The EPAS (Electric Power Assisted Steering) module experienced an internal sensor failure, causing a sudden and complete loss of power steering assist. The vehicle is available for inspection upon request and the EPAS module has been retained. The loss of power steering occurred without warning while the vehicle was in motion, requiring significantly greater physical force to maintain control of the steering wheel. This creates a serious risk of losing vehicle control, particularly at higher speeds, during lane changes, or in emergency maneuver situations. Because the failure occurs randomly and without any prior indication, there is no opportunity to safely prepare or pull over before it happens, putting both the driver and other motorists at risk. Yes. The issue has been acknowledged by multiple Ford dealerships and independent mechanics, all of whom confirmed this is a known and recurring problem with this vehicle’s EPAS system. Numerous online automotive forums also document other owners experiencing the identical failure, suggesting this is a widespread issue rather than an isolated incident. The vehicle was inspected by an authorized Ford dealership, which confirmed the EPAS module failure and acknowledged familiarity with the issue. No warning lights, dashboard messages, unusual sounds, or handling changes occurred before the failure. The power steering loss happened suddenly and without any prior symptoms, leaving no opportunity to anticipate or react to the malfunction before it occurred.

I am writing to bring to your attention a critical safety issue affecting my 2021 Ford F-150, caused by a recent SYNC software update. On or around August 2024, Ford issued a software update that has resulted in significant malfunctions in multiple vehicle systems, including intermittent freezing and non-responsiveness shortly after vehicle startup. These issues persist for several minutes, severely impairing safety features—most notably the federally mandated backup camera that all vehicles sold after 2018 are required to have for driver and pedestrian safety. Following Ford's issuance of a recall number, I scheduled a service appointment with my local dealership and was later informed that Ford's specified service procedures would incur additional charges since my vehicle is now out of warranty. Ford has indicated that rolling back to a previous software version is not an option, as only Ford has access to the software. The company further requires an official diagnostic assessment at an estimated cost of $500 to $1,000 to even consider covering part of the repair costs. This approach is unreasonable, given that the malfunction originates from Ford’s own software update and directly impacts the safe operation of the vehicle. This is especially concerning as it compromises the backup camera and other critical driver-alert systems, which could lead to severe injury or even fatalities if the issue remains unaddressed. This issue is not isolated; a significant number of Ford F-150 owners are reporting similar problems on public forums, where Ford has acknowledged the software defect but has not offered a resolution without additional financial burden on affected customers. This vehicle is equipped with Ford BlueCruise hands-free highway driving system. This system was activated when this occurred and my hands were off the wheel. Driving Southbound on [XXX] in Georgia, [XXX], left-most lane, with an ~8 ft shoulder and barrier, a few miles before mile [XXX], right around Sparks and Adel, GA. There were two dark tire skid marks in the road leading from the lane into the shoulder/barrier. It seemed like the truck thought those tire skid marks were the lines in the road and took the truck with them into the shoulder. Luckily I was paying close attention and consider myself to have quick reflexes. I was able to grab the wheel and correct before the truck got too far out of the lane. Only when the truck started going out of the lane did the system turn off and tell the driver to take control. This dash warning occurred fractions of a second after I grabbed the wheel to correct. I had my dash camera operating at the time, but I was too slow to capture and save the exact occurrence, however I do have an image of the same portion of highway where this occurred. I have uploaded two images. One unedited image and one "example" image of what the tire skid marks looked like in the road. Someone paying less attention or with slower reflexes could have easily ended up in the barrier at 70-80 MPH. The vehicle has not been inspected for this. Other than this incident, it has functioned seemingly fine and without indicating any error codes or check engine lights. In April 2023, while driving to work, at least a dozen error messages were displayed on the instrument cluster. When I arrived at work and was driving at a slower speed, I noticed some irregular feedback in the steering wheel and quickly realized the power steering was intermittently disabling itself. The truck had just rolled over 30k miles at this point and was well within the manufacturer's warranty period. I took the truck to the dealer for repair and got the truck back within a few days and was told it was repaired. The very next morning while driving to work again, the same problem ensued again with a dozen error messages and the power steering intermittently disabling itself. I took the truck bad to the dealer, contacted Ford customer care, and they got an engineering team to help work the problem. The service advisor also told me there were 3 other vehicles at their dealer with the same problem. After more than 30 days of being at the dealer for repair, the truck was returned to me again. Everything seemed to be fine for a couple months and then it returned again with many error messages and power steering failures. On Monday this week I started to pull out in traffic and the power steering disabled and caused me to cross the 2-lane road into oncoming traffic.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: An obstructed rearview camera image can reduce the driver's rear visibility, increasing the risk of a crash.

Remedy: The accessory protocol interface module (APIM) software will be updated by a dealer or through an over-the-air (OTA) update, free of charge. Interim letters, notifying owners of the safety risk, were mailed August 7, 2026. Additional letters will be sent once the remedy is available, anticipated in September 2026. The moonroof wind deflector clips may be inadequately attached to the moonroof frame, which can result in the wind deflector detaching from the vehicle.

Consequence & remedy

Consequence: A detached wind deflector can become a road hazard and increase the risk of a crash.

EA23002 · Loss Of Motive Power

On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family.  If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles.
