On [XXX], I was driving westbound from Missouri into Oklahoma in my 2021 Ford F-150 XL. While traveling approximately 75 mph on the highway, I moved into the passing lane to overtake another vehicle. As I accelerated and applied additional throttle to pass, the transmission downshifted into passing gear. After completing the pass, the vehicle continued to accelerate on its own even after I released the accelerator pedal. The engine remained at a high throttle setting, and the truck increased speed to approximately 85 mph. At that time, I was not using cruise control, nor had I pressed the cruise control switch, which was involved in a previous acceleration-related incident I experienced with this vehicle. To regain control, I carefully created distance from surrounding traffic, applied the brakes, and maneuvered the vehicle onto the shoulder. After coming to a stop well off the roadway, I shifted the vehicle into Park and turned off the engine. I then restarted the truck, and the unintended acceleration condition was no longer present. Because of the incident, I did not attempt to use the cruise control system again during the trip, and I avoided using heavy throttle or passing gear out of concern that the condition could reoccur. The vehicle has since been inspected by a Ford dealership. As of this report, their evaluation has not identified the cause of the problem, and no fault has been found despite the incident occurring on the highway. This incident created a significant safety concern because the vehicle continued to accelerate after the accelerator pedal was released, resulting in an unexpected increase in speed and requiring me to pull off the highway and shut off the vehicle to regain normal operation. Ford has a VERY well-documented flaw in their LED headlight assemblies on 14th generation F150s (2021 +). The daytime running lights regularly burn out within several years / 35,000 miles, and each headlight costs over $2000 per side to replace with OEM components! They cannot be repaired; they can only be replaced. Ford specifically excludes these headlight assemblies from their extended warranty as well. The problem is so common that, when you see a 14th generation F150 on the road that's a few years old, it likely has one of the LED DRL lights out. Ford dealers verbally acknowledge how common of an issue it is, but Ford Motor Company refuses to issue a TSB or recall. Customers are left with shockingly high bills for what really is safety equipment. As you know, DRLs are indeed considered safety equipment! Customer shouldn’t have to pay over $4000 to fix this clearly common OEM supplier flaw that Ford has yet to remedy. Of course, what many owners do is simply allow the DRLs to fail because of the prohibitively expensive repair costs. This is a safety issue.

The CDF drum is going bad cause the transmission to slip and downshift while going at highway speeds. Many many many owners report this and Ford created a new CDF drum for owners to put in. However this costs thousands of dollars to replace when they know the CDF drum is faulty.

Vehicle Information 2021 Ford F-150 XL Supercrew approximately 35,000 miles. While traveling southbound on [XXX] near [XXX], Ok, I was operating my 2021 Ford F-150 XL at approximately 75 mph with the cruise control engaged. As I approached slower traffic ahead, I attempted to disengage the cruise control by pressing the cruise control cancel/off button on the steering wheel. Despite pressing the button multiple times, the cruise control would not disengage. Same thing while tapping the brakes it would not disengage. The vehicle continued to maintain speed, and the cruise control system remained unresponsive to my commands. In response, I applied the brakes to safely reduce speed, I did not perform a panic stop or abrupt braking maneuver. Once I had the vehicle slowed, I moved safely onto the shoulder of the highway, well away from traffic. I then shifted the vehicle into park, shut off the engine, and restarted the truck. After restarting the vehicle, I elected not to use the cruise control system for the remainder of my trip, which lasted approximately eight additional hours to Houston, Texas, I have not used the cruise control since the incident because I am concerned that the condition could recur and create a safety hazard. No collision, property damage, or injuries occurred. However, the apparent failure of the cruise control system to disengage when commanded raises significant safety concerns and warrants investigation. The contact owns a 2021 Ford F-150. The contact stated that while driving at an undisclosed speed, the transmission unexpectedly downshifted from fourth to first gear. There was no warning light illuminated. The contact pulled over and waited before restarting the vehicle. The contact stated that two days prior to the failure, a message was displayed indicating that an over-the-air software update was performed under NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline). The vehicle was taken to the dealer, who determined that an unknown sensor inside the transmission had damaged the transmission and that the unknown sensor needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 82,449.

Within the last 6 months, the driver's side front door, the driver's side rear passenger door, and the passenger side rear doors actuators stopped working and the doors do not lock. The doors do not lock manually with the key fob or lock automatically when the truck reaches 13 mph. This is a safety issue as passengers could inadvertently open the doors when driving when the doors should be locked. This also prevents the factory installed alarm to work properly. it seemed odd that the 3 actuators seem to stop working around the same time.

I own a 2021 Ford F-150, VIN [XXX]. A software error may cause the rearview camera image to be obstructed by the infotainment home screen menu when the vehicle is placed in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."
Consequence & remedy
Consequence: An obstructed rearview camera image can reduce the driver's rear visibility, increasing the risk of a crash.
Remedy: The accessory protocol interface module (APIM) software will be updated by a dealer or through an over-the-air (OTA) update, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 7, 2026. Additional letters will be sent once the remedy is available, anticipated in September 2026. The moonroof wind deflector clips may be inadequately attached to the moonroof frame, which can result in the wind deflector detaching from the vehicle.
Consequence & remedy
Consequence: A detached wind deflector can become a road hazard and increase the risk of a crash.

On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family. If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles.
