My 2014 Ford F-150 was previously included in Ford Safety Recall 24S37 regarding unintended transmission downshifts caused by an intermittent Output Shaft Speed (OSS) sensor issue. The vehicle was inspected by a Ford mobile technician on January 20, 2026. No qualifying codes were found at that time, and the recall was closed under my VIN. The recall involved the risk of an unintended downshift into first gear that could result in loss of vehicle control. On August 20, 2026, while my husband was driving approximately 40-50 mph, the truck suddenly downshifted and rapidly lost speed. The vehicle would not accelerate beyond approximately 10-15 mph and had to be moved into a parking lot to avoid remaining in traffic. After parking, the truck would no longer move under its own power. The wrench warning light illuminated and the vehicle became undrivable. The symptoms experienced appear substantially similar to those described in Recall 24S37. The dealership has indicated that the current failure may involve the transmission lead frame and speed sensor system, but I am concerned that this event may be related to the same transmission downshift condition addressed by the recall. I am requesting that NHTSA document and review this incident.

I was driving on the highway at about 60-70 mph and my truck shifted into first gear with my tires basically locking up and slid along the highway. I brought my truck into a ford dealership and they told me they couldn’t get it to replicate the incident so they turned me away. I personally replaced my battery terminals and it hasn’t done it again. Now I will be driving and randomly my truck will rev bomb and my gauge cluster will shut off and it shows up with an orange wrench symbol but when I turn the car off it resets. The research I’ve done it can come from a speed sensor or the lead frame. I know 2014 fords have a recall on transmissions but my truck does not have a documented one but there is something funky going on with it.

On July 2, 2026, the vehicle was brought to an authorized dealer for a scheduled appointment to resolve active Safety Recall 26S45 / NHTSA Campaign 26V378 (6R80 transmission sudden downshift defect). The dealership refused to perform the recall software update, claiming it was not needed for the vehicle. On July 12, 2026, while traveling at highway speeds on a public highway, the transmission suffered the exact un-commanded sudden downshift described in the recall. The violent mechanical force instantly snapped the rear driveshaft in half and severed the undercarriage air conditioning lines, creating a sudden, life-threatening loss of control and wheel speed reduction on a live highway. The vehicle is now permanently disabled due to the dealer's refusal to fulfill a federal safety mandate.

I am the owner of a 2014 Ford F-150 that is experiencing sudden, unexpected downshifts into a lower gear while driving at highway speed. This has happened on multiple occasions and has nearly caused accidents, including loss of vehicle control momentarily. This defect matches the description of Ford safety recall 26S45 (loss of signal between the transmission output shaft speed sensor and the powertrain control module, causing unexpected downshift into first gear). Despite this, my vehicle was not included when I checked recall status, even though I have an open Ford claim number: Cxh-08412521-F6L5j4. I am filing this complaint to ensure this incident is on record given the ongoing safety risk, and to request that NHTSA confirm whether my VIN should be captured under recall 26S45 or a related campaign (24S37/24V444). Description: Transmission suddenly and dramatically downshifts at highway speed without warning, causing loss of vehicle control and near-accidents.

Hello, I recently brought my 2014 Ford F-150 (VIN: [XXX]) to the dealership regarding Recall 24S37. The vehicle experienced the following symptoms: • Check Engine Light illuminated. • Wrench Light illuminated. • Intermittent loss of power. • Abnormal transmission behavior. • Intermittent no-start condition. • Vehicle would only start after moving the gear selector. The dealership informed me that no physical repairs would be performed because the diagnostic trouble codes retrieved were different from those associated with Recall 24S37. However, the vehicle continues to exhibit transmission-related symptoms, and I am concerned that the underlying condition may be related to or aggravated by the conditions addressed in Recall 24S37. I am requesting that Ford review this case and determine whether additional diagnostic evaluation or repairs should be considered. However, they refuse to accept that the codes have anything to do with the recall, even though it is the same part and the affected sensor that is triggering the error codes on the scanner. They are asking me to pay for the transmission repair because the codes are not the ones approved for the recall, but based on the signs and symptoms the truck is showing, it is a chain reaction that triggered the previously mentioned failures. My 2014 Ford F-150 is exhibiting symptoms consistent with the Ford transmission molded lead frame safety recall 24S37. While driving approximately 55 mph late at night on a five-lane roadway, the vehicle suddenly and violently jerked, downshifted and came to a complete stop becoming stalled out as I was approaching a major traffic intersection. The truck was disabled in traffic and would not restart for approximately 2-3 minutes, leaving me stranded in an active travel lane and at risk of being struck from behind by another vehicle at high speed. The vehicle has displayed transmission-related symptoms including inability to correctly indicate gear position, harsh shifting, unexpected downshifts into 1st gear while driving, transmission fault warnings, and check engine lights. Initially it was mostly dying while idling and we thought it was the common "Fuse 27" issue, being unaware of the recall. Then the condition reproduced multiple times and symptoms worsened dramatically. Ford dealership inspected the vehicle. According to the technician's written report, the truck was found to be "having transmission problems, unable see what gear the truck is in and shifts to 1st gear while driving" and recommended replacement of the molded lead frame. The dealership confirmed transmission-related diagnostic trouble codes were present and documented symptoms consistent with lead frame failure. Despite the recommendation of the lead frame replacement and documenting symptoms consistent with the safety recall, the repair was not approved. The vehicle continues to experience symptoms associated with the recalled defect and is unsafe to drive away. Dealership confirms software update will not correct problem. I am submitting this complaint as the transmission defect poses a significant safety hazard to me, my children, and other drivers due to unexpected downshifts, loss of power, stalling, and the risk of becoming disabled in traffic. The truck remains available to inspect

I talked to the Ford dealership by my house and they told me I needed to buy a new transmission from them and there was no remedy for my problem other than the new transmission. I noticed the are recalling the 2015 trucks with the same transmission problem. Will my truck be added to the recall ? I have the same transmission

RQ24005 · Unintended Transmission Downshift

Opened Mar 27, 2024 · Closed Jul 29, 2024

Status: closed (inferred from source dates) · Power Train:automatic Transmission:control Module (tcm/pcm/tecm)

In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%).
