The body control module was destroyed when water leaked on it. Apparently there is a failed windshield seal(maybe) that causes leaking down the A pillar. That pillar leads the water directly to the wiring harness for the BCM. When my BCM went out, wipers, blinkers, external lights, speedo/tach, power windows (in the front), all HUD, all stereo, and I think the security system too, stopped working. It was raining hard, it was a very dark day/difficult to see. I and others could easily have been killed. VERY, VERY DANGEROUS! Why the BCM isn’t in a sealed box, is mind blowing. The fact that the computer to my truck can get wet at all is nothing short of asinine. The vehicle was inspected by ICDI Diesel in [XXX] [XXX]). The part is currently on order and isn’t expected to be delivered until XXX. The truck is available for inspection. I would like to file complaint against Ford for their latest recall (26N041) and how they are not doing replacing the defective Lead Frame in customers vehicles. It's a big scam and a waste of time. I brought my 2016 Ford F-150 in for Ford’s latest recall (26N04) affecting 2015–2017 models due to the transmission leadframe issue. I explained that I’ve been dealing with an intermittent problem for the past seven months: while driving, the gear indicator lights on the dash begin to flicker, and when that happens, the transmission shifts extremely hard and slams into the next gear. Ford kept my truck for a week and told me they were unable to duplicate the issue. They stated that multiple technicians drove the vehicle and none of them observed any problems. When I arrived to pick it up, I was handed a bill for more than $250 for a diagnostic fee. Within four miles of leaving the dealership, the gear indicator lights began flickering again. It’s difficult to accept that several technicians drove the truck over multiple days and never encountered the issue I experienced almost immediately. The entire process felt like a waste of time and money, and I’m left without any resolution to a problem that clearly still exists. They are not honoring the recall, they are trying to charge customers for the repair.

This vehicle 2016 ford F150 is known to have shorting issues. From literally all wiring harnesses. I had a small fire in my truck due to this issue, caused melting of the ribbon cable in my seat heater control module, which is a known issue from Ford. I have shorting in my turn signal switch (no wipers). Is that safe? I am a research scientist and engineer, I can tell you now it is not safe. If i need to prove it, I can do so with ease and write the necessary literature. If I cant use my AC sometimes, the windows fog, then i cant use the wipers to unfog them, how is this not a visibility issue? Not to mention all the shorts. I have a 40 gallon fuel tank, if any shorts get near it, is that going to be a fun experience for me? Please give Ford a penalty, the average owner of this truck pays thousands a year in repairs. I paid $17,800.00 in 2025 to repair this piece of shit. And it still has major issues. This is something ford needs to address, i want my wiring to be replaced, i bought a new engine, what if i cant use it especailly with the 3 year warranty bc the wiring consistently shorts and fails. unacceptable for a truck that's sticker price is $80k. This is a major problem, I will file claims until im afforded a response

I was at my granddaughters and I went to leave and it was like dead except the wrench came up in the dash panel , I waited before calling my husband and I went back to try and start it again and it started no wrench in the display or anything , this was in February 2026. In March of 2026 I was in the middle turn lane on [XXX] by Tuttle, Ok. And it stalled as I was trying to turn and would not move I put it back into park and then back to drive and it turned . Next incident was in April2026 and I was on [XXX] south of Mustang , Ok. And I was in the passing lane passing a vehicle going 65 mph and it just stalled and went down into 1 st gear I managed to get it to the shoulder of road an then it died it would not start but the radio and lights were still on and there was a wrench popped up in the panel . I waited for about 30 minutes and kept trying to start it after several attempts it started again and I made it home . The next incident my husband drove to doctors office and it did the same thing to him and would not move in the parking lot . About 15 minutes later he got it to start and he made it home . We quit driving it. Then we got a letter from Ford with the recall 26V237 .i waited for 2 nd letter and had not gotten it with a fix for it so I called the Ford Service Repir Center and they got us on the phone connected to a Ford Dealership in Yukon , Ok and they came and got the pickup on August 10 th and took it and brought it back on August 11 th and they did the soft ware update and told us no codes came up and we know better . When I had previously talked to the Ford Repair Center I was told if the wrench came up that they would replace the lead frame at no cost to us and give us a 150000 mile warranty. So it is now our word against the mechanics that the wrench ever came up INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The contact owned a 2016 Ford F-150. The contact stated that while driving 70 MPH on the freeway, another vehicle driver pulled beside the contact on the front passenger’s side of the vehicle, and informed the contact that there was smoke coming from the vehicle, and the contact became aware of smoke coming from the rear passenger’s side seats. No warning light was illuminated. The contact pulled over to the right side of the road, turned off, and then exited the vehicle. The contact became aware of the smoke and poured water between the seats of the vehicle, opened the rear passenger’s side door, pulled the rear passenger down, and noticed flames were coming from the rear of the vehicle. The contact sustained minor burns to the left and right hands. Medical attention was not needed. The contact called the emergency services, but a police report was not filed. The contact immediately extinguished the fire. The Fire Department arrived on the scene, folded the driver’s side passenger seat down, and detached the electrical wiring component that was attached to the rear passenger’s side seat. The vehicle was deemed a total loss. The vehicle was towed to a dealer but was not repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 141,000.

My 2016 Ford F-150 (VIN [XXX]) experienced an intermittent failure involving the transmission range sensor system. While driving, the vehicle displayed a check engine light and a “See Service Manual” message. The truck became noticeably underpowered. When shifted into Reverse, the transmission engaged harshly and the PRNDL (gear position indicator) disappeared from the instrument cluster. The backup camera also became inoperative. After shutting the vehicle off, it would not restart. Electrical accessories and the instrument cluster remained functional, but the engine would not crank. After sitting for a period of time, the vehicle eventually restarted and the PRNDL display returned. The same failure occurred on three separate occasions within approximately 24 hours, including while arriving at a Ford dealership for service. The failure creates a safety concern because the vehicle can unexpectedly become unable to start, loses indication of the selected gear, and may experience transmission-related issues while being operated. The vehicle is currently unreliable for transportation and could leave occupants stranded. An independent repair facility scanned the vehicle and documented DTCs P0706 (Transmission Range Sensor A Circuit Range/Performance), P0707 (Transmission Range Sensor A Circuit Low), P1702 (Transmission Range Sensor Circuit Intermittent), and P1921 (Transmission Range Signal). The vehicle is included in Ford Safety Recall 26S28 involving 2015-2017 F-150 vehicles equipped with the 6R80 transmission. A Ford dealership inspected the vehicle and advised that the concern appears related to the transmission range sensor/lead frame issue associated with the pending recall. However, the dealership advised that a repair cannot currently be submitted because the recall remedy has not yet been activated. The vehicle had approximately 124,000 miles at the time of failure. VEHICLE SAFETY COMPLAINT STATEMENT Prepared for NHTSA Submission Regarding Recall 26V237 / 26S28 To: National Highway Traffic Safety Administration (NHTSA) Subject: Report of Accident and Vehicle Control Loss Due to Defect Specified in Recall 26V237 VEHICLE INFORMATION: Year/Make/Model: 2016 Ford F-150 VIN: [XXX] INCIDENT DESCRIPTION: In March, while driving during a snowstorm, the vehicle experienced a sudden and unexpected loss of traction and stability. Without warning, the rear wheels appeared to lock or drastically reduce speed, causing the truck to slide uncontrollably and strike a roadside barrier. Following the initial impact, while attempting to move the vehicle to a safe location as instructed by insurance/authorities, the engine suddenly stalled and shut down completely while in motion, nearly resulting in a second high-speed collision with the vehicle ahead. CORRELATION WITH RECALL 26V237: In May, I received a Safety Recall Notice (Ford Recall 26S28 / NHTSA Recall 26V237). The description of the defect—an "unexpected downshift to a lower gear causing abrupt wheel speed reduction and rear tire slide"—exactly matches the catastrophic event I experienced in March. The recall also notes that the engine may malfunction or stall, which occurred during my incident. IMPACT & DAMAGES: The insurance company evaluated the damage at approximately $6,000 for structural repairs. However, the vehicle remains unsafe and mechanically compromised. Despite the structural fixes, the core safety defect involving the transmission and engine software remains unresolved as Ford has stated that a remedy will not be available until July 2026. REQUEST: I am filing this formal complaint to document that the defect in Recall 26V237 resulted in a real- world accident and total loss of vehicle utility prior to the notification being sent. I request that NHTSA takes note of this incident as part of its oversight of Ford's recall execution, as the vehicle is currently un-dri INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATIO

4EA26001 · Unintended Transmission Downshift And Rear Wheel Lock-up
Opened Jan 30, 2026 · No close date supplied
Status: open (inferred from source dates) · Power Train:automatic Transmission:control Module (tcm/pcm/tecm)
On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift. ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. ODI is opening this Engineering Analysis (EA) to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified. To review the reports cited in the Opening Resume ODI Report Identification Number document, go to NHTSA.gov.

Status: closed (inferred from source dates) · Seat Belts:pretensioner
On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires. Ford will notify owners and require dealers to remove B-pillar insulation material and wiring harness tape, and install heat resistant tape, free of charge.
