Major transmission failure that causes sudden unintended downshift to first gear, even at high speeds. This occurs often and randomly without warning, causing the engine to "redline", rear tires to slide and the vehicle to come to a near sudden stop. This is an extreme safety concern, especially in slippery road conditions as the vehicle could overturn or slide off the road or at a minimum be rear-ended for suddenly stopping unexpectedly on a freeway. This is a widely known problem by Ford. The local Ford service manager admitted that his 2013 F150 has the same issue and he continues to drive it because he tried fixing it by replacing the Molded Lead Plate in the transmission and that didn't resolve the problem. Remember, I noted this is a Ford Service Manager! Ford has issued a recall (24S37); however, the prescribed solution is to have the Powertrain Control Module (PCM) software updated and that prescribed fix actually makes the sudden downshifting happen more often. This is the cheapest, band-aid solution that Ford hides behind so they can say that they are addressing this issue. A second Ford dealership down the road actually admitted that they have identified that the recommended software update actually makes the problem worse and they practically refuses to perform the software update for their customers. After several discussions with Ford's Recall Assistance Team and several discussions with a local dealership's service manager, Ford is not willing to do anything more, despite this major safety issue not only not getting resolved, but actually has gotten worse due to their band-aid approach of a software update. In my case, if further diagnosis is to be performed, I have to be the one that covers that cost and also I would be the one to cover the cost of repairs. To me, this is completely unacceptable. The rational that Ford uses to deny accountability is that the vehicle's computer doesn't register any DTC codes when the incident occurs.

The contact owns a 2014 Ford F-150. The contact stated while driving highway speeds, the transmission unexpectedly downshifted to first gear without warning. The vehicle was taken to the local dealer where the failure was acknowledged, and the dealer performed a software update that the contact was informed would solve the failure. Despite the software update, the failure worsened upon return of the vehicle. The contact then reached out to another dealer that stated that the software update was no longer performed due to the update worsening the failure. The dealer refused to service the vehicle and directed the contact back to the original dealer. The vehicle was not repaired. The manufacturer was notified of the failure and acknowledged that the failure was a known issue. The manufacturer also informed the contact of NHTSA Campaign Number: 24V444000 (Power Train) and a case was filed. The failure mileage was unknown.

After recall 24S37 was performed on my Ford F-150 to address potential sudden downshifting to first gear, my truck began experiencing serious transmission issues that had never occurred prior. The recall required PCM reprogramming, and immediately afterward, the vehicle started hard shifting and unexpectedly downshifting at highway speeds (one example 5th to 3rd gear and also while passing or decelerating). This was alarming and unsafe. Before this recall, the transmission operated perfectly with no symptoms, warning lights, or signs of malfunction. I returned the truck to the Ford dealership that performed the recall. Their road test confirmed the issue, and they diagnosed it as damage to the overdrive clutch. However, they stated the damage was “likely present before” the recall work—despite no prior symptoms—and recommended full transmission replacement at a cost of nearly $10,000, which I declined. The issue clearly began immediately after Ford’s recall remedy and fits the exact risk the recall was supposed to prevent. The vehicle is available for inspection. I believe the recall work either caused or triggered the failure and that Ford should be held accountable.

After I had the recall repair was done my truck began acting erratic so I took it back to Duval Ford three times before my truck became undrivable.On 1/10/2025 Duval Ford Could not dispute my Claim.They repaired my transmission and every time I drive my truck i feel that I may have the same issues as stated in the recall letter.The didn't want to give me a warranty I only got a 12 month or 12 k miles warranty. I feel that they did not see the seriousness of the situation and I want other to know the impact that this could have on their lives.Thank you for being there for the consumer.

[XXX] was driving the 2014 F150 on a multi lane state road in the afternoon on a sunny and dry afternoon. The vehicle was traveling straight with steady traffic at an intense rate of speed (approximately 45-50 mph) when it all of a changed gears and became difficult for the [XXX] to control, causing the vehicle to hit a guard rail. There was no contributing factors to the accident other than the vehicle's malfunction (i.e. no negative weather, road signals, other vehicles, animal or plant obstructions, intersections, driver error, etc.). At the time of the impact, all of the vehicle's airbags deployed and the driver's seatbelt locked in place. There were no personal injuries - only property damage to the outside of the vehicle, the airbags and seatbelt. Thus far, Ford has refused to replace the airbags, seatbelt/repair the vehicle's property damage despite repeated requests. Ford only fixed the software problem associate with the recall and has taken the position that is the only remedy they are required to do. The contact owns a 2014 Ford F-150. The contact stated that the vehicle was taken to a dealer and was repaired under NHTSA Campaign Number: 24V444000 (Power Train). The contact stated that upon driving the vehicle from the dealer's parking lot, the vehicle experienced hard transmission shifts to low gear and to the higher gears. The was no warning light illuminated. Additionally, the engine revved significantly. The vehicle was taken back to the dealer, where the contact was informed that the failure was not caused by the recall remedy but could be related to the transmission. The vehicle was taken to Master Tech Auto Repair, where a transmission replacement was recommended. The transmission was replaced; however, the failure persisted. The contact stated that the vehicle was scanned with a code reader; however, no diagnostic trouble code was retrieved. The contact stated that the failure had progressively worsened. The contact stated that the vehicle was not experiencing a failure prior to the recall repair, and the recall remedy had failed to prevent the failure. The manufacturer was notified of the failure, and a case was filed. The manufacturer also referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 101,000.

As is the way with a lot of f150s, the leadframe control module in my automatic transmission failed, causing me to suddenly go into limp mode on [XXX] ; I had no prior warning until the dash went blank and the wrench symbol appeared. I was able to get the truck completely off the roadway and had to wait for a tow. As it was later in the afternoon I had the truck towed to my house and contacted my normal repair facility in the morning. I had the truck towed to the repair facility that day. They diagnosed the leadframe failure, ordered the parts from Ford and I had my truck back within a few days. This part has been a known issue in these F150s - I had the software update "recall" performed on my truck September 2024 and it still happened to me. The contact stated that while almost coming to a stop and then accelerating, the vehicle jerked, accelerated unintendedly, then responded as needed. The contact stated that the vehicle was taken to the dealer and was repaired under NHTSA Campaign Number: 24V444000 (Power Train); and the failure ceased temporarily. The contact stated that the failure had been recurring. The dealer was contacted and informed the contact that a fee would be charged for a diagnostic test and for the repair. A loss of signal between the transmission output shaft speed sensor and the powertrain control module (PCM) can cause the transmission to unexpectedly downshift into first gear.

Consequence & remedy

Consequence: Unexpectedly downshifting into first gear may result in a loss of vehicle control, increasing the risk of a crash.

Remedy: Dealers will update the PCM software, free of charge. Owner notification letters were mailed July 8, 2026. Owners may contact customer service at 1-866-436-7332. Ford Motor Company (Ford) is recalling certain model year 2013-2014 F-150 vehicles manufactured August 1, 2013, to August 31, 2014 equipped with 3.5L engines. The master cylinder rear cup seal may roll, resulting in brake fluid leakage from the primary reservoir into the brake booster.

Consequence & remedy

Consequence: Loss of brake fluid from the primary reservoir results in a complete loss of front brake function, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the brake master cylinder and, if necessary, the brake booster, free of charge. The recall began on October 28, 2016. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 16S24.

Ford Motor Company (Ford) is recalling certain model year 2014 Ford F-150 and Flex vehicles manufactured March 1, 2014, to May 31, 2014. The affected vehicles were manufactured with a reduced clearance between the seat frame and the seat track assembly. This can cause the Occupant Classification System (OCS) to inaccurately register passenger weight and improperly suppress the passenger front airbag when the passenger seat is occupied by an adult passenger. Thus, these vehicles fail to comply with Federal Motor Vehicle Safety Standard (FMVSS) No. 208, "Occupant Crash Protection."

Consequence & remedy

Consequence: Improper suppression of the passenger airbag increases the risk of an injury to the seat occupant in the event of a crash necessitating deployment of the front passenger airbag.

Remedy: Ford will notify owners, and dealers will widen the gap between the seat frame and seat track assembly and recalibrate the seat OCS, free of charge. The recall began November 25, 2014. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 14C10.

The affected vehicles may have an incorrectly installed EPAS gear motor position sensor magnet that can lead to a total loss of steering control while driving.

Consequence & remedy

Consequence: A loss of steering control while driving increases the risk of a vehicle crash.

Remedy: Ford will notify owners, and dealers will replace the EPAS steering gear, free of charge. The recall began in July 2014. RQ24005 · Unintended Transmission Downshift

In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24-005). The agency reserves the right to take additional action if warranted by future circumstances.
