My 2015 Ford F-150 experienced multiple sudden downshifts at highway speeds 75 mph and then started downshifting at lower speeds. Heavy clunking heard and wrench warning lamp came on with a very general warning message and turned both off after moving to the side of the highway and restarting the car. Warning sign returned while driving at significantly lower speeds on city streets, locked up tires when turning from a low speed twice. Extremely unsafe and could have caused a serious accident if people were following close behind me. Took it to the dealership to get repaired and they diagnosed DTC P0722 (Output Shaft Speed Sensor - No Signal) and confirmed that the signal failure caused the transmission to lock in 3rd gear and operate at maximum line pressure. Technician notes it resulted in damage to forward, direct, and overdrive clutches and recommended the replacement of the entire transmission ($5,739 for parts). I paid $8,636.93 out-of-pocket at a Ford dealership for this service repair.

Execution of manufacturer safety recall service resulted in immediate, catastrophic transmission malfunction and sudden loss of motive power upon leaving the franchised dealership, creating an imminent road hazard. Details of the Incident: Recall Intake: On XXX, the vehicle was driven into the authorized Ford dealership Cloninger Ford of Morganton in [XXX] under its own power for an open manufacturer safety recall involving the transmission molded leadframe and PCM software update to prevent sudden, unintended downshifts. Immediate Failure Post-Service: After technicians completed the recall service and returned the keys, the vehicle was driven out of the dealership lot. Upon reaching normal road speed and shifting from 3rd to 4th gear (less than 0.2 miles from the service bay), the transmission violently malfunctioned, lost forward drive engagement, and rendered the truck completely disabled and undrivable in active traffic. No road test was performed by the dealership prior to releasing the vehicle. Safety Risk: The sudden failure and inability to shift gears properly left the vehicle stranded in roadway traffic, presenting an immediate collision risk. The vehicle had to be limped back into the dealership service drive immediately. Remedy Failure & Denial: The dealership acknowledged that the recall-mandated PCM software calibration update triggered the severe transmission failure. However, both the manufacturer and the dealer are now claiming the failure is a "pre-existing condition," refusing to complete the safety remedy without charging the owner $7,000 for a full transmission replacement. The truck exhibited the abrupt downshifting upon driving out of the parking lot, this caused great stress and possible accidents to me and family. The transmission continued to get worse and we were in fear of our lives to drive the vehicle on the highway. Once we got the vehicle home it literally would not shift into gear and has been disabled since the first week in XXX. We are plumbing and heat contractors and have lost a great deal of business due to not having a truck/this truck to use for work. We had it towed to Greico Ford in [XXX] on advice from Keith, the Service Mgr. We know the recall is due in XXX. This dealership is now trying to tell us that all the hoses are damaged and wants to charge us $2,000 to repair the hoses. We obviously don't want to do this since we believe the violent jarring and downshifting of the transmission caused by this MFG defect probably caused this and a lot more other problems. MFG Recall #26S28, NHTSA Recall# 26V237. I need to know when this recall remedy will be submitted and what are my rights to get this fixed? On Thursday, XXX, at approximately 5:10 AM, I was driving my Ford F-150 XLT (VIN: [XXX]) eastbound on [XXX]. While exiting the elevated bridge at the intersection of [XXX] and [XXX] at 55 mph (6th gear), the vehicle experienced a sudden, uncommanded transmission downshift directly from 6th to 1st gear. This caused immediate extreme deceleration, engine over-speed, and locked the rear tires, causing tire slippage and an absolute loss of traction control on a high-speed highway. This severe event put my life and my wife's life in imminent danger of catastrophic injury. After safely pulling over, the Malfunction Indicator Light (Check Engine) and the Powertrain Fault Warning Light (wrench icon) illuminated. The electronic cluster froze and failed to display the gear position. Drive (D) mode was completely unresponsive. I managed to vacate the active roadway using Manual (M) mode. While driving in 4th gear in manual mode under extreme caution, a second violent, unintended downshift to 1st gear unexpectedly occurred, confirming the persistent, dangerous nature of this failure. Consequential mechanical damage includes severe transmission clunking noises during gear engagement and a constant internal metallic knocking sound from the engine block caused by the forced over-speed. This failure perfectly matches the technical failure mode described in NHTSA Safety Recall Campaign 26V237 (OSS to PCM signal loss). On XXX, at 11:10 AM, I personally visited Arceneaux Ford Inc. (Authorized Dealership in [XXX]) to report this safety defect, open a service case, and request a safe vehicle inspection and loaner. The dealership explicitly refused to open a case or take action, stating that "Ford Motor Company does not currently have an engineered solution for this problem," leaving me stranded with an unroadworthy, highly hazardous vehicle. The truck has sporadically gone into “limp” mode for 6+ years. In the last 4 weeks it has happened a total of 6 times while driving. On Tuesday 6/9/2026 it happened while leaving a driveway. I was able to disconnect the battery and let the throttle body reset. I reconnected the battery, it started and drove without problems. I connected a loaded horse trailer, drove approximately 12 miles and it happened again, this time on a two lane road with a soft , uneven shoulder going from 60 miles per hour to 8 miles per hour very quickly. I was able to turn on my hazard lights and pull onto the grass, but only about 5 inches off the 60 mph road. In order to reset the throttle body, I disconnected the battery again, disabling the hazard lights while vehicles passed by narrowly colliding with us several times. The check engine light was illuminated, but I was not in a safe place to scan the truck. It usually disappears after resetting the throttle body, but did not this time. After 10 minutes, I heard the throttle body click, it started up and I drove another 15 miles when it happened again. This time at a temporary stoplight with no shoulder. I was able to scan the computer and the P2111 code was displayed. I reset the throttle body again, drove 5 miles and it happened again. I was able to navigate to a safer area, reset the throttle body again and drive less than two miles home. On 6/11, after letting the truck sit for two days, I started it with no issue to move the trailer and it happened again. I tried resetting the throttle body again and if did not work. The P2111 code is still showing and the throttle body will not reset. The truck is currently inoperable. It will only idle, roughly, and pressing the accelerator pedal had no effect on the truck.

The truck often shifts from 6th gear down to 1st or second gear, sometimes at highway speeds and all warning light come on. It sometimes resolved itself upon starting it again, but seems to affect the throttle body. When the electrical system resets, you can hear a click from the throttle body indicating that the valve reset when the computer reset. We also have been affected by intense suffering from the transmission that started about the same time, around 2019 and just over 100,000 miles, after the warranty expired. Both the limp mode and shuddering seem to be results of the TRS signal. I have replaced the battery and throttle body (that repair was first suspected to be the sensor) at Lakeland Ford, then later at Midas in north Lakeland I had work done to the wheel mounts and other axle/wheel relate repairs. In total we have spent approximately $4000 on repairs caused by the improper TRS signal. In addition, we have had at least 4 near accidents caused by the faulty signature. I called Ford with no solution, such as a loaner very, for safety before the recall can be fixed.

Consequence & remedy
Consequence: Unexpectedly downshifting into second gear may result in a loss of vehicle control, increasing the risk of a crash.
Remedy: Dealers will update the powertrain control module software, free of charge. Owner notification letters were mailed July 24, 2026. Owners may contact Ford customer service at 1-866-436-7332. Consequence & remedy
Consequence: A loss of brake fluid may result in a change in brake pedal travel and feel, increased pedal effort and reduced front brake function, possibly requiring greater effort and distance to stop the vehicle. These conditions can increase the risk of a crash.
Remedy: Ford will notify owners, and dealers will replace the brake master cylinder and inspect the brake booster, replacing it as necessary, free of charge. Water and contaminants may get into the block heater cable's splice connector causing corrosion and damage.

Ford Motor Company (Ford) is recalling certain 2015-2017 Ford F-150, and 2017 F-250, F-350, F-450, and F-550 Super Duty trucks. The doors on these vehicles may have a bent or kinked actuation cable or the door latches may freeze after water gets into the latch. Either condition can result in the doors opening while driving although they appear to be closed.
Consequence & remedy
Consequence: A door that opens while driving can increase the risk of injury.
Remedy: Ford will notify owners, and dealers will inspect the door latch actuation cables and repair if necessary, and install water shields over the door latches, free of charge. The left rear inflatable seat belt buckle assembly may be inadequately attached to its mounting bracket. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 210, "Seat Belt Assembly Anchorages."
Consequence & remedy
Consequence: If the left rear inflatable seat belt buckle separates from it's mounting bracket, the seat occupant may not be adequately restrained, increasing their risk of injury.

The affected vehicles may be equipped with driver or front passenger seat belt anchorage assemblies whose pretensioner cable was not properly crimped. As a result, the seat belts may not properly restrain the seat occupant in the event of a crash. On March 21, 2025, NHTSA’s Office of Defects Investigation (ODI) opened a Preliminary Evaluation (PE25002) to investigate instances of unexpected transmission downshifting without driver input in model year (MY) 2015-2017 Ford F-150 vehicles (subject vehicles) manufactured by Ford Motor Company (Ford). The complaints allege that, without warning or driver input, the subject vehicles experienced a sudden and rapid deceleration often accompanied by temporary rear wheel lockup, seizure, or skidding, resulting in a loss of vehicle control that increases the risk of crash and injury to all motorists, including those not within the subject vehicles. The MY 2015 and MY 2016 F-150 vehicles were equipped with the “6R80” transmission, while the MY 2017 vehicles were equipped with either the “6R80” transmission or the “10R80” transmission. This investigation was limited to MY 2015-2017 Ford F-150 with the “6R80” transmission. ODI identified 329 Vehicle Owners Questionnaires (VOQs) related to this investigation and sixty percent of these consumers were interviewed to confirm the details of their allegations. Consumers reported that their vehicles’ transmission suddenly downshifted to a lower gear, often to 1st or 2nd gear, without driver input or advance warning. Consumers described the downshift events to be regular and repeated occurrences, with some consumers stating that they no longer drive the vehicle due to safety concerns. Forty-three percent of consumers reported experiencing at least one wheel lockup event, during which the rear tires locked, screeched, or skidded during the transmission downshift. ODI also found that 114 of the 329 consumers reported having their vehicle’s molded lead frame or valve body assembly (which comes with a molded lead frame) replaced, 80 of which were confirmed through consumer submitted repair invoices. In its response to ODI’s Information Request, Ford provided an assessment of potential root cause of failure for the subject vehicles and how this failure is different from the failure associated with MY 2011-2014 Ford F-150 vehicles that was addressed by four safety recalls (16V-248, 19V-075, 19V-433, and 24V-444). Ford has identified that the defect in the recalled vehicle population involved lead frame supplier production issues resulting in signal loss from the Output Shaft Speed (OSS) sensor. Ford has identified that the alleged defect in the subject vehicle population involved degradation of electrical connections within the lead frame due to thermal cycling and vibration over extended time in service resulting in signal loss from the Transmission Range Sensor (TRS). Signal loss from the TRS can result in an unintended shift to neutral, unintended upshift, or unintended downshift which are regulated by a gear “shift map” based on the vehicle’s speed at the time of signal loss. According to Ford’s “shift map”, at speeds between 35-64 mph, the maximum allowable downshift would be to 2nd gear (i.e. a 6th to 2nd gear downshift event) which Ford has designated to represent the worst-case scenario for subject vehicles. Ford has also acknowledged that the worst-case scenario may also involve temporary wheel lockup. However, Ford has stated that the failure mechanism in the MY 2011-2014 F-150 vehicles—previously addressed through recalls—primarily involved OSS sensor signal loss that could result in 6th to 1st gear downshift events. This OSS-related failure mode is distinct from the TRS-related signal loss identified in the subject vehicle population. ODI has identified an additional potential safety defect associated with the alleged defect in the subject vehicles. Preliminary testing performed by NHTSA’s Vehicle Research Test Center (VRTC) showed that when the TRS experiences an intermittent signal loss, one possible outcome is a change in vehicle direction. Specifically, if a vehicle is operating in reverse up an incline when the TRS signal loss occurs, then the vehicle may shift into neutral causing the vehicle to change directions and roll forward. ODI is opening this Engineering Analysis (EA) to, among other things, perform component-level testing and vehicle testing, as well as review additional technical information to better understand the alleged defect that has been identified. On June 18, 2018, the Office of Defects Investigations (ODI) opened a Preliminary Evaluation (PE18-008) to investigate complaints alleging post-crash fires at the B pillar in the area of the seat belt pretensioners on MY 2014-2018 Ford F-150 Regular Cab, and Super Crew Cab Vehicles. On August 24, 2018, Ford Motor Company recalled (FSA 18S27 - NHTSA #18V568) the affected population. Ford's investigation found that some front seat belt pretensioners may generate sparks during deployment in excess of Ford's specification. Sparks in the pretensioner exhaust gases inside the lower portion of the B-pillar may subsequently ignite combustible components contained in the B-pillar, such as insulation or carpet, and may cause a subsequent vehicle fire. ODI also requested relevant information from other manufacturers to assess the potential scope of post-crash pretensioner fires. Ford will notify owners and require dealers to remove B-pillar insulation material and wiring harness tape, and install heat resistant tape, free of charge. On June 8, 2020, Ford Motor Company issued a safety recall (NHTSA Recall No. 20V-332) of 292,311 model year (MY) 2014-2017 Ford F-150 vehicles equipped with 3.5L Ecoboost engines and built from 9/1/2014 to 8/1/2016. Ford stated that the causes of brake fluid loss in the master cylinder are brake fluid contamination that can interfere with rear cup seal function, and rolling of the rearmost cup seal.Prior to total loss of the front brakes, depending on the amount of brake fluid loss in the master cylinder, the driver may receive an audible chime, message center alert and red brake warning indicator in the instrument cluster, and may experience a change in brake pedal travel and increased pedal effort.
