We where approaching the ramp to the bridge and our dashboard indicated front camera malfunction - ever since we’ve taken it to the dealer numerous times and the problem has not been resolved. It’s a safety issue. In addition also the cross camera doesn’t work sometimes and the dealer told us they know of this issue.

This car has a problem with the emergency brake system activating when there is nothing in any danger zone. We have owned this car for about a year, and when slowly reversing, we have been repeatedly annoyed by the warning lights & sounds going off & emergency brakes locking the wheels. Each time we check the cameras, mirrors and even get out of the car to look, but there was nothing anywhere near that would trigger such an event. This happened 4-5 times over the past year, and we assumed this was just an ultra-sensitive feature which was annoying and startling when it happened, but generally harmless. Until a similar issue with the brake system happened recently while we were coming to a stop while driving forward, except this time after the warnings went off and brakes locked, the car then shot forward, with my foot still on the brake, and hit the car in front of me. Then it locked up and shot forward again 2 MORE TIMES. I've detailed that event in another complaint on this site, but I wanted to put the original emergency brake problems on the record, in case they end up being related.

I own a 2024 Honda Prologue (VIN: [XXX]). Since approximately XXX, the vehicle has experienced recurring failures involving critical driver-assistance and safety systems. The problems include: • "Forward Collision System Unavailable" warnings • "Adaptive Cruise Control Temporarily Unavailable" warnings • Automatic braking while reversing when no obstacle is present • Repeated loss of confidence in the operation of safety-related driver assistance systems The vehicle has been presented to authorized Honda service facilities approximately four times for diagnosis and repair. The vehicle has spent approximately 20 to 22 days out of service. Despite multiple repair attempts, the problems continue. The vehicle has never been involved in a collision. A prior insurance claim involved hail damage only and was repaired. I do not believe the hail damage is related to the recurring failures of the driver-assistance systems. Because these systems are intended to help avoid collisions and improve vehicle safety, I am concerned that their repeated failure may create a safety risk. The unexpected braking while reversing is particularly concerning because it occurs even when no obstacle is present. I have opened Honda Case Number 162-31526 with American Honda Motor Company and have requested assistance, including consideration of a repurchase, replacement, or other resolution. I am submitting this complaint so that the issue can be documented and reviewed as a potential safety-related defect. The contact owns a 2024 Honda Prologue. The contact received notification of NHTSA Campaign Number: 26V306000 (Back Over Prevention). The contact stated that while in reverse, the rear-view camera was glitching, and the red triangle on the rear-view camera screen indicated that there was an object behind the vehicle; however, there was no object behind the vehicle. The vehicle was taken to the dealer for the recall repair however, the part to do the recall repair was not available. The manufacturer was not notified of the failure. While getting ready to parallel park the car, the entire screen system (dashboard, center console) went completely blank. The contact owns a 2024 Honda Prologue. The contact stated while driving at 45 MPH with her wife in the vehicle, the vehicle instrument panel blacked out and displayed blank screen independently than minute and three seconds with instrument panel began to reset it self. A dealer was contacted. The vehicle was taken to the dealer where it was diagnosed that the instrument panel and front camera needed to replaced. The vehicle was repaired as they performed a software update and the front camera was replaced. The contact was relating the failure to NHTSA Campaign Number: 26V112000 (BACK OVER PREVENTION, ELECTRICAL SYSTEM). The manufacturer was notified of the failure and they informed her that they reviewing the case. The approximate failure mileage was 20,000 .

I have had two safety issues with this vehicle where there were system failures that did not record DTC codes. And an additional failure that does not record codes and cannot be duplicated. After leasing the vehicle for seventeen months, and during repair for an unrelated non-safety issue, it was discovered that the steering wheel airbag had a loose connection. No warning appeared at any time on the instrument panels. As well the one pedal function would switch into regular drive mode, as in a gas powered vehicle, randomly. After multiple tries and incidents the dealer service department has been unable to duplicate. Then on March 6 of this year I bought in the vehicle yet again for this one pedal failure, and was notified there was an existing recall. This software update was performed. On March 17, just 11 days after the update, while backing into my garage with the garage door going up the windshield wiper was clipped by the door, destroying both the wiper and shattering the back window. I have backed into my garage thousands of times and never had an issue. Why this happened on this date I do not know. What I do know is that the vehicle gave no warnings and did not break for the object. At the time my dogs were clipped into their seat belt harnesses in the center well. By a miracle no glass shattered beyond the back storage area and I was able to safely remove them. I checked after and all of the collision warning systems were listed as active, including automatic braking for rear collisions. I had the dealership run scans twice and all scans showed no DTC codes and all systems active. I can only assume that all prior updates have failed, and there is a substantial defect to the vehicles ability to communicate. I have a copy of the scan performed. The second scan was run on April 9 because the dealership failed to provide a printed copy of the first. I have that second scan. My vehicle fails to communicate or activate essential information, a safety hazard.

The contact stated that while driving on several occasions at various speeds, infotainment screen and instrument panel went black and did not work as intended. The vehicle was turned off for approximately five minutes and then turned on the infotainment screen and instrument panel began working. The vehicle was taken to a dealer where it was diagnosed that a software update was needed. The vehicle was repaired but the failure reoccurred the next day. The manufacturer was not notified of the failure. The failure mileage was a 12,100.

Additionally, while making a left turn, there was an abnormal clicking sound coming from the front passenger’s side of the vehicle. The vehicle was taken to the dealer several times. The contact was informed that the front axle needed to be replaced. The contact was advised not to fully charge the battery. The vehicle was not repaired. The contact was informed that the Warranty Department had to approve the request for a loaner vehicle, to allow the repairs to be performed. The contact stated that the request was submitted in December 2025, however the vehicle was still not repaired. The contact received notification of NHTSA Campaign Number: 26V112000 (Back Over Prevention, Electrical System). The manufacturer was made aware of the failure. The failure mileage was approximately 20,000.

American Honda Motor Company (Honda) is recalling certain 2024-2025 Honda Prologue and 2024 Acura ZDX vehicles. The rearview camera screen may display a distorted or blank image.
Consequence & remedy
Consequence: A rearview image that does not display correctly reduces the driver's view behind the vehicle, increasing the risk of a crash or injury.
Remedy: Dealers will replace the rearview camera, free of charge. Interim letters, notifying owners of the safety risk, were mailed July 7, 2026. Additional letters will be sent once the remedy is available, anticipated in late 2026. Owners may contact Honda's customer service at 1-888-234-2138. Honda's numbers for this recall are DO5 and RO6. The information in your report suggests that Honda (American Honda Motor Co.) may have been aware of this issue more than five business days before filing a report with NHTSA. Consequence: A right front lower control arm fracture can cause loss of vehicle control, increasing the risk of a crash.
