COMPONENT/SYSTEM: Air Conditioning (HVAC) / Climate Control System and closed-loop refrigerant lines. The vehicle and all climate control components are available for immediate inspection. SAFETY RISK: Total loss of cooling in peak summer heat (90°F-95°F+) causes cabin temperatures to rapidly exceed 110°F. This sudden heat trapped in the vehicle caused me (the driver) acute heat exhaustion, culminating in a medical fainting episode. The vehicle regularly transports my newborn baby and toddler, subjecting them to severe heatstroke risks. Additionally, an ongoing 4-year chemical refrigerant leak inside an enclosed EV cabin creates an active toxic inhalation hazard for all passengers. DEALER CONFIRMATION: Yes. The failure has been repeatedly confirmed and documented by authorized Hyundai dealerships, including Paramus Hyundai, Garvey Hyundai, and Liberty Hyundai. Technicians repeatedly measured zero refrigerant charge/pressure, performed UV dye tests, and completed major repairs including AC line and radiator rebuilds without resolving the leak. INSPECTION HISTORY: Yes. Inspected multiple times by authorized Hyundai dealership technicians under official Corporate Case Nos #42549212, #41020884, and #23781133. WARNING LAMPS/SYMPTOMS: No dashboard warning lamps or error messages ever appeared. The only symptom was the AC suddenly blowing boiling hot air as refrigerant leaked out. This failure first appeared in Summer 2022 (shortly after vehicle purchase) and has recurred every summer for 4 consecutive years despite repeated warranty repair attempts.

I purchased my 2022 Hyundai Kona on August 28, 2024, and I am the first registered owner. Only 48 days after purchase, the vehicle suddenly broke down without warning while stopped at a red light in [XXX] after traveling from [XXX] . The vehicle had to be towed. Since then, the vehicle has repeatedly experienced dangerous engine and timing-related issues despite major repairs, including an engine replacement, timing chain repairs, and multiple dealership inspections. The most dangerous incidents involve sudden power loss and large clouds of smoke while driving. On January 1, 2026, while attempting to pass a semi-truck on a multi-lane highway, the vehicle suddenly lagged, lost acceleration power, and emitted a large cloud of white smoke from the exhaust. This created an extremely unsafe situation because I was actively passing traffic at highway speeds and could have been struck by surrounding vehicles. I have also experienced these failures late at night on busy highways while traveling between [XXX] and [XXX] . In March 2026, the vehicle again emitted a large cloud of smoke and resumed lagging during acceleration. On April 23, 2026, Hyundai of Meridian informed me they personally witnessed the vehicle emit another large cloud of black smoke during test driving. These smoke events can reduce visibility for surrounding drivers and create hazardous roadway conditions. The vehicle has spent approximately 149 days in repair facilities, nearly 24% of the time I have owned it. Multiple dealerships documented timing chain slack, cam timing issues, and major engine concerns. Despite the recurring failures and safety concerns, Hyundai denied replacing the engine again and denied buying back the vehicle. I no longer feel safe operating this vehicle due to the risk of sudden power loss, engine failure, and smoke emissions while driving in traffic. In January 2026, my engine light illuminated and the vehicle lost power. The dealership found a sudden loss of compression across all four cylinders, requiring a total engine replacement. This occurred without warning, only two months after a routine oil change at the same facility where no issues were reported. Despite opening a case with Hyundai corporate, there has been no progress. My research indicates that while NHTSA Recall 21V301 (Safety Recall 203) specifies 2019–2021 models, the 2022 model shares the same engine design and identical reports of stalling and compression loss. My vehicle has been at the dealership for three months with no resolution. The dealership says they are running tests requested by tech line without providing documentation of the tests or the results. Although, the initial test performed indicated that it was a compression ring issue.

I found a lot of shaved metals when I did the oil change for the Gear Reduction. It made a lot of noises when driving or the wheel of fortune sounds whether the car is in Regenerative Braking. The sounds developed as time goes on but Hyundai failed to recall this issue. I found this has been an ongoing issue since 2019 when Hyundai make this Kona EV, but their engineers didn’t fix this type of issue and continue to manufacture the Kona EV with the same Gear Reduction Assembly and it caused a lot of noises with these shaving metals found in the oil changes. This is abnormal for a GRU to have so many metals found in the oil. Thus, it made a lot of noises. Now my car can’t even drive because of this. The GRU has been broken because of this issue that Hyundai didn’t even fix since 2019.

On Saturday, [XXX], while driving on the highway, the oil plug on my vehicle suddenly fell out, causing all the oil to drain rapidly. This created an extreme safety hazard not only for myself but also for other drivers on the road. I did not notice the oil spillage until the oil light on the dash appeared. At the time, I was in the leftmost lane traveling at highway speeds, so I could not stop immediately. I had to carefully veer across five lanes of traffic and navigate out of a construction zone before I was able to pull over safely. The engine lost lubrication during this process, and I was forced to have the vehicle towed to Ed Napleton Hyundai in Hazelwood, the same dealership where I purchased and serviced the car, for inspection and repairs. This incident occurred just three weeks after I had an oil change performed at the same dealership. I have been attempting to contact the dealership for updates on the status of my vehicle and the investigation into potential engine damage for the past three days, but I have received limited information until today, when the service manager, Jerry, stated that they installed a new plug and added oil, and since it's not ticking, it's okay. However, having worked in the automotive industry for years, I know that a full teardown is necessary to properly check for damage, including inspecting the bearings, rings, cylinder walls, and turbo for signs of oil starvation. This service was not performed. Additionally, Jerry informed me that speaking to someone higher up would do absolutely nothing for me. This is not an isolated issue. Over the course of my ownership, the dealership has damaged my oil filter by over-tightening and stripping it on two separate occasions during routine maintenance, requiring me to return to the facility each time. On both instances, I had to wait an additional 2 to 3 hours while they installed replacement parts to correct their errors. NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

The National Highway Traffic Safety Administration (NHTSA) received a letter dated July 13, 2022, petitioning the Agency to initiate a safety defect investigation into loss of motive power allegations attributed to oil drain pan assembly failures on certain Hyundai Motor America, Inc. and Kia America, Inc. vehicles spanning Model Years (MY) 2005-2021. The Petitioners cited examples of complaints indicating that the subject vehicles were experiencing rapid loss of engine oil due to oil drain plug back outs, resulting in loss of motive power and/or catastrophic engine damage. Additional complaints were provided which alleged that the oil drain pans lack sufficient structural integrity and are susceptible to cracking. The petition itself can be reviewed at NHTSA.gov under ODI number 11481745. On September 12, 2022, the Office of Defects Investigation (ODI) opened Defect Petition (DP22-003) to evaluate the Petitioners’ request. The Petitioners have provided additional complaints within four supplemental letters dated January 31, 2023, March 21, 2023, May 23, 2023, and August 4, 2023. ODI’s evaluation included the review and analysis of the complaints and supporting information submitted by the Petitioners, as well as additional complaints submitted directly to NHTSA which referenced either the oil drain plug or a crack in the oil pan. After reviewing the available data, ODI has not identified evidence that would support opening a defect investigation into the oil drain pan assemblies of the subject vehicles. Based on a review of the available information, a loss of motive power resulting from an oil drain plug back out has not occurred between the initial sale of a vehicle and the completion of its first oil change. If during an oil change service, a technician fails to remove the existing drain plug gasket before installing a new gasket, it is likely that a proper seal between the drain plug and the oil pan will not be made. An improper seal between the drain plug and oil pan could lead to the backing out on the drain plug, resulting in the sudden loss of oil while driving. Additionally, the provided examples of oil pan cracks were localized to the immediate vicinity of the oil drain plug and/or appeared to be the result of direct-contact with a tool during servicing.  There is no information to support that a vehicle-based defect exists relating to the oil pan assembly, and instead the documented incidents are related to the failure to follow the proper maintenance procedures during oil change service. In October 2023, Hyundai published a Technical Service Bulletin (TSB) that provided detailed instructions for oil change service on its gasoline powered vehicles, specifically highlighting the need to remove and replace the existing drain plug gasket prior to reinstalling the drain plug. After a thorough review of the material submitted by the petitioner, the information already in NHTSA's possession, and the potential risks to motor vehicle safety implicated by the petitioners' allegations, NHTSA does not believe that a formal investigation is warranted. Therefore, the petition is denied. A Federal Register Notice (FRN) further detailing NHTSA’s reasons for denial of the petition will be published.
