010 Yaris, 2008-2010 Highlander, and 2009-2010 Tacoma, Corolla, Corolla Matrix and General Motors Pontiac Vibe vehicles. In the affected vehicles, the steering column assembly contains electrical connections to the driver's airbag module housed in a spiral cable assembly, which includes a Flexible Flat Cable (FFC). Due to the shape and location of the FFC's retainer, the FFC could become damaged when the steering wheel is turned.

Consequence & remedy

Consequence: If the FFC is damaged, connectivity to the driver's air bag module could be lost and the air bag deactivated. The failure of the driver's air bag to deploy in the event of a crash that typically necessitates deployment increases the risk of injury to the driver.

Remedy: Toyota will notify the Toyota vehicle owners, and General Motors will notify the Pontiac Vibe owners. Toyota sent owners an interim notification letter in May 2014 to advise owners of the recall. Toyota will mail owners a second letter when remedy parts are available. General Motors sent Pontiac Vibe owners an Interim letter on June 20, 2014 notifying them that parts are currently unavailable. General Motors sent notices to the Pontiac Vibe owners on September 15, 2014, that a remedy is available. The recall began on October 27, 2014. Owners may contact Toyota at 1-800-331-4331. Owners may contact General Motors at 1-800-762-2737.

Aug 28, 2013

Toyota is recalling certain model year 2006 through 2011 RAV4 and 2010 Lexus HS250h passenger vehicles manufactured from October 2005 through September 2010. If the nuts for adjusting the rear wheel alignment are improperly tightened during service, the rear suspension arm (rear tie rod) may develop unwanted movement and rust leading to thread damage and eventual failure. Failure of the rear tie rod will cause an abrupt change in the vehicle's alignment.

Consequence & remedy

Consequence: Failure of the rear tie rod could cause a loss of vehicle control, increasing the risk of a crash.

Remedy: Toyota will notify owners, and dealers will inspect the tie rods and replace any that have rust or corrosion on the alignment adjustment threads. All of the rear tie rods will then be encased in an epoxy to prevent future environmental damage. Due to limitations in parts, the recall remedy will be launched in phases from September 2013 through mid-April 2014. All owners will receive notification of the recall, with most receiving second notifications when parts are available in their area. IMPORTANT: This recall supersedes Toyota and Lexus recall 12V-373. All vehicles that have been inspected and remedied under the previous recall need to be re-inspected and have the appropriate remedy applied.

Oct 10, 2012

Toyota is recalling certain model year 2007-2009 Camry, Camry Hybrid, RAV4, Corolla, Corolla Matrix, Tundra, Sequoia, Highlander, Highlander Hybrid, Yaris, Scion xB, Scion xD and Pontiac Vibe vehicles. The power window master switch assemblies in some of these vehicles were built using a less precise process for lubricating the internal components of the switch assemblies. Irregularities in this lubrication process may cause the power window master switch assemblies to malfunction and overheat.

Consequence & remedy

Consequence: If the switch overheats, it may melt, possibly resulting in a fire.

Remedy: Toyota will notify its owners, and dealers will inspect the switch and apply a special grease that inhibits heat build up or replace any switch damaged by the defect, free of charge. Toyota owners that experience a problem with a switch should not attempt to make repairs, but should contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331. General Motors will contact the Pontiac Vibe owners affected by this recall and GM dealers will inspect and repair switches as necessary. Those owners may contact Pontiac Customer Care at 1-800-762-2737.

Status: closed (inferred from source dates) · Suspension; Suspension:rear

In a letter dated August 1, 2012 Toyota Motor Engineering & Manufacturing North America, Inc. (Toyota) submitted a Defect Information Report (NHTSA Recall No. 12V-373) to the NHTSA identifying a safety defect regarding possible separation of the Rear Suspension Arm No. 1 Assembly (arm) at the threaded portion (shaft and turn buckle) of the component in MY 2006 through MY 2011 Toyota RAV4 vehicles built from October 2005 through September 2010 and in MY 2010 Lexus HS250h vehicles manufactured from July 2009 through August 2010.According to Toyota, if the adjustment nuts for the rear wheel alignment are improperly tightened when alignment is performed in service, backlash may develop at the threaded portion of the arm, followed by rust formation. The threads may then wear causing the arm to separate, which may result in a loss of vehicle control. In analysis of both Vehicle Owner Questionnaire (VOQ) data submitted to the Office of Defects Investigation (ODI) and consumer complaint data submitted to Toyota, ODI identified 131 non-duplicative reports alleging failure of the subject rear suspension arm (Note:some of the complaints and related service records refer to the arm as a rear tie rod).Six of the complainants allege the failure resulted in some loss of vehicle control. For purposes of this analysis, loss of control events involve alleged complete or partial departure from the intended lane of travel. An additional four complaints reported difficulty maintaining control of the vehicle when the arm failed.Recalled vehicles will be inspected for proper torque of the adjustment nuts and replaced as necessary. In addition, Toyota will take several actions to reduce the potential for improper torque in future alignment procedures. This includes:(1) providing proper tightening sequence and torque specification information to two major wheel alignment equipment manufacturers, who would disseminate the information to alignment shops using their equipment; (2) installation of a clip on each arm with an instructional label showing proper tightening sequence and torque specification (the clip must be removed to perform a wheel alignment); (3) installation of additional labels on the front and back vertical surfaces of the Rear Suspension Arm No. 2 assemblies; and (4) providing an instruction booklet to owners to place in the glove box. This investigation is closed.
