RE.: PREMATURE FAILURE OF 2017 TOYOTA RAV-4 SE HYBRID 4-WD ALUMINUM WHEEL. ON SUNDAY, OCTOBER 7TH, 2018, WHILE DRIVING EASTBOUND ON HIGHWAY 70 AT AROUND 10:25 A.M., THE LEFT REAR WHEEL FAILED UNEXPECTEDLY ON MY WIFE'S 2018 TOYOTA RAV-4 SE HYBRID. I WAS TRAVELING WITH MY WIFE, MY 13-YEAR OLD DAUGHTER, AND MY 9-YEAR OLD SON WHEN WE WERE STARTLED BY A LOUD BOOM, WHICH SOUNDED LIKE A GUN SHOT. AT FIRST, I THOUGHT I HAD RAN OVER SOMETHING, BUT QUICKLY RULED THAT OUT BECAUSE WITHIN SECONDS I COULD FEEL THE VEHICLE'S REAR END MAKING LOUD NOISES AND VIBRATED STRONGLY. I KNEW THEN THAT I HAD A FLAT TIRE. WE WERE GOING ABOUT 65MPH AND TRAVELING EAST ON THE FAR LEFT HAND OF A 4-LANE HIGHWAY. WHEN I ARRIVED AT THE BUDDHIST TEMPLE, I TOOK THE DAMAGED TIRE OUT TO INSPECT IT. I WAS TRYING TO SEE WHAT HAD CAUSED THE TIRE TO GO FLAT. BUT AS I WAS INSPECTING THE TIRE I THEN NOTICED THE FRACTURED AREA IN THE MIDDLE OF THE ALUMINUM WHEEL. AS YOU CAN SEE FROM MY PICTURES, THE INNER AND OUTER ALUMINUM WHEEL BEADS HAS NO SIGN OF DAMAGE. IN ADDITION, I COULD NOT DETECT ANY PUNCTURE HOLE IN AND AROUND THE RUBBER TIRE THAT COULD HAVE RESULTED IN THE METAL FRACTURES. I WANT TO ALERT YOU TO THE POTENTIAL ALUMINUM WHEEL DEFECT THAT MAY EXIST IN MY OTHER THREE FACTORY SUPPLIED WHEELS, AS WELL AS OTHER RAV-4 THAT HAVE THE SAME SE WHEELS MANUFACTURED IN JAPAN. I HOPE THAT NHTSA WILL REVIEW THE EVIDENCE I HAVE PROVIDED AND BEGIN A FORMAL INVESTIGATION INTO THIS DANGEROUS WHEEL DEFECT. I WILL KEEP THE DAMAGED WHEEL AND WILL NOT ALLOW TOYOTA OR BOMMARITO TO TAKE POSSESSION OF MY DAMAGED ALUMINUM WHEEL UNTIL I HAVE RECEIVED FURTHER INSTRUCTION FROM YOUR OFFICE. 17V295000 · Tires:temporary/emergency Spare Tire

May 2, 2017

Gulf States Toyota, Inc. (Gulf States) is recalling certain 2017 4Runner , 86, Avalon, Camry, Camry Hybrid, Corolla, Corolla iM, Highlander, Highlander Hybrid, Prius, Prius C, RAV4, RAV4 Hybrid, Sienna and Yaris vehicles. The spare tire air pressure was not adjusted to the proper pressure as stated on the Tire Pressure Label.

Consequence & remedy

Consequence: A spare tire that is not inflated to the proper value may not perform as intended, increasing the risk of a crash.

Remedy: Gulf States will notify owners, and dealers will inspect the spare tires, adjusting their pressure as necessary, free of charge. The recall began June 15, 2017. Owners may contact Gulf States customer service at 1-800-444-1074. Toyota's number for this recall is 17R2.

DP22005 · Pedestrian Alert Sounds

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.
