Had to slow way down almost causing myself to be rear-ended on one occasion and the other time it shifted the truck in the other lane. Both situations caused me to have NO CONTROL of my vehicle. The contact owned a 2015 Ford F-250. The contact stated their husband was driving at 70 mph, the vehicle was in cruise control and the driver attempted to get out of cruise control by depressing on the brakes, however, the cruise control did not disengage and the vehicle began to accelerate. The driver attempted to manually turn off the cruise control from the steering wheel but it failed and continued to accelerate. The contact attempted shifting out of the drive gear shift position and taking the key out of the ignition to turn off the vehicle but the vehicle still was accelerating in speed which caused the vehicle to crash and hit the rear end of the vehicle in front. Both of the driver's feet were on the brake pedal where excessive force was used but the vehicle continued to accelerate leading to a crash. The contact mentioned that the vehicle independently increased in speed up to 100 MPH or more during the failure. The vehicle was finally able to stop once it crashed into a guard rail. The front driver's side tire and door were detached from the vehicle. The air bags did deploy. No warning lights illuminated from the vehicle. The driver sustained injuries for a fractured left kneecap from the dashboard collapsing and bruises on their left leg and chest. The contact sustained injuries as well with a broken left big toe on their foot and bruises on their leg and upper chest. Medical treatment was at the scene and the ambulance did take the contact and the driver to a local hospital. A police report was filed. The vehicle was towed to a salvage yard where it was deemed a total loss. The vehicle was not diagnosed. The manufacturer was not aware of the failure. The failure mileage was approximate 172,000.

25V572000 · Back Over Prevention: Sensing System: Camera

Ford Motor Company (Ford) is recalling certain 2015-2019 Lincoln MKC, Mustang, F-350 SD, F-250 SD, F-450 SD, 2015-2017 Lincoln Navigator, Expedition, 2015-2018 Edge, Transit Connect, 2016-2019 F-550 SD, Transit, 2017-2019 Econoline, and 2019 Ranger vehicles. The rearview camera may display a distorted, inverted, or blank image when the vehicle is in reverse.

Consequence & remedy

Consequence: A rearview camera that displays an inverted, distorted, or blank image can reduce the driver’s view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will inspect and replace the rearview camera as necessary, free of charge. Owner notification letters were mailed January 13, 2026. Ford owners will receive letters in phases, beginning January 2026 and June 2026. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 25S89. This recall expands previous NHTSA recall number 25V270. Vehicle Identification Numbers (VINs) involved in this recall will be searchable on NHTSA.gov beginning September 8, 2025.

Ford Motor Company (Ford) is recalling certain model year 2015 F-250, F-350, F-450, and F-550 Super Duty trucks manufactured September 13, 2013, through March 28, 2014, and equipped with 6.2L gasoline or 6.7L diesel engines and 6R140 transmissions. An error with the transmission control software may be such that while shifting from "Park" to "Drive" the vehicle may display "Drive" but possibly engage "Reverse" for 1.5 seconds before engaging "Drive." Therefore, the vehicles do not conform to Federal Motor Vehicle Safety Standard (FMVSS) number 102, "Transmission Shift Lever Sequence, Starter Interlock, and Transmission Braking Effect."

Consequence & remedy

Consequence: If the vehicle is in "Reverse" while the while the gear selection shows "Drive," unexpected movement could occur, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will update the transmission software, free of charge. The recall began on May 6, 2014. Owners may contact Fords customer service at 1-800-392-3673. Ford's number for this recall is 14C04.

The Office of Defect Investigations (ODI) has received 27 complaints alleging diesel fuel leaks due to a fracture in the secondary fuel filter housing in model year (MY) 2015-2021 Ford Motor Company (Ford) F-250, 350, 450, and 550 Super Duty vehicles equipped with 6.7L diesel engines. Of these complaints, 12 describe an engine compartment fire. The proximity of the secondary fuel filter to high heat sources creates a potential for autoignition thermal events and fires. ODI has contacted the vehicle manufacturer, Ford, and the filter manufacturer, Allevard Sogefi USA (Sogefi), and learned that the low-pressure fuel system is capable of a maximum operating pressure of 8.5 Bar, while the secondary fuel filter is only tested to seven (7) Bar during the manufacturing process. Additionally, changes starting in 2022 by both Ford and Sogefi align with a lack of similar complaints on MY 2022 and newer vehicles. ODI is opening this Preliminary Evaluation (PE) to evaluate the severity of the potential problem and to fully assess the potential safety-related problems. The Office of Defects Investigation (ODI) opened this RQ to investigate allegations that the scope and remedy of safety recall 13V-535 were inadequate. Recall 13V-535 was limited to a relatively small population of Ambulance Package vehicles and ODI received complaints of pickup trucks and Fire Engine Prep Package vehicles being affected by the same defect. Ford manufactured approximately 555,985 subject vehicles between February 22, 2010 and January 30, 2015 that may be affected by defective EGT sensors and Powertrain Control Module (PCM) software calibration. The subject vehicles are equipped with four (4) EGT sensors to monitor engine operating conditions. An EGT sensor fault may produce false exhaust over-temperature signals. If these signals are produced, the Powertrain Control Module (PCM) implements a Failure Management Effects Mode (FMEM) and the driver receives a Stop Safely Now (SSN) system warning on the instrument cluster message center. The FMEM triggers a 70% reduction in available engine power over a period of no more than 45 seconds, during which it may not be possible to accelerate or maintain vehicle speed. The PCM then commands the engine to shut down when vehicle speed reaches approximately 1 mph. There is no immediate restart capability.
