During the failure, the vehicle abruptly decelerated and failed to exceed 20 MPH. The contact stated the failure had occurred while driving at undisclosed speeds. The failure also occurred while the vehicle was idling at a stop light. During the failure, the message "Service Advance Track" was displayed. The wrench icon was displayed on the instrument panel. The vehicle was taken to the local dealer who was unable to determine the cause of the failure. The contact stated that the vehicle was recently repaired under NHTSA Campaign Number: 26V237000 (POWER TRAIN). The contact stated that the vehicle had experienced the failure listed in the recall. dangerous, unpredictable transmission behavior Violent Downshifting Sudden jerks and Deceleration and Power Loss: heavy jerking loud clunks gears making noises following the forced downshift, the truck will experience a massive, unexpected drop in speed, sluggish and difficult maintaining speed like limp mode driving in normal speeds then loss of power engine stalling in the middle of rush hour traffic The Orange Wrench Light and check engine light will come on with codes p0720 I went did the reprogramming as recommended Ford dealer performed Safety Recall 26S28, which immediately aggravated the transmission failure and left the vehicle completely disabled. in the highway the dealer's recall software update directly caused a total transmission failure the next day, and i feel unsafe driving the vehicle due to the severe unprompted downshifting risks outlined in NHTSA Campaign 26V237 Felt unsafe to drive truck and also it became undrivable and had to have it towed to the Ford dealership that did recall.

Ford recently did a software update for a unexpected downshift recall that had previously happened to me while doing 60 miles an hour in my vehicle downshifted into first gear in my truck went sideways because the rear end locked up. Drove my vehicle home and took it out today and while doing 45 miles an hour it downshifted into first gear locking up the rear wheels again. Thankfully, this was at a lower speed so I was able to control the vehicle easily.. the turnpike speed limit in Maine is 70 miles an hour so if this happened at that speed in traffic, I’m almost positive there could have been a serious accident. So their update did not fix this issue.

On May 10, 2026, my 2016 Ford F-150 (6R80 transmission) went into limp mode and displayed a wrench warning light while driving. The truck lost normal transmission function and would not shift properly, creating a hazardous situation in traffic. The PCM has stored active Output Shaft Speed sensor codes P0720 and P0722, confirmed by a Paul Miller Ford IDS diagnostic scan, indicating a failing molded leadframe inside the transmission. When the OSS signal drops out at road speed, the PCM can command a sudden unintended downshift into a low gear, causing abrupt deceleration and potential rear-wheel lockup and loss of control — a serious safety hazard. Ford Recall 26S28 (26V-237) / Program 26N04 for 2015–2017 F-150s only covers a PCM software recalibration for the Transmission Range Sensor codes (P0705/P0706). It does not replace the leadframe hardware for the OSS codes (P0720/P0722) my truck is actually throwing. Ford's system auto-rejected the claim, and a dealer SSSC prior-approval request was denied. I am being told to pay out of pocket for a known defective part. For 2014 trucks, Ford replaced this same leadframe for free under Recall 24S37 when P0720/P0722 were present. My 2016 has the same transmission, same part, and same failure, but the current recall remedy is software only. I'm filing so NHTSA has documentation that the OSS leadframe failure is occurring on 2015–2017 F-150s and to request the remedy be expanded to cover leadframe replacement. No crash or injury occurred; the defect was confirmed by a Ford dealer.

The contact owns a 2016 Ford F-150. The contact stated that while driving at various speeds, the transmission unexpectedly downshifted and became stuck in a lower gear before shifting into the intended gear. The powertrain warning light was intermittently illuminated. The vehicle was taken to the dealer, where DTC: P0705, P0706, P0707, and P1702 were retrieved, indicating a powertrain control module (PCM) and leadframe failure. The vehicle was not repaired because the warranty had expired the day prior. The contact later received notification of NHTSA Campaign Number: 26V237000 (Power Train), and the vehicle was taken to the dealer, where the PCM software update was completed. The contact was advised to drive the vehicle for an extended period and return to the dealer to determine if the leadframe needed to be replaced. The failure persisted while driving, and upon returning to the dealer, DTC: P0717 and P0715 were retrieved. The dealer submitted a claim to the manufacturer, but coverage for the leadframe replacement was denied because the diagnostic trouble codes did not match the recall specifications. The contact stated that the PCM software update erased the initial diagnostic trouble codes, preventing the leadframe replacement under the recall. The vehicle was not repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The failure mileage was approximately 81,000.

The contact was informed that parts were unavailable to repair the vehicle. The contact was informed that there was an open recall on the vehicle; however, there was no notification that the vehicle should not be driven. The contact was advised that the vehicle could be driven until the parts became available. The contact later received notification of NHTSA Campaign Number: 26V237000 (Power Train); however, the part to repair the vehicle was not yet available. The vehicle was taken to the dealer where it was diagnosed with a PCM failure. The contact was informed that the repair was not covered under the recall. Ford Motor Company (Ford) is recalling certain 2015-2017 F-150 vehicles. A loss of signal between the transmission range sensor and the powertrain control module can cause the transmission to unexpectedly downshift into second gear.
Consequence & remedy
Consequence: Unexpectedly downshifting into second gear may result in a loss of vehicle control, increasing the risk of a crash.
Remedy: Dealers will update the powertrain control module software, free of charge. Owner notification letters were mailed July 24, 2026. Owners may contact Ford customer service at 1-866-436-7332. Consequence: A loss of brake fluid may result in a change in brake pedal travel, increased pedal effort and reduced front brake function, requiring greater effort and distance to stop the vehicle. These conditions can increase the risk of a crash.
Remedy: Dealers will replace the brake master cylinder. If the master cylinder is leaking, the brake booster will also be replaced. Repairs will be performed free of charge. Water and contaminants may get into the block heater cable's splice connector causing corrosion and damage.

Consequence: When the heater is plugged in, prolonged corrosion could cause a resistive short, increasing the risk of overheated or melted wiring which can increase the risk of a fire.
Remedy: Ford will notify owners, and dealers will disable the block heater cable, free of charge. Once parts are available, Dealers will replace the engine block heater cable, free of charge. Ford Motor Company (Ford) is recalling certain 2016 F-150 and Explorer vehicles. Loose power seat adjuster pivot bolts may cause the front seat cushions to detach and not properly restrain the seat occupant in the event of a crash.
Consequence & remedy
Consequence: In the event of a crash, a seat occupant that is not properly restrained has an increased risk of injury.
Remedy: Ford will notify owners, and dealers will inspect the tightness of the power seat track upper pivot link bolt, either applying threadlocker or replacing the lift link and hardware, free of charge. The doors on these vehicles may have a bent or kinked actuation cable or the door latches may freeze after water gets into the latch. Either condition can result in the doors opening while driving although they appear to be closed.

Consequence: A door that opens while driving can increase the risk of injury.
Remedy: Ford will notify owners, and dealers will inspect the door latch actuation cables and repair if necessary, and install water shields over the door latches, free of charge. The left rear inflatable seat belt buckle assembly may be inadequately attached to its mounting bracket. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 210, "Seat Belt Assembly Anchorages."
Consequence & remedy
Consequence: If the left rear inflatable seat belt buckle separates from it's mounting bracket, the seat occupant may not be adequately restrained, increasing their risk of injury.
Remedy: Ford will notify owners, and dealers will inspect the seat belt assembly, replacing the attaching rivet as necessary, free of charge. Interim notices informing owners of the safety risk were mailed October 18, 2017. Owners will receive a second notice when the remedy becomes available. The front passenger seat Occupant Classification System (OCS) that calibrates the air bag deployment level may incorrectly classify an adult as a child when the seat massage feature is activated.
Consequence & remedy
Consequence: If an adult is incorrectly classified as a child, the passenger frontal air bag will not deploy during a crash, increasing the risk of injury.

On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift. ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. ODI is opening this Engineering Analysis (EA) to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified. On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires. Ford will notify owners and require dealers to remove B-pillar insulation material and wiring harness tape, and install heat resistant tape, free of charge. On June 8, 2020, Ford Motor Company issued a safety recall (NHTSA Recall No. 20V-332) of 292,311 model year (MY) 2014-2017 Ford F-150 vehicles equipped with 3.5L Ecoboost engines and built from 9/1/2014 to 8/1/2016. Ford stated that the causes of brake fluid loss in the master cylinder are brake fluid contamination that can interfere with rear cup seal function, and rolling of the rearmost cup seal.Prior to total loss of the front brakes, depending on the amount of brake fluid loss in the master cylinder, the driver may receive an audible chime, message center alert and red brake warning indicator in the instrument cluster, and may experience a change in brake pedal travel and increased pedal effort. It should be noted that a small number of late-built MY 2014 and early-built MY 2017 F-150 vehicles are included in the recall along with the subject vehicles (MY 2015-2016).This Recall Query is closed based on the safety recall issued by Ford.
