The contact owns a 2021 Ford F-350. The contact stated that while the vehicle was hauling a trailer with a tractor, the tailgate unintendedly opened, causing damage to the tailgate. The vehicle was taken to the dealer where it was diagnosed that since there was no water leaking under the front passenger's side floor mat, the contact needed to pay out-of-pocket for tailgate replacement. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 23,000.
I have owned the truck for just over a year and in that time occasionally when I put my vehicle in park and get out of the truck it will start moving without me touching it. I have owned the truck for just over a year and in that time occasionally when I put my vehicle in park and get out of the truck it will start moving without me touching it. It has happen 5 or 6 times within the year. I took it to the dealer and they told me there is nothing they can do unless the could recreate the problem. This situation is extremely dangerous especially if someone is in the way. One time when it did it the truck ran into my camper and broke the tongue jack. Another time it took off and ran into my hunting camp gate severely bending a solid steel gate. I was just about to step in front of the truck to go unlock the gate when it happened. One second longer I would have probably been killed. The last time it happened was about 2 months ago and after putting it in park twice it continued to roll and I had to put my plow down to keep it from moving

The contact disconnected the trailer and drove the vehicle to the dealer. The dealer could not duplicate the failure with the trailer being disconnected. The passenger sustained broken ribs, right ankle sprain or fractured, and the seatbelt bruised the chest. The driver struck the steering wheel and sustained chest pains, lower, mid, and upper back issues, and neck injuries. Medical attention was provided for both occupants. The driver of the Toyota succumbed to his/her injuries. A police report was filed. There was no reported fire. The vehicle was towed to an independent lot. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure and referred the contact to NHTSA. The failure mile was approximately 6,600.

This truck on a daily basis, shuts down with out warning. Check engine light comes on some time, some times get P0191, P0171 and other codes dealing with fuel rail and pressure. Vehicle had less than 10000 miles when started. Dealer don't know what to do. Shuts off when you are in a turn and leaves you with out steering, and shuts off when coming to a stop sign, or trying to park. Fuel filters have been changed 2 times in 2 months. When my F-350 had only 2,265 miles on it, I was getting in my truck and closed the door. When it closed, the rear window shattered. There was no object that had been thrown at or through the window(no lawn service around that day, etc.). I immediately called the local Ford dealership(Landers Ford in Collierville, TN) and was told that the window would not be covered under warranty. I then called the Ford customer service # on the ford.com website and they stated that this was not a quality issue and that it would not be covered under warranty. It is clear by looking at the picture that the window shattered out as though the pressure in the truck when the door closed caused the break. This occurred on 9/10/21 and I took the truck to Safelite on 9/13/21 to have the window replaced at my own expense.

An unknown electrical issue possibly sensor, module or wiring related causes several faults in the system and they appear on the dash. These faults disable many safety features built in to the vehicle and a few convenience features as well. The faults displayed are: "pre-collision assist not available", "hill start assist warning", "service advance trac" and "drive mode not available" When this happens randomly the vehicle will stop producing forward acceleration and slow down very quickly as if I took my foot off the accelerator. When this occurs it is a dangerous situation made worse at higher speeds and I must quickly move off the road without causing an accident. The vehicle has been to the dealer 4 times since new (4765 current miles) and there is no definite solution at this time.

Biodiesel deposits may form on the pump drivetrain roller components, which could lead to failure of the high pressure fuel pump.
Consequence & remedy
Consequence: High pressure fuel pump failure can cause a loss of drive power, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2020-2022 F-250 SD, F-350 SD, and F-450 SD vehicles. The 360-degree view camera system may not display a rearview image properly in certain lighting conditions.
Consequence & remedy
Consequence: A rearview camera image that does not display properly can reduce the driver’s view behind the vehicle, increasing the risk of a crash.
Remedy: Dealers will update the image processing module software, free of charge.This will be a phased campaign, with the remedy becoming available in different phases based on model years. Interim letters notifying owners of the safety risk were mailed on October 30, 2025. Additional letters will be mailed once the remedy is available on each phase. The front wheel hub extenders may be improperly attached to the front rotor hub assemblies, which could result in separation of the wheels and wheel extenders from the vehicle.
Consequence & remedy
Consequence: A detached wheel and wheel extender can increase the risk of a crash.
Remedy: Owners are instructed not to drive their vehicles until the torque of the wheel extender nuts can be verified. The Office of Defect Investigations (ODI) has received 27 complaints alleging diesel fuel leaks due to a fracture in the secondary fuel filter housing in model year (MY) 2015-2021 Ford Motor Company (Ford) F-250, 350, 450, and 550 Super Duty vehicles equipped with 6.7L diesel engines. Of these complaints, 12 describe an engine compartment fire. The proximity of the secondary fuel filter to high heat sources creates a potential for autoignition thermal events and fires. ODI has contacted the vehicle manufacturer, Ford, and the filter manufacturer, Allevard Sogefi USA (Sogefi), and learned that the low-pressure fuel system is capable of a maximum operating pressure of 8.5 Bar, while the secondary fuel filter is only tested to seven (7) Bar during the manufacturing process. Additionally, changes starting in 2022 by both Ford and Sogefi align with a lack of similar complaints on MY 2022 and newer vehicles. ODI is opening this Preliminary Evaluation (PE) to evaluate the severity of the potential problem and to fully assess the potential safety-related problems. ODI has contacted the vehicle manufacturer, Ford, and the filter manufacturer, Allevard Sogefi USA (Sogefi), and learned that the low-pressure fuel system is capable of a maximum operating pressure of 8.5 Bar, while the secondary fuel filter is only tested to seven (7) Bar during the manufacturing process. Additionally, changes starting in 2022 by both Ford and Sogefi align with a lack of similar complaints on MY 2022 and newer vehicles. ODI is opening this Preliminary Evaluation (PE) to evaluate the severity of the potential problem and to fully assess the potential safety-related problems. The Office of Defect Investigations (ODI) has received 27 complaints alleging diesel fuel leaks due to a fracture in the secondary fuel filter housing in model year (MY) 2015-2021 Ford Motor Company (Ford) F-250, 350, 450, and 550 Super Duty vehicles equipped with 6.7L diesel engines. Of these complaints, 12 describe an engine compartment fire. The proximity of the secondary fuel filter to high heat sources creates a potential for autoignition thermal events and fires.
