Chest Clip, Buckle, Harness

I have a Nuna Rava carseat that has malfunctioning harness straps. The straps can be pulled loose even after proper buckling and tightening. My child is at risk of injury, bodily harm, or death in the event of a car accident due to the harness straps failing to stay tight. The car seat has not been inspected.

I recently bought this vehicle and the drivers seat height does not raise approropriately. I have brought it to the dealership for them to take a look and they say that’s how it’s designed however the passenger seat height goes much higher. I called Lexus corporate who mentioned there is nothing they can do. I have driven the LX and RX models that do not have this issue. This vehicle is only 1 year old and I believe this is a safety concern. I am unable to see above the hood of the car and I am [XXX]. The contact owned a 2024 Lexus TX350. The contact stated while driving 15-20 MPH from a complete stop and making a left turn, another vehicle who was driving at an undisclosed speed, crashed into the front passenger's side end of the vehicle, causing the front end of the vehicle to separate. The contact shifted into park(P), and the vehicle stopped on the road. The other vehicle continued to accelerate and veered off into a ditch. The contact did not recall whether there were warning lights illuminated after the incident. The contact stated that several messages indicating the brake system, the Advance Autosonic detection, the Intelligence alert, the ABS, the parking brake, the electric power steering, the electronic stability control systems malfunctions, and other malfunctions were displayed on the vehicle mobile app. The contact stated that the front driver's side air bag and the knee air bag failed to deploy as intended and partially deployed. The contact sustained whiplash and hit her knees against the dashboard during the incident. The contact sustained neck, hips and legs pain. The ambulance transported the contact to the hospital, where a CT scan and X-Rays were performed, and the contact was informed that an internal neck injury was found. Additionally, the contact's legs had become swollen between the knees and the ankles. The contact stated that she was in the process of receiving further medical assistance. A police report was filed but the contact did not have the police report information available. The vehicle was towed and totaled by the insurance company. The dealer was notified of the failure and referred the contact to the manufacturer for assistance. The manufacturer was notified of the failure and a case was opened. The manufacturer referred the contact to the NHTSA Hotline for further assistance. The failure mileage was approximately 10,750.

My Lexus TX 500h has an intermittent problem. It seems to have trouble shifting between 1st gear and second gear. Sometimes when I try to drive from a stop, the car will go a tiny bit and then feels like it switches into neutral instead of the next gear, leaving me stopped in the middle of traffic or an intersection. I have been stuck pulling out of my street onto a busy street with traffic bearing down on me. I have also started rolling backwards on a hill when my foot is on the gas and the car is in drive. It seems that the car puts itself into neutral vs second gear. I have taken it into the dealer twice for this and they “cant duplicate the problem” so tell me to go home and bring it back if it happens again. It keeps happening and it’s a huge safety issue. I’m not sure where I go from here.

Recall on my Lexus tx, where originally I was working with the dealership until a remedy was established but it’s been 4 months and still nothing. Then the Lexus of Orlando dealership has lied and treated me like crap because I don’t feel safe in a car that has a high ticket price. Refusing to do anything to help me get out of the car with statements such as”you’re going to have to eat the depreciation” this is sickening to know I have to deal with this, not to mention my insurance company has threatened to drop coverage on the car!

Failure of component are the curtain shield airbags. The safety of my customers are at risk when car is being rented out due to this concern the Lexus TX has not been in use, for safety concerns. This issue is a known safety risk which was known to the public on June 20th, 2024.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2025 Lexus NX350, NX250, 2023-2026 RX350, and 2024-2026 TX350 vehicles. The rearview camera image may not display when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview image that does not display reduces the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the rearview camera software or replace the rearview camera, as necessary, free of charge. Owner notification letters were mailed May 15, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 26LB02 and 26LA02.

25V744000 · Back Over Prevention:display Function

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

25V595000 · Electrical System: Instrument Cluster/panel

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2023-2024 Toyota Venza, 2023-2025 RAV4 Prime, RAV4, Highlander, GR Corolla, Crown, 2024-2025 Lexus TX, LS, Toyota Tacoma, Grand Highlander, and 2025 Lexus RX, Toyota Crown Signia, Camry, RAV 4 Plug-in Hybrid (PHEV), and 4 Runner vehicles. Due to an error in the instrument panel software at vehicle startup, the instrument panel may fail to display vehicle speed, brake system, and tire pressure warning lights.

Consequence & remedy

Consequence: An instrument panel display that does not show critical information can increase the risk of a crash or injury.

Remedy: Dealers will update the instrument panel software over-the-air (OTA) for non-PHEV vehicles, free of charge. For PHEV vehicles, dealers will inspect the instrument panel assembly, and either replace it, or update the software, free of charge. Owner letters were mailed December 5, 2025. Additional letters will be sent, anticipated in May 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB08 and 25TA08. Lexus' numbers for this recall are 25LB05 and 25LA05.

24V461000 · Air Bags:side/window:curtain

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2024 Toyota Grand Highlander, Grand Highlander Hybrid, Lexus TX350, TX500 Hybrid, and TX550 Hybrid+ vehicles. The driver and passenger side curtain air bags may not unroll as intended and could partially deploy outside of an open window. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 226, "Ejection Mitigation."

Consequence & remedy

Consequence: An improper deployment of the side curtain air bag can reduce occupant protection during a crash, increasing the risk of injury.

Remedy: Dealers will replace the driver and passenger curtain shield air bag assemblies, free of charge. Owner letters were mailed November 4, 2024. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's numbers for this recall are 24TB09/24TA09 and Lexus' numbers are 24LB05/24LA05.
