VEHICLE SUDDENLY LOST POWER WHILE DRIVING BOTH FROM A STOP AND AT HIGHWAY SPEEDS. I SHUT OFF VEHICLE AND RESTARTED IT WHICH SEEMED TO TEMPORARILY SOLVE THE PROBLEM ONLY TO HAVE IT REOCCUR DAYS LATER. ENGINE CODE REGISTERED ON THE DASH DISPLAY. CODE WAS RETRIEVED (P2112) WHICH CORRELATES TO A FAULTY THROTTLE BODY. FORD HAS ACKNOWLEDGED THIS PROBLEM WITH AN EXTENDED WARRANTY REPLACEMENT, BUT MY LOCAL DEALERSHIP WILL NOT REPLACE THE THROTTLE BODY, SO I WAS FORCED TO REPLACE IT MYSELF AND PAY FOR THE FAULTY PART. MY VEHICLE HAD LESS THAN 36,000 MILES AT THE TIME OF THIS INCIDENT. WHEN THE VEHICLE LOSES POWER IN TRAFFIC, IT IS DEFINITELY A LIFE, HEALTH, SAFETY ISSUE. THERE ARE NUMEROUS EXAMPLES THAT CAN BE FOUND ON THE INTERNET OF THIS DEFECTIVE PART/THROTTLE BODY ON VARIOUS FORD VEHICLES. I FEEL THAT FORD SHOULD INITIATE A RECALL FOR OBVIOUS SAFETY REASONS. FORD HAS PUT OUT VARIOUS TSB'S REGARDING THIS DOCUMENTED PROBLEM.

WHILE DRIVING ON A HIGHWAY ON 8/30/20, THE TRUCK EXPERIENCED A COMPLETE LOSS OF FORWARD MOTION, BEGAN "BUCKING" AND CODES AND WARNINGS STARTED APPEARING ON THE DASHBOARD. WERE ABLE INITIALLY TO COAST TO A SAFER LOCATION TO STOP. TRUCK WOULD NOT MOVE ALTHOUGH THE ENGINE WAS RUNNING. TURNED THE TRUCK OFF, RESTARTED AND ALL CODES DISAPPEARED. HOWEVER, AS SOON AS WE STARTED AGAIN, WITHIN A QUARTER MILE, SAME FAILURE OCCURRED. IN 20 MILES, THIS HAPPENED APPROXIMATELY 6 MORE TIMES AND WE COULD NOT GET SAFELY OFF THE ROAD (NO SHOULDERS WITH DITCH ON SIDE) HAVING TO COME TO A COMPLETE STOP, TURN ENGINE OFF AND RESTART THE VEHICLE WHILE IN THE TRAFFIC LANE. VERY UNSAFE AND WE WERE LUCKY NOT TO BE HIT BY PASSING VEHICLES. AFTER RESEARCHING WHAT THIS POSSIBLE COULD BE, FOUND IT LIKELY THAT IT WAS A FAILURE OF THE ELECTRONIC THROTTLE BODY, WHICH WAS THE SUBJECT OF A RECALL IN PREVIOUS YEAR MODELS OF THIS VEHICLE AS WELL AS A CUSTOMER SATISFACTION ACTION #16B32 FOR REPLACEMENT OF THE ELECTRONIC THROTTLE BODY FOR OUR YEAR OF MANUFACTURE. (OUR TRUCK WAS MANUFACTURED IN DEARBORN DURING THE PERIOD THAT THIS ACTION WAS TO BE TAKEN 8/1/15 THROUGH 5/17/16.) MADE APPOINTMENT AT LOCAL DEALER AND OUR TRUCK WAS SERVICED YESTERDAY AT PITTSVILLE FORD IN PITTSVILLE MD AND THE DIAGNOSIS WAS THE ELECTRONIC THROTTLE BODY FAILURE. THE ETB AND GASKET WERE REPLACED AT A COST TO US OF $551.14. I HAVE SEEN ONLINE THREADS OF OTHERS HAVING THIS SAME FAILURE AND WANT TO KNOW WHY OURS WAS NOT PART OF A RECALL OR OTHER, LONGER CUSTOMER SATISFACTION ACTION, AS THIS COULD HAVE CAUSED A SEVERE OR EVEN FATAL ACCIDENT. VERY DANGEROUS TO HAVE THIS TYPE OF CATASTROPHIC FAILURE WITH NO WARNING AT ALL. I HAVE REPORTED THIS ISSUE FOR FORD AND MY QUESTIONS REGARDING WHY THIS WAS NOT ALREADY REPLACED AND HAVE BEEN ASSIGNED A CASE NUMBER - CAS-27541116. TOLD OUR VIN WAS NOT A PART OF THIS ACTION. HOW?

THIS HAPPENED AGAIN A COUPLE OF WEEKS LATER. I CALLED THE FORD DEALER ON SATURDAY AND WAS INFORMED THAT IT WAS PROBABLY A THROTTLE BODY ISSUE. I WAS ABLE TO GET AN APPOINTMENT ON MONDAY, BUT THIS PROBLEM OCCURRED AGAIN ABOUT 10 TIMES OVER THE WEEKEND. IN MY OPINION THIS SHOULD BE A SAFETY RECALL AS THIS ISSUE COULD OCCUR WHILE PASSING ON A NARROW TOW LANE ROAD, GOING THROUGH AN INTERSECTION, CROSSING A RAILROAD TRACK, ETC.,. FROM AN INTERNET RESEARCH I I FOUND THAT FORD HAS ISSUED TSB 16-0139 ON OCTOBER 10, 2016 AND THERE WERE NUMEROUS COMMENTS FROM OTHERS HAVING THE SAME PROBLEM. IN FACT THE MAINTENANCE MANAGER AT FORD TOLD ME THIS HAD HAPPENED TO HIM TWICE.

THE WRENCH LIGHT, HILL DECENT AND TRACTION CONTROL WARNING LIGHT ILLUMINATED. THE ENGINE IDLED BUT WOULD NOT ACCELERATE. I SHUT OFF THE ENGINE AND RESTARTED IT. THIS SEEMED TO CORRECT THE PROBLEM CLEARING THE ERROR CODES. HOWEVER, CONTINUING ON THE ROAD, THE SAME PROBLEM OCCURRED 3 MILES LATER. I WAS IN A REMOTE AREA IN THE STATE AND HAD TO TRAVEL 60 MILES TO A URBAN AREA. THIS PROBLEM OCCURRED AT LEAST 20 MORE TIMES WITH GREATER FREQUENCY UNTIL ARRIVING AT MY RESIDENCE. THE VEHICLE WAS REPAIRED BY THE FORD DEALERSHIP IN SHERIDAN, WY. THE FIX WAS AS FOLLOWS. DIAG, P2112-THROTTLE ACTUATOR A CONTROL SYSTEM, STUCK CLOSED, PINPOINT TO THROTTLE BODY. REPLACED THROTTLE BODY, CLEARED CODES. MY VEHICLE HAD 38,450 MILES WHEN THIS INCIDENT OCCURRED ON OCTOBER 17, 2019, 4:30 PM. AFTER DOING SOME RESEARCH WITH OTHER FORD F150 OWNERS, THIS APPEARS TO BE A PROMINENT PROBLEM AND IN MY MIND, A DANGEROUS ONE. MOST OF THE INCIDENCES APPEAR TO HAPPEN IN THE SIMILAR MILEAGE RANGE BUT IN OTHER MODEL YEARS, 2017 AND 2018 WERE PREVALENT ALSO. THE FORD SERVICE MANAGER RELATED HE HAD DONE ALLOT OF THESE REPAIRS RECENTLY. THEIR IS NO RECALL FOR THIS ISSUE AT THIS TIME.

WHILE USING MY VEHICLE UNDER NORMAL CONDITIONS THE CHECK ENGINE LIGHT CAME ON SO I HAD VEHICLE CHECKED AT AUTOZONE AND O'REILY'S WHICH SHOWED 2 DIFFERENT CODES AND TOOK IT TO MACHAIK DEALERSHIP. TURNS OUT I HAD TO HAVE BOTH CYLINDER HEADS REPLACE, CRANKSHAFT BOLT REPLACED AS WELL AS A FEW OTHER ITEMS. I WAS TOLD IT WAS AN ISSUE THAT FORD WAS AWARE OF AND DIDN'T NOTIFY ME OF IT WHEN I BOUT THE VEHICLE. THEY COVERED THE COST DUE TO THE VEHICLE BEING UNDER WARRANTY WHICH WAS ABOUT $5,000. $ MONTHS LATER I GOT MY VEHICLE BACK AND ABOUT 30 MILES OF DRIVING 55MPH IT STARTED BLOWING A THICK CLOUD OF WHITE SMOKE WITH SOME GREY SMOKE. I TAKE THE VEHICLE HOME AND LET IT SIT FOR 28 DAYS BEFORE I HAVE TO USE IT AND AFTER ABOUT 23 MILES IT STARTED SMOKING AGAIN. I TAKE IT TO THE SAME DEALERSHIP AGAIN AND I TOLD THEM ABOUT THE SMOKING ISSUE, ROUGH IDLE AND THE EXHAUST SMELLS LIKE GAS WHICH IS NOT NORMAL. I SHOWED THEM A VIDEO OF THE ISSUE WHICH CONTAINS MY MILEAGE AT THE TIME AND MY LICENSE PLATE WHICH CLEARLY SHOWS MY VEHICLE SO THEY CANNOT SAY IT WASN'T MY VEHICLE. WAITING FOR THE RESULTS OF THEIR FINDINGS AND PLAN TO UPDATE THE COMPLAINT. I WILL NOT UPLOAD THE VIDEO AT THIS TIME DUE TO IT CONTAINING MY LICENSE PLATE, DON'T KNOW HOW TO UPLOAD IT FROM PHONE AND I DON'T KNOW HOW TO EDIT THAT OUT. IF IT IS NECESSARY TO UPDATE THIS COMPLAINT I WILL UPLOAD THE VIDEO AS PROOF AS WELL AS WORK ORDER FROM PREVIOUS AND CURRENT WORK.MY MILEAGE WILL NOT BE 100% ACCURATE BUT IT WILL BE WITHIN 400 MILES PLUS OR MINUS

BEHAVIOR MATCHES THE ISSUES DESCRIBED IN FORD RECALL 16B32. WHEN I ACCELERATE THE VEHICLE THE ENGINE OCCASIONALLY STOPS ACCELERATING AND GOES INTO A FAILURE MODE WHERE I CANNOT ACCELERATE. A FEW WARNINGS SHOW UP ON THE DASHBOARD RELATED TO HILL START ASSIST AND BEING DISABLED AND TO SERVICE ADVANCE TRACK. TURNING THE CAR OFF AND BACK ON RESOLVES THE PROBLEM, BUT THIS IS VERY DANGEROUS AS I'M USUALLY ACCELERATING TO GET ONTO A HIGHWAY WHERE IT'S NOT SAFE TO HAVE MY VEHICLE STOP. FROM THE DESCRIPTION OF THE RECALL MY VEHICLE SHOULD BE INCLUDED IN 16B32 (MANUFACTURED BETWEEN 8/31/15 TO 4/27/16 IN KANSAS CITY), BUT FORD TELLS ME MY VIN IS NOT LISTED AS AFFECTED. THIS PROBLEM HAS OCCURRED TWICE NOW AND IT IS UNPREDICTABLE IN WHEN IT WILL OCCUR. A QUICK GOOGLE SEARCH OF "HILL START ASSIST FAILURE" SHOWS A LARGE NUMBER OF OTHER FORD OWNERS WITH THE EXACT SAME PROBLEM.

MY TRUCK LOST COMPLETE POWER ON 8/3/19. SEVERAL DIFFERENT WANING LIGHTS CAME ON. FORTUNATLEY, I WAS LOST POWER ON A STRAIGHT STRETCH OF ROAD AT ABOUT 30 MPH. I WAS ABLE TO "LIMP" MY TRUCK BACK HOME. I HAVE A CODE READER AND IT SHOWED CODE P2112 "THROTTLE ACTUATOR CONTROL SYSTEM STUCK CLOSED". I SEARCHED ONLINE AND FOUND A FORD CUSTOMER SERVICE LETTER FROM 2016-2017 ADDRESSING THIS ISSUE WITH AN EXPIRATION NOTICE OF SEPTEMBER 2017. I PURCHASED THE VEHICLE USED IN DECEMBER 2016 AND WAS NEVER SENT ANY NOTICE. I FIND IT INCREDIBLE THAT FORD KNOWS ABOUT A DEFECT THAT CAUSES A VEHICLE TO LOSE POWER SUDDENLY, AND ADDRESSES IT WITH THIS BANDAID LEVEL OF SERVICE. IT IS A SIMPLE AND INEXPENSIVE FIX. I FIXED IT MYSELF IN LESS THAN AN HOUR WITH A PART PURCHASED FROM FORD THAT WAS LESS THAN $200. MY CONCERN IS THAT FORD DID NOT NOTIFY ME AND WILL NOT STAND BY THIS DANGEROUS DEFECTIVE ISSUE. THEY HAVE TOLD ME THERE WAS AN EXPIRATION ON THAT LETTER AND THEY WILL NOT HELP ME. PLEASE FORCE THEM TO DO AN ACTUAL RECALL ON THIS ISSUE BEFORE SOMEONE IS KILLED! THANK YOU FOR YOUR TIME.

EA26001 · Unintended Transmission Downshift And Rear Wheel Lock-up

Opened Jan 30, 2026 · No close date supplied

Status: open (inferred from source dates) · Power Train:automatic Transmission:control Module (tcm/pcm/tecm)

On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift.  ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. ODI is opening this Engineering Analysis (EA) to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified. To review the reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Opened Aug 3, 2018 · Closed Dec 17, 2018

Status: closed (inferred from source dates) · Seat Belts:pretensioner

On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires. Ford will notify owners and require dealers to remove B-pillar insulation material and wiring harness tape, and install heat resistant tape, free of charge.
