I was parking the car at a low speed (2mph) when it suddenly accelerated with a great speed, jumped the curb in front and moved forward and up a small hill. The car collided with a tree before I was able to stop it. All ADAS features were activated in the car and no alarms sounded prior or during the event, which lasted between 1-2 seconds. The vehicle was inspected by Mazda and deemed safe to continue to operate. The dealer did not find a cause for the malfunction and suggested external influence. The inspection took about 3hrs, which included addressing multiple recalls on the vehicle. At time of incident I have owned the car for about two months and did not experience similar issues, however others did according to my web search. I found reports on popular forums describing sudden acceleration of Mazda CX-90 vehicles.

I took delivery of this car about a month ago from the dealership in Kansas. On my way home to Grand Rapids, Michigan I noticed that the driver side rear view mirror was vibrating severely to the point that the image in the rear-view mirror is blurred. This is during daytime driving, at nighttime this issue severely impacts safe driving as I am unable to locate the type of vehicle accurately. Mazda seems to be aware of this issue since 2023 and have been trying to address it through TSB's indicating the dealers to replace the mirror glass. I have taken my car to my local Mazda dealer in grand rapids Michigan, and they have performed the latest TSB on the car, but the mirror continues to vibrate although less than before but still presenting a safety risk while driving. Per the latest conversation with the dealership, Mazda is still continuing to fix a better fix for this issue and I am asked to keep checking each month.

I was traveling at 40mph on August 14 when the engine stopped running suddenly and without warning. I lost all power. I tried pressing the gas pedal and the engine shuddered and sputtered with the speed ranging from 0 mph to about 18mph with no steady duration longer than a second or two. At the same time an engine icon appeared and a message displayed reading engine malfunction. I safely exited the travel lane despite having very little power. I waited 10 seconds and restarted the car and everything worked fine. I called the dealer explained the incident and to made arrangements to have the vehicle serviced. The appointment was for Friday. When I asked whether I should continue driving the car. I was told that so long as the engine icon was not flashing the car was ok to drive. The next day I started the car and did not see any warning message or engine icon. I thought all was ok so I cancelled the appointment. A second incident occurred later in the morning. I called the dealer again and was told to bring the car in that afternoon, Thursday. On the way over to drop off the car a third incident occurred with the same characteristics as the first two. Each time waiting 5 to 10 seconds restored power to the car. One week later the car was returned to me with the diagnosis of low fuel pump pressure problem. I am wary of driving this car on any trip especially on a speed highway for fear this situation will occur when I am driving at 70mph. I appreciate your taking the time to read this description of a very scary and unnerving situation that no one should face.

The contact owned a 2024 Mazda CX-90. The contact stated that while driving at various speeds, a clunking noise would emit from underneath the engine compartment. The vehicle was taken to a dealer where it was diagnosed that the control arm had failed. The vehicle had been repaired. The contact stated 3 days later while driving approximately 35 -45 MPH, the brake pedal was depressed but failed to respond. In addition, the vehicle was attempted to be veered to the left or right and it failed to drive in either direction. As a result, the vehicle crashed into the rear-ended of the vehicle in front. The air bags deployed. The vehicle was towed and deemed totaled. The contact stated that the other party's vehicle was damaged and towed away. A police report was filed. The contact sustained a left leg, right arm, shoulder, wrist, and neck injuries that required medical attention via hospital. The contact's wife was seated in the passenger seat at the time of the crash and sustained injuries to the right shoulder, wrist, and both the left and right foot's toes that required medical attention via hospital. The contact was unsure of any injuries to the occupants of the other vehicle. A dealer was contacted. The vehicle was not diagnosed or repaired. The contact received notification of NHTSA Campaign Number: 24V349000 (Forward Collision Avoidance) however, the part to do the recall repair was unavailable. The contact stated that the manufacturer exceeded a reasonable amount of time for the recall repair. The manufacturer had been made aware of the failure and opened a case. I was pulling into a parking spot. When I do this, my foot is always on the break. I'm an experienced driver (30+ years) and have never had an incident when I felt the car accelerate without my initiation. When I was maybe 5-7 ft from the car in front of me, the car suddenly accelerated (like it was possessed). I immediately slammed on the break and turned the wheel. Thankfully, and amazingly, I avoided hitting the car both in front of me and immediately on my left (the direction I swerved the wheel). No warning lamps, actions, etc were taken by the car itself (other than willing me to smash into the car ahead of me). This incident occurred in a school parking lot. Thankfully, there was no harm to anyone but my confidence in Mazda. But, if I had chosen to pick my children up through the pick-up line, I may have easily hit the car in front of me or a child! Obviously, this can be a tragic situation. Prior to this incident, I have felt that the car regularly does not operate smoothly at slow speeds, especially when accelerating from stop. This is my 3rd Mazda, and I have never felt this lack of control before. I have also noticed that the i-stop works erratically, including when I am moving at 10-30 mph. The light goes on and off when I am slowing down at a light, etc. Plus, when I am stopped at a light, the i-stop will go on for about 20 seconds, then suddenly turn off and the engine will rev up. I shard the stoplight information with the dealer the week after I took possession of the car and was told by the sales manager that this was just normal and a weird part of the electronics of the car. Following this incident, I called the dealer. The service department and manager said that they weren't aware of this issue but took my concern seriously. I then did an online search and found a conversation string on [XXX] with others sharing the same problem. Software in the Approaching Vehicle Audible System (AVAS) Information Sound Box (ISB) amplifier may be missing, which can cause inaccurate, external pedestrian warning sounds.
