I was involved in a frontal collision in which I rear-ended another vehicle. The impact was significant enough to cause a fractured sternum due to the seat belt restraint. However, the front airbags did not deploy. I had a passenger in the vehicle, and we were both wearing our seatbelts at the time of the crash. I am concerned that the airbags failed to deploy despite the severity of the impact and the resulting injury. I believe this may indicate a defect in the airbag system or the airbag control unit (ACU). I am submitting this report to ensure the issue is investigated and to help prevent potential harm to other drivers. After the collision the car was not drivable due to an airbag error message on the dash of the vehicle. It had to be towed away from the scene and will not be repaired. Injury: Yes — fractured sternum Medical Attention Received: Yes — X-Ray and CT Scan indicate a fracture to the upper sternum Police Report Filed: No Crash Photos or Documentation Available: Yes - insurance company has them

I am writing to complain about a serious safety defect in my 2022 Toyota Corolla Cross (VIN [XXX] ). On August 11, 2023, I recently received a letter from Toyota stating that the passenger airbag in my car may not deploy as designed in the event of a crash. This is a serious safety defect, and I am concerned about the safety of my car. I have owned my Corolla Cross for in excess of a year, and I have never been in an accident. However, I am a father with two young children, and I use my car every day to take them to school and activities. I am worried about the possibility of being in a car accident, and I do not want my children to be injured because of a defect in my car. I understand that Toyota is offering to fix the airbag for free. However, I am not comfortable with having a car that has a known safety defect. I would like to request that Toyota recall my car and replace it with a new car. I believe that I am entitled to a new car, because it is not my fault that my car has a safety defect. I am a loyal Toyota customer, and I have always been satisfied with my Toyota vehicles. However, I am disappointed that Toyota has not recalled my car to fix this safety defect. I hope that the NHTSA will investigate this matter and take action to ensure that Toyota recalls my car and replaces it with a new car. Thank you for your time and consideration. The instrument panel may have been produced without the required perforation, which could result in the front-passenger air bag not deploying properly. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 208, "Occupant Crash Protection."
Consequence & remedy
Consequence: An air bag that does not deploy as intended increases the risk of injury during a crash.
Remedy: Dealers will inspect and replace the instrument panel as necessary, free of charge. Owner notification letters were mailed January 26, 2024. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 23TA16. Vehicles previously inspected under 23V-384 will need to return to the dealership for additional inspection.
