I STUDIED THE FRACTURE FACE AND SAW TYPICAL FATIGUE FAILURE WITH ORIGIN AND PROGRESSION FEATURES. IT WAS QUITE EVIDENT THE PROBLEM STARTED MUCH EARLIER THAN TODAY. I ONLY SAW ONE BROKEN LUG, BUT THE SERVICE REP SAID THAT THERE WERE TWO LUGS THAT BROKE PREMATURELY WHEN THEY WERE REINSTALLING ONE WHEEL. THIS CONCERNS ME SO I AM GIVING THIS INFORMATION TO NHTSA. I HAVE TAKEN CLOSE-UP PICTURES OF THE FRACTURE FACE IF YOU WOULD LIKE THEM SUBMITTED. TO BE SAFE, I AM HAVING ALL THE FRONT WHEEL LUGS REPLACED ON THIS VEHICLE. AGREED, THIS ISN'T NECESSARILY A TOYOTA PROBLEM. IT COULD HAVE BEEN CAUSED BY OVERTORQUE WHEN I HAD THE TIRES REPLACED. IF IT IS A TOYOTA PROBLEM, YOU'LL HAVE OTHER REPORTS SIMILAR TO MINE. A FEW DAYS AFTER PICKING UP MY VEHICLE, WHILE I WAS PARKED DROPPING OFF MY PASSENGER, MOST OF THE CONTROL PANEL LIGHTS ILLUMINATED (BRAKES, ABS, TRAC OFF, VSC, CHECK ENGINE). I REPORTED THIS TO THE DEALER. THEY SAID TO BRING THE CAR BACK. I TOOK THE CAR BACK AND DIAGNOSTIC TESTS WERE RUN AND CONCLUDED THAT A CODE WAS STORED BUT IT WAS NOTHING SPECIFIC THAT HAD FAILED. BRAKE SYSTEM WAS BLED. IN JUNE 2011, THE LIGHT CLUSTER ("CHRISTMAS TREE" IS HOW THE DEALER REFERS TO IT) ILLUMINATED AGAIN WHILE I WAS ATTEMPTING TO PICK UP MY CAR FROM MY LOCAL TOYOTA DEALER (BOB SMITH TOYOTA). AGAIN, THE CAR WAS LEFT. AGAIN IT WAS REPORTED TO ME THAT THE SYSTEM HAD STORED A CODE BUT NO SPECIFIC COMPONENT WAS IN FAILURE AND THE BRAKES WERE BLED. APPROXIMATELY, JULY 20, 2011 AND JULY 23, 2011, THE LIGHTS ILLUMINATED AGAIN, ONLY FOUR OF THEM (TRAC OFF, VSC, ABS, BRAKES). I CONTACTED THE TOYOTA DEALER ON TUESDAY, JULY 26 AND WAS FORCING THE ISSUE ON JULY 29, 2011 AND MADE AN APPOINTMENT FOR AUGUST 1, 2011. ON JULY 31, 2011, THE SAME FOUR LIGHTS ILLUMINATED WHILE DRIVING AND STAYED ON. I STOPPED, TURNED OFF THE ENGINE, RESTARTED THE VEHICLE, AND EXPERIENCED A VERY SPONGY, SQUISHY BRAKE PEDAL. I CONTACTED TOYOTA MOTOR CORPORATION TO FURTHER ADDRESS THE CONCERN OF MY VEHICLE NOW BEING UNSAFE AS A RESULT OF THEIR RECALL. TOYOTA MOTOR CORPORATION WAS VAGUE AND SAID THAT THE MATTER COULD ONLY BE ADDRESSED BY LONGO TOYOTA, THE DEALER THAT PERFORMED THE RECALL, SINCE THE PARTS WARRANTY WOULD ONLY BE WITH THEM. IN CONTACTING BOB SMITH TOYOTA, THE SERVICE ADVISORS SWEARS THAT THE PROBLEM CAN BE RESOLVED BY THEM. IT APPEARS THAT PERFORMING THE RECALL WEAKENED THE INTEGRITY OF MY BRAKE SYSTEM. IT HAD BUZZERS AND WARNING LIGHTS GOING OFF. THE DEALER LOOKED AT IT AND THOUGHT THAT IT WAS THE ANTI-LOCK BRAKE & VSC "ECU", WHICH WAS GOING TO COST $2,400, BUT THERE COULD BE EXTRA SENSORS THAT NEEDED TO BE REPLACED TOO. THIS SEEMED VERY EXCESSIVE SO I DIDN'T HAVE THE WORK DONE. NOW THE CAR IS SITTING. I'VE HEARD THERE IS A SERVICE BULLETIN OUT ON THIS BUT I HAVEN'T BEEN ABLE TO FIND IT. I'VE HEARD THAT IT CAN BE THE STEERING ANGLE SENSOR, OR THE WHEEL ROTATIONS SENSORS, OR THE G-CENTER (ACCELEROMETER). EACH ONE IS SEVERAL HUNDRED DOLLARS APIECE. A FAILURE IN THE SKID CONTROL SYSTEM SHOULD JUST STOP WORKING AND NOT CAUSE THE BRAKES TO BE SLAMMED ON, LEAVING THE CAR UNDRIVABLE. AND THE REPAIR COST SHOULD NOT BE $2,500 TO OVER $4,000. THIS SITUATION FORTUNATELY OCCURRED NEAR OUR RESIDENCE ON DRY PAVEMENT. BY DRIVING VERY SLOWLY WE MANAGED TO GET THE VEHICLE TO A SAFE LOCATION. IT WAS THEN TOWED TO A LOCAL TOYOTA DEALER WHERE IT WAS INSPECTED AND THE FOLLOWING DIAGNOSIS OR SIMILAR WAS PROVIDED: CUSTOMER STATES THAT THE VSC LIGHTS ARE COMING ON AND THE BRAKES ARE LOCKING UP, YAW RATE SENSOR IS OUT OF CALIBRATION. RECALIBRATED YAW RATE SENSOR. THE VEHICLE WAS RELEASED TO US AS REPAIRED. THE PRECISE SAME VSC FAILURE SITUATION OCCURRED ON OCTOBER 11TH OF THIS YEAR AND AGAIN WE WERE LUCKY TO HAVE BEEN IN A REASONABLY SAFE LOCATION AND DRIVING UNDER GOOD ROAD CONDITION WHEN IT HAPPENED. THE VEHICLE WAS PICKED UP AND TOWED AGAIN TO THE DEALER FACILITY FOR ANALYSIS AND REPAIR. WE RECEIVED THE DIAGNOSIS: ?ELECTRICAL REPAIR, CUSTOMER STATES THAT THE VSC LIGHTS ARE COMING ON, AND THE BRAKES ARE LOCKING UP, YAW RATE SENSOR IS OUT OF CALIBRATION. RECALIBRATED YAW RATE SENSOR?. I ASKED THE DEALER TO DISCONNECT THE ANTI-SKID SYSTEM, BECAUSE THIS SYSTEM WAS MORE DANGEROUS THAN THE PROBLEM IT WAS TRYING TO ADDRESS. THE DEALER SAID THAT WAS NOT POSSIBLE AND IF THE STEERING SENSOR WAS NOT REPLACED, THE CAR WOULD NOT BE DRIVABLE. IF A SENSOR IN A CONTROL SYSTEM FAILS, IT SHOULD NOT CAUSE A DANGEROUS DRIVING SITUATION. IF A SENSOR FAILS, THE BRAKES SHOULD NOT ENGAGE, THE SYSTEM SHOULD JUST BECOME INOPERABLE. THE LOGIC IN THEIR CONTROL SYSTEM IS FAULTY AND THE COMPUTER SHOULD BE RE-PROGRAMMED TO ELIMINATE THE PROBLEM. TOYOTA MOTORS WAS INFORMED OF THE PROBLEM AND THEY CHOOSE TO IGNORE MY COMPLAIN. THE DEALERSHIP REPLACED THE STIRRING SENSOR FOR A RIDICULOUS PRICE AND THE CAR IS ABLE TO MOVE FORWARD AGAIN WITHOUT THE ANTI-LOCK BRAKES ENGAGING. Consequence: IN SOME CASES, THE BELT FOR THE POWER STEERING PUMP MAY BECOME DETACHED FROM THE PULLEY WHICH COULD RESULT IN A LOSS OF POWER STEERING AND A SUDDEN INCREASE IN STEERING EFFORT, INCREASING THE RISK OF A CRASH.

IF REPLACEMENT BRAKE FLUID IS USED THAT DOES NOT CONTAIN SUCH POLYMERS, OR THAT CONTAINS ONLY SMALL AMOUNTS, A PART OF THE RUBBER SEAL LOCATED AT THE REAR OF THE BRAKE MASTER CYLINDER MAY BECOME DRY, AND THE SEAL MAY CURL DURING MOVEMENT OF THE PISTON. IF THIS OCCURS, A SMALL AMOUNT OF BRAKE FLUID COULD SLOWLY LEAK FROM THE SEAL INTO THE BRAKE BOOSTER.

CERTAIN SPORT UTILITY VEHICLES HAVE A CHILD PROTECTION LOCK (CPL) SYSTEM ON BOTH OF THE REAR SIDE DOORS. WHEN AN OPERATOR OF THE VEHICLE CLOSES THE DOOR VERY HARD WITH THE CPL LEVER SET TO THE LOCK POSITION, THERE IS A POSSIBILITY THAT THE LEVER MAY CONTACT THE BODY PANEL, CAUSING THE CPL LEVER TO MOVE INTO THE UNLOCK POSITION.

Consequence & remedy

Consequence: THIS CONDITION COULD ALLOW THE DOOR TO BE OPENED WITH THE INSIDE WHILE THE OPERATOR BELIEVES THAT THE CPL WAS ACTIVATED. IF THE DOOR WERE OPENED FROM THE INSIDE WHILE THE VEHICLE WAS IN MOTION, AN OCCUPANT COULD FALL OUT AND BE INJURED.

Recall 11V-113: (1) MY 2003-2009 through 2009 Toyota 4Runner; (2) MY 2006-2010 Toyota RAV4; and, (3) MY 2008-2011 Lexus LX570.The total estimated population under this recall is 1,381,000 vehicles.This recall supplements recall 09V-388 and remedies potential accelerator pedal entrapment by an unsecured floor mat.Recall 11V-115: (1) MY 2006-2007 Lexus GS300 (all wheel drive vehicles); and, (2) MY 2006-2007 Lexus GS350 (all wheel drive vehicles). The total estimated population under this recall is 19,647 vehicles.This recall remedies potential accelerator pedal entrapment caused by inadequate clearance between the pedal linkage and a plastic pad embedded in the vehicle's carpet.II. Potential Electronic Causes of Unintended Acceleration.After NHTSA opened RQ10-003, NHTSA launched a ten-month study of potential electronic causes of unintended acceleration in Toyota vehicles.NHTSA launched the study in the spring of 2010 in light of concerns aired in Congressional hearings.NHTSA enlisted engineers at the National Aeronautics and Space Administration (NASA) with expertise in areas such as computer controlled electronic systems, electromagnetic interference and software integrity to assess whether electronic systems or electromagnetic interference played a role in incidents of unintended acceleration in Toyota vehicles.That study has concluded.Two reports are associated with the study and are available on NHTSA's website.NASA's report is entitled Technical Support to the National Highway Traffic Safety Administration (NHTSA) on the Reported Toyota Motor Corporation (TMC) Unintended Acceleration (UA) Investigation, NESC Assessment No. TI-10-00618 (Jan. 18, 2011).NHTSA's report is entitled Technical Assessment of Toyota Electronic Throttle Control (ETC) Systems (Feb. 2011).Both reports should be read in conjunction with each other. As stated in its report, NASA did not find an electronic cause of large throttle openings that can result in unintended acceleration incidents. NHTSA did not find a vehicle-based cause of unintended acceleration incidents other than the physical pedal interference causes that are being addressed by Toyota's recalls. This RQ is closed.
