The contact owns a 2026 Lexus TX 350. After purchasing and taking ownership of the vehicle, the contact discovered that the vehicle was included in a recall prior to the purchase. The contact notified a local dealer and was referred to the manufacturer. The manufacturer was made aware of the issue and arranged for the vehicle to be inspected or serviced at the dealer. The dealer scheduled a service appointment; however, the dealer later declined to service the vehicle and informed the contact that the VIN was not associated with a recall. The dealer referred the contact to the manufacturer for loaner vehicle assistance. Additionally, the contact stated that the rear passenger-side tire frequently became flat. The contact also stated that there was an abnormal burning odor while coming to a stop, and that the TPMS warning light was illuminated. The vehicle was not diagnosed or repaired. The contact received notification of NHTSA Campaign Number: 26V437000 (Power Train); however, the part for the recall repair was not yet available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the failure. The approximate failure mileage was 3,647.

On Saturday, August 22, 2026, while traveling on the highway with my two toddler-aged children in the vehicle, the temperature gauge entered the red zone and the vehicle automatically reduced power due to overheating. This forced me to navigate a power-reduced vehicle on a highway with minor children — a serious safety hazard. This incident occurred just five days after my vehicle was in the Sewell Lexus service department on August 17, 2026 for its 10,000-mile service and a warranty repair to the rear evaporator case (a known design defect). The proximity of this safety incident to that service visit raises significant concerns about whether the August 17 service contributed to or caused the overheating event.

We were traveling 80 miles west of Fargo ND on I94 when we heard a loud bang. It became clear immediately from the wind noise that the sunroof had broken. We did not hear or see that anything was dropped on it. Thankfully the shade was closed. We pulled off at the next possible stop to inspect and noted a large whole in the sunroof and that the glass bowed outward suggesting the explosion was an outward force rather than an item falling on the car. There were no warning lights etc. prior to this happening. This was a very frightening experience and had the shade not been closed our family including young son could have been hurt or even blinded from the falling glass. We called the dealer (we only had the car for 3 weeks) and they were able to get the car in right away to look at it but cannot guarantee the window is covered under warranty.

On Juky 7, 2026 @ 10:30pm a catastrophic safety failure occurred involving a 2026 Lexus TX 350. The vehicle experienced a complete electrical and software failure while an occupant (my mother) was inside. All electronic door release buttons failed simultaneously, completely locking the occupant inside the vehicle. The emergency manual double-pull override mechanism on the interior door handles failed to mechanically release or open any of the doors. Concurrently, the climate control system locked on high heat, causing the cabin temperature to rise rapidly. Due to the total entrapment and extreme heat, the occupant began losing consciousness, and her blood pressure spiked to a dangerous level of 188/100. Family members outside the vehicle were unable to open the doors or bypass the electronic locks from the exterior. Emergency services were called to the scene. The local Fire Department had to perform an emergency extraction to rescue the occupant from the vehicle. The occupant required immediate medical attention for severe heat stress and a hypertensive crisis.

I purchased my vehicle 2026 Lexus tx350 from Lexus of Manchester located in [XXX]. As soon as I left the lot I noticed the hood would shake violently and almost pop up which signaled the safety latch alarm. The alarm would signal multiple times throughout a highway trip. Most noticeable around 70 mph. The dealership was helpful and set up an autobody repair where they added adhesive into the hood. Unfortunately the hood flutter is still present and it’s only a matter of time before the adhesive wears down. After conducting research Lexus issued a bulletin repair acknowledging the fault in engineering regarding the aluminum light hoods. The hood is clearly to light and this is an obvious engineering defect which should signal recalls. I have read thousands of similar cases and Toyota is failing to take accountability of this problem. They need to replace the hoods instead they are refusing to acknowledge this is a recall and sneakily sending vehicles out to autobody shops. I have read the Lexus GX and TX both have the same problem. It is not safe to have a hood flutter like that it puts way to much strain on the hood safety latches and eventually they will fail. I do find it absurd and ridiculous that a brand new 70,000 SUV has such a problem. My hope is submitting this complaint it helps force Toyota do the right thing and take accountability for their engineering defect. The contact owns a 2026 Lexus TX 350. The contact stated that after an extended driving period, the vehicle was parked in the garage, and the contact became aware of a strong gasoline odor. The fumes caused persistent headaches that lasted two days. The contact had not sought medical attention. Upon inspection, the owner was unable to locate a visible fuel leak. The vehicle was moved to the driveway and was later towed to the dealer. After an extensive diagnostic test was performed by the dealer, the cause of the failure was not yet determined. The vehicle remained with the dealer and was not repaired. The manufacturer was notified of the failure, and the contact was provided a case number. The failure mileage was 3,563.

The Lexus TX 500h is equipped with a 6-speed sequential-shift automatic transmission, which is paired with the Lexus DIRECT4 all-wheel drive system. This setup utilizes a 2.4-liter turbocharged hybrid powertrain to deliver 366 horsepower and 406 lb-ft of torque. During many occasions, the transmission on the 2026 Lexus TX500h would shudder during acceleration or deacceleration. I noticed it happening around 25-40mph mostly and it has happened more than once. I have demonstrated the issue to the dealer on Wed, May 20th around 10:15am Pacific but they didn't know how to address the issue. I did a ride along with the service tech from Lexus of Pleasanton and it happened again during acceleration. The dealer noted that there were no error code when they scanned the system and no observed leaks. I have a video clip but was unable to upload it. The issue with transmission shift thump when this issue happen feels like the transmission locked up. It would shake the vehicle and lose power while accelerating before it starts to gain power back in 1-2 seconds. This causes a safety issue since that pause in acceleration thus this can be dangerous while merging onto a freeway or areas with lots of traffic. I noticed the same issue on some occasions, it would shake during deceleration as well. This issue is intermittent. Maybe Lexus/Toyota can address this with the transmission shifting update. IMO, It's a safety issue that needs to be looked and fixed by Lexus/Toyota. Dealer asked me to come back only. They reset my information screen to address a sliding driving issue where when the power is turned off on the vehicle the seats will go back and immediately go forward.

To Whom It May Concern, I am writing to file a formal safety complaint regarding my 2026 Lexus TX 350 F Sport (VIN: [XXX] ), purchased from Treasure Coast Lexus in Florida in December of 2025. Shortly after purchasing the vehicle I noticed significant hood flutter at 60-70 MPH. This is visually distracting and poses a safety concern while driving. Despite three separate repair attempts at the dealership in the past two months the defect has not been resolved. Lexus corporate has since informed me that nothing further can be done to correct the issue. I am aware that this is a known issue across the Lexus TX lineup and that a related Technical Service Bulletin (L-SB-0013-25) was issued in February 2025 for the GX 550, which shares the same hood construction. I am requesting that NHTSA investigate this defect as it affects driver safety and may impact other owners of the 2025 and 2026 Lexus TX 350. Thank you for your attention to this matter. The vehicle is a brand new Lexus TX. The primary issue involves a brake system malfunction. While driving, a “BRAKE” warning message appeared on the dashboard. At the same time, the brakes were actively applied, and this occurred multiple times in succession during the same driving event. At the time of the incident, we were not aware that the vehicle should not be driven when this warning appears and drove the vehicle home. After reviewing the owner’s manual, we learned that the vehicle should not be driven under these conditions. The vehicle was then towed to the dealership for inspection. This event posed a significant safety risk. Repeated, unintended brake application while driving could result in sudden deceleration, loss of vehicle control, or increased risk of being struck by other vehicles, increasing the risk of a crash, injury, or death. Prior to this incident, the vehicle also experienced a complete failure of the camera/display system, where the screen went black, impacting driver visibility systems. The vehicle is currently at the dealership and under inspection by the manufacturer’s authorized service center. The issue has not yet been confirmed or reproduced at the time of this report. There were no prior warning messages before the brake warning appeared. The failure occurred unexpectedly. The component(s) responsible for the failure are currently unknown, pending diagnostic evaluation by the dealership. The vehicle is available for inspection upon request.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2026 Lexus TX Plug-In Hybrid, TX Hybrid, TX, Toyota Grand Highlander Hybrid, and Grand Highlander vehicles. The rear axle carriers may have been manufactured with insufficient material strength and could break.

Consequence & remedy

Consequence: A broken rear axle carrier can cause a sudden loss of vehicle stability and control, increasing the risk of a crash.

Remedy: Dealers will inspect the rear axle carrier sub-assembly code, and replace the sub-assemblies as necessary, free of charge. Owner notification letters were mailed September 8, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26TB13, 26TA13, 26LB08, and 26LA08.

The load carrying capacity modification label may display inaccurate added weight values. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims."

Consequence & remedy

Consequence: A vehicle with an incorrect maximum capacity weight value may be overloaded, which can increase the risk of a crash.

Remedy: Dealers will replace the load carrying capacity modification label, free of charge. Owner letters were mailed May 26, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB04, 26LA04, 26TB07, and 26TA07.

The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.
