The contact owns a 2014 Ford F-150. The contact stated that while driving at 55 MPH, the transmission unexpectedly downshifted from the 6th to 2nd gear, and the rear wheels locked up, causing the vehicle to slide to a stop. The vehicle then stalled but was restarted after several attempts. The contact stated that once the vehicle was restarted, there was a knocking sound coming from the engine compartment with smoke exiting from the tailpipe. The vehicle was taken to the residence, where a handheld diagnostic machine retrieved undisclosed codes. The vehicle was not diagnosed or repaired. The contact then received notification of NHTSA Campaign Number: 24V444000 (Power Train). The manufacturer was not notified of the failure. The failure mileage was 217,000.

Just seen about recall of f150 shifting problem. On a recent trip my 2014 f150 did exactly that. While making an right turn onto a street as it shifted it jerked pretty bad, scared the heck out of me. as I drove it would not shift out of 3rd gear. I had to drive several miles before I could pull over, My display showed a wrench on the screen. I turned off the engine and let it cool down, started it again. The wrench icon did not come on. It seemed to drive normal after that. I'm not sure this problem is with this year as well. I have not had it looked at by any shop.

Ford Motor Company issued a recall for my truck (the transmission could suddenly shift into neutral), stating that they needed to update the PCM, and my local dealership service center said they could send an employee out to fix it remotely. I had not experienced the shifting into neutral prior to the "fix". Less than a week later, as I was driving around 35mph, the speedometer dropped to 0 and the transmission stuck in 3rd gear. I waited until the next stop sign and turned off the engine and started it again and it went back to normal. The next day the same issue occurred. The speedometer would drop to 0 and the transmission would stick in whichever gear it was last in. This time is was 5th gear on a highway. I had to pull over and restart it 3 times before it resumed normal driving. I went back to the dealership and they said the electronic update wouldn't have caused the OSS to malfunction and that it was a coincidence that the exact issue I was having was the same as the cause and reason for the recall. I went to an independent Transmission mechanic to have the OSS replaced and it has now been over a month and the problem has not returned. I don't know if the cause of the OSS malfunctioning was a coincidence or if it was related to a faulty electronic update (like if the Ford mechanic didn't disconnect the battery or overloaded an electric charge when he updated the PCM). The warning sign of a wrench did appear each time the OSS would drop to 0. The check engine light went on the first time but not after that.

The contact stated that the vehicle was repaired under NHTSA Campaign Number: 24V444000 (Power Train); however, the repair failed to prevent a failure from occurring. The contact stated that 9 months after the recall repair was completed, while accelerating from a stop, the RPM increased; however, the vehicle failed to accelerate as intended while the accelerator pedal was depressed. The contact pulled over into a parking lot, and the vehicle was towed to an independent mechanic to be diagnosed; however, no fault codes were retrieved. The mechanic was unable to duplicate the failure. The local dealer was contacted. The vehicle was not diagnosed or repaired due to cost. The manufacturer was made aware of the failure. The failure mileage was approximately 125,000.

I received a recall notice on my F150 and took it to the dealership for repair even though I have not had any check engine lights or codes on my vehicle. 20 days After taking my vehicle to the ford dealership for the 24S37 recall on the OSS while driving on a two lane back road my truck experienced a hard downshift immediately followed by a check engine light. I lost all acceleration and had to coast to a safe place to pull over. I connected a code reader to the vehicle and immediately got a P0721 code for output speed sensor. I had the truck towed to the dealership and was told they drove it and could not get it to repeat the issue. The dealership only drove 2.9 miles while the had the vehicle for 5 days. I picked up the truck and drove 122 miles before the truck repeated the same issue only this time I was on the highway and luckily was able to exit the highway safely. I connected a code reader to the truck again and received a code for P0720 for the output speed sensor. The software update for this recall that FORD has pushed out as a repair has done nothing but cause even more substantial problems and safety concerns. I have reached out to Ford (at a manufacturer level) and have received absolutely no assistance from them.

The contact stated that after the recall repair, the vehicle started to experience the failure listed in the recall. The contact stated that the day the vehicle was picked up from the dealer, the vehicle abruptly downshifted while depressing the accelerator pedal. The contact also stated that an abnormal, banging noise was coming from the vehicle with the check engine warning light illuminated. The contact informed the dealer immediately about the failure and was provided an estimate for a diagnostic test. The vehicle was then taken to an independent mechanic where the mechanic replaced the motherboard inside the transmission; however, the failure persisted. The vehicle was taken back to the independent mechanic where it remained. The manufacturer was not notified of the failure. The vehicle was not repaired. The failure mileage was approximately 219,000.

Truck started downshifting to 2nd gear at highway speeds, submitted claim, (no recall) Local Ford said no problem with my transmission. I replaced the leadframe in transmission. This fixed the problem. Ford came out with recall, Ford mobile came out and updated software for transmission. Since software update, Started down shifting again, sticking in 5th gear, or sticking in 3rd gear. Wrench came on a few times and engine light randomly comes on and clears too. I have pictures of 5th gear going slow, the wrench appearing and engine toruble light on. See attached pictures.

Ford Motor Company (Ford) is recalling certain model year 2014 F-150 vehicles previously repaired incorrectly under recall number 24V444. A loss of signal between the transmission output shaft speed sensor and the powertrain control module (PCM) can cause the transmission to unexpectedly downshift into first gear.

Consequence & remedy

Consequence: Unexpectedly downshifting into first gear may result in a loss of vehicle control, increasing the risk of a crash.

Remedy: Dealers will update the PCM software, free of charge. Owner notification letters were mailed July 8, 2026. Owners may contact customer service at 1-866-436-7332. Ford's number for this recall is 26S45. Vehicle Identification Numbers (VINs) involved in this recall will become searchable on NHTSA.gov on July 6, 2026.

Ford Motor Company (Ford) is recalling certain 2014 F-150 vehicles. A loss of signal between the transmission output shaft speed sensor and the powertrain control module can cause the transmission to unexpectedly downshift into first gear, regardless of vehicle speed.

Consequence & remedy

Consequence: Unexpectedly downshifting into first gear may result in a loss of vehicle control, increasing the risk of a crash.

Consequence & remedy

Consequence: A loss of brake fluid may result in a change in brake pedal travel and feel, increased pedal effort and reduced front brake function, possibly requiring greater effort and distance to stop the vehicle. These conditions can increase the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the brake master cylinder and inspect the brake booster, replacing it as necessary, free of charge. Ford Motor Company (Ford) is recalling certain model year 2013-2014 F-150 vehicles manufactured August 1, 2013, to August 31, 2014 equipped with 3.5L engines. The master cylinder rear cup seal may roll, resulting in brake fluid leakage from the primary reservoir into the brake booster.

Consequence & remedy

Consequence: Loss of brake fluid from the primary reservoir results in a complete loss of front brake function, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the brake master cylinder and, if necessary, the brake booster, free of charge. The recall began on October 28, 2016. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 16S24.

Ford Motor Company (Ford) is recalling certain model year 2014 Ford F-150 and Flex vehicles manufactured March 1, 2014, to May 31, 2014. The affected vehicles were manufactured with a reduced clearance between the seat frame and the seat track assembly. This can cause the Occupant Classification System (OCS) to inaccurately register passenger weight and improperly suppress the passenger front airbag when the passenger seat is occupied by an adult passenger. Thus, these vehicles fail to comply with Federal Motor Vehicle Safety Standard (FMVSS) No. 208, "Occupant Crash Protection."

Consequence & remedy

Consequence: Improper suppression of the passenger airbag increases the risk of an injury to the seat occupant in the event of a crash necessitating deployment of the front passenger airbag.

Remedy: Ford will notify owners, and dealers will widen the gap between the seat frame and seat track assembly and recalibrate the seat OCS, free of charge. The recall began November 25, 2014. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 14C10.

Consequence & remedy

Consequence: If the brake lights fail to illuminate, or illuminate after a delay, there is an increased risk of a crash.

Ford Motor Company (Ford) is recalling certain model year 2014 Ford F-150 trucks manufactured May 26, 2014 to June 19, 2014, and equipped with Electronic Power Assist Steering (EPAS). The affected vehicles may have an incorrectly installed EPAS gear motor position sensor magnet that can lead to a total loss of steering control while driving.

Consequence & remedy

Consequence: A loss of steering control while driving increases the risk of a vehicle crash.

Remedy: Ford will notify owners, and dealers will replace the EPAS steering gear, free of charge. The recall began in July 2014. RQ24005 · Unintended Transmission Downshift

In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24-005). The agency reserves the right to take additional action if warranted by future circumstances.
