On [XXX], at the intersection of [XXX] and [XXX] in Los Angeles, [XXX] was operating the subject vehicle when it was involved in a hit-and-run that resulted in a secondary collision with multiple vehicles. Photos (attached) document the significant impact. Emergency responders were required to extricate [XXX]from the vehicle, during which they informed her that the airbags failed to deploy—a fact also documented in the official LAPD report. The failure of the airbag system represents a significant and dangerous malfunction of the vehicle’s safety features. This is particularly troubling in light of Toyota’s recent recall, which affects more than 400,000 vehicles. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)
