On [XXX], about 20 minutes into my 30-minute commute, while driving at highway speed, my trucks transmission suddenly downshifted dramatically, jerking and throwing me forward in my seat and causing the truck to slow down so abruptly that I was almost rear ended, putting drivers around and behind me at risk of high-speed collisions. Less than 5 minutes later it happened a second time. This is the exact same abrupt transmission downshift issue that happened to me in this vehicle a few times before in early 2024. I received a Safety Recall notice on this known issue about a week after the first incident in 2024. I took my vehicle into the AutoNation Ford Dealer in Littleton, Colorado immediately after I received that notice and they supposedly 'fixed' the issue under the recall 24S37. Obviously, the issue has not been resolved. I am asking for a deeper look into the safety issue as I am sure I am not the only one this is happening to. I am also concerned that there were no warning lamps or messages that would indicate a problem that needed to be addressed! The truck is currently back at the AutoNation Ford Dealer in Littleton and is available for inspection. They are telling me there is no current recall for this issue. How can that be true if it’s the exact same problem and their Safety Recall ‘repair’ didn’t work? I believe the repair was just a software patch, but I can’t be sure. Can you please help me find out what the recall (24S37) was supposed to fix and why it didn’t fix the problem? Their suggestion now is to replace the Valve Body with a cost of $2,554.63 (in addition to a $219.95 diagnostic fee). My concern is that even this expensive repair might not actually solve the problem, and I am truly afraid to drive this vehicle. As is the way with a lot of f150s, the leadframe control module in my automatic transmission failed, causing me to suddenly go into limp mode on [XXX] ; I had no prior warning until the dash went blank and the wrench symbol appeared. I was able to get the truck completely off the roadway and had to wait for a tow. As it was later in the afternoon I had the truck towed to my house and contacted my normal repair facility in the morning. I had the truck towed to the repair facility that day. They diagnosed the leadframe failure, ordered the parts from Ford and I had my truck back within a few days. This part has been a known issue in these F150s - I had the software update "recall" performed on my truck September 2024 and it still happened to me. Additionally, the engine was revving. The contact stated that upon slamming on the brake pedal, the vehicle responded as needed while stopped. The contact stated that while almost coming to a stop and then accelerating, the vehicle jerked, accelerated unintendedly, then responded as needed. The contact stated that the vehicle was taken to the dealer and was repaired under NHTSA Campaign Number: 24V444000 (Power Train); and the failure ceased temporarily. The contact stated that the failure had been recurring. The dealer was contacted and informed the contact that a fee would be charged for a diagnostic test and for the repair. The vehicle was not yet diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 93,000.

Fuse 27, a known problem with the 12th generation F150, manufactured from 2009-2014, burned through in my truck melting the fuse block in the process. The fuse block in question is the main fuse block located toward the front of the engine bay near the radiator. This resulted in a no start condition while parked in a parking lot. I was required to purchase Ford’s repair kit for their engineering problem and relocate the fuse to a new spot in the fuse block. If this had happened while driving, I fear my truck could have resulted in an electrical fire with my family inside. Please consider requiring Ford to issue a safety recall for this incredibly dangerous situation.

The contact stated that while driving at 55 MPH, the transmission unexpectedly downshifted from the 6th to 2nd gear, and the rear wheels locked up, causing the vehicle to slide to a stop. The vehicle was taken to the residence, where a handheld diagnostic machine retrieved undisclosed codes. The vehicle was not diagnosed or repaired. Just seen about recall of f150 shifting problem. On a recent trip my 2014 f150 did exactly that. I had to drive several miles before I could pull over, My display showed a wrench on the screen. I turned off the engine and let it cool down, started it again. The wrench icon did not come on. It seemed to drive normal after that. I'm not sure this problem is with this year as well. I have not had it looked at by any shop.

Ford Motor Company issued a recall for my truck (the transmission could suddenly shift into neutral), stating that they needed to update the PCM, and my local dealership service center said they could send an employee out to fix it remotely. I had not experienced the shifting into neutral prior to the "fix". Less than a week later, as I was driving around 35mph, the speedometer dropped to 0 and the transmission stuck in 3rd gear. I waited until the next stop sign and turned off the engine and started it again and it went back to normal. The next day the same issue occurred. The speedometer would drop to 0 and the transmission would stick in whichever gear it was last in. This time is was 5th gear on a highway. I had to pull over and restart it 3 times before it resumed normal driving. I went back to the dealership and they said the electronic update wouldn't have caused the OSS to malfunction and that it was a coincidence that the exact issue I was having was the same as the cause and reason for the recall. I went to an independent Transmission mechanic to have the OSS replaced and it has now been over a month and the problem has not returned. I don't know if the cause of the OSS malfunctioning was a coincidence or if it was related to a faulty electronic update (like if the Ford mechanic didn't disconnect the battery or overloaded an electric charge when he updated the PCM). The warning sign of a wrench did appear each time the OSS would drop to 0. The check engine light went on the first time but not after that.

A loss of signal between the transmission output shaft speed sensor and the powertrain control module (PCM) can cause the transmission to unexpectedly downshift into first gear.

Consequence & remedy

Consequence: Unexpectedly downshifting into first gear may result in a loss of vehicle control, increasing the risk of a crash.

Remedy: Dealers will update the PCM software, free of charge. Owner notification letters were mailed July 8, 2026. Owners may contact customer service at 1-866-436-7332. Ford Motor Company (Ford) is recalling certain model year 2013-2014 F-150 vehicles manufactured August 1, 2013, to August 31, 2014 equipped with 3.5L engines. The master cylinder rear cup seal may roll, resulting in brake fluid leakage from the primary reservoir into the brake booster.

Consequence & remedy

Consequence: Loss of brake fluid from the primary reservoir results in a complete loss of front brake function, increasing the risk of a crash.

Ford Motor Company (Ford) is recalling certain model year 2014 Ford F-150 and Flex vehicles manufactured March 1, 2014, to May 31, 2014. The affected vehicles were manufactured with a reduced clearance between the seat frame and the seat track assembly. This can cause the Occupant Classification System (OCS) to inaccurately register passenger weight and improperly suppress the passenger front airbag when the passenger seat is occupied by an adult passenger. Thus, these vehicles fail to comply with Federal Motor Vehicle Safety Standard (FMVSS) No. 208, "Occupant Crash Protection."

Consequence & remedy

Consequence: Improper suppression of the passenger airbag increases the risk of an injury to the seat occupant in the event of a crash necessitating deployment of the front passenger airbag.

Consequence & remedy

Consequence: If the brake lights fail to illuminate, or illuminate after a delay, there is an increased risk of a crash.

The affected vehicles may have an incorrectly installed EPAS gear motor position sensor magnet that can lead to a total loss of steering control while driving.

Consequence & remedy

Consequence: A loss of steering control while driving increases the risk of a vehicle crash.

Remedy: Ford will notify owners, and dealers will replace the EPAS steering gear, free of charge. The recall began in July 2014. In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24-005). The agency reserves the right to take additional action if warranted by future circumstances.
