THE THROTTLE BODY FAILED CAUSING THE VEHICLE TO GO INTO LIMP MODE. THIS HAPPENED ON A BACK ROAD BUT IF IT HAD OCCURRED ON A TWO LANE ROAD WHILE PASSING IT COULD HAVE BEEN VERY DANGEROUS. A TROUBLE LIGHT CAME ON BUT WHEN THE VEHICLE WAS TURNED OFF AND BACK ON AGAIN THE TROUBLE LIGHT WAS OFF AND THE TRUCK WOULD DRIVE NORMALLY. THE CODE WAS NOT HELD IN THE MEMORY SO THE DEALERSHIP WOULD NOT CHANGE IT UNDER WARRANTY. I KNOW OF ONE OTHER PERSON WITH THE SAME YEAR AND POWER TRAIN THAT HAD THE SAME PROBLEM.

MY VEHICLE FAILED TO START. AFTER HAVING THE VEHICLE TOWED TO MY DEALER, I WAS TOLD THE ISSUE WAS A BLOWN FUSE. I ASKED THEM TO REPLACE AND WAS TOLD THE ACTUAL FIX IS TO RELOCATE THE FUSE LOCATION BASED ON A SAFETY BULLETIN. I BELIEVE THIS IS A MAJOR SAFETY ISSUE AND AFTER READING ALL THE OTHER COMPLAINTS BELIEVE SOMEONE SHOULD TAKE A CLOSER LOOK. IF I HAD BEEN DRIVING ON THE HIGHWAY WHEN THIS KNOWN FUSE ISSUE HAPPENED I COULD HAVE LOST CONTROL AND WOULD HAVE CERTAINLY LOST POWER UNEXPECTEDLY POSSIBLY CAUSING AN ACCIDENT. FORD HAS KNOWN ABOUT THE PROBLEM FOR YEARS BUT HAS CONTINUED TO CHARGE OWNERS TO RELOCATE THE FUSE WHEN THE WHOLE ISSUE IS A FACTORY DEFECT. IF IT WAS NOT A FACTORY DEFECT THEN THE ISSUE WOULD NOT CONTINUE TO AFFECT SO MANY VEHICLES ON THE ROAD. I WAS LUCKY MY FUSE BLEW IN MY DRIVEWAY BUT OTHERS ARE AT RISK SHOULD THIS HAPPEN WHILE DRIVING.

ON 2 SEPARATE OCCASIONS WITH ME AND 1 OCCASION WITH A FORD SERVICE TECH. (ALL OF WHICH OCCURRED BETWEEN THE DATES OF SEPT 13 AND 18, 2019) MY F150 ENGINE SHUTDOWN WHILE DRIVING. MY FIRST OCCURRENCE WAS WHILE ATTEMPTING TO ACCELERATE ONTO INTERSTATE 24 IN CHATTANOOGA,TN. SECOND OCCURRENCE WAS WHEN I WAS DRIVING IN HEAVY AFTERNOON TRAFFIC ON US HWY 27 IN ROSSVILLE, GA. BOTH TIMES I WAS ABLE TO COAST THE VEHICLE TO SIDE OF THE ROAD OR PARKING LOT TO RE START VEHICLE. AFTER EACH OCCURENCE I TOOK VEHICLE TO A MECHANIC TO CHECK VEHICLE. THE FIRST TIME WAS A LOCAL SHOP DUE TO KNOW DEALERSHIPS CLOSE BY, SECOND TIME I WAS CLOSE TO MTN VIEW FORD IN CHATTANOOGA, TN. AFTER TROUBLE SHOOTING AND VEHICLE SHUTTING OFF ON THE TECHNICIAN, THEY WERE ABLE TO TRACE THE USE DOWN THAT IS PICTURED. THE TRUCK DID NOT LEAVE ANY ERROR CODES OR INDICATIONS TO ISSUE. WHEN THE VEHICLE WAS NOT CAUSING THE IMMEDIATE ISSUE IT WAS RUNNING FINE WITH NO PROBLEMS INDICATED. THE TECHNICIAN DID INDICATE TO ME THAT THIS HAS HAPPENED SEVERAL TIMES THAT HE WAS AWARE OF. WITH THAT BEING SAID, I FEEL THAT THIS ISSUE SHOULD HAVE BEEN PLACED INTO A RECALL DUE TO THE DANGER THAT I ENCOUNTERED WITH THE VEHICLE SHUTTING OFF WHILE ON INTERSTATE AND MAJOR HIGHWAY WITH NO INDICATIONS OF ISSUES. I HAVE ATTEMPTED TO CONTACT FORD ABOUT THIS ISSUE BUT THEIR AUTOMATED SYSTEM DOES NOT OFFER A PATH TO REPORT ISSUES.I WAS TOLD THAT THIS FUSE IS THE ONE FOR THE FUEL PUMP. MDF

WE THEN TOOK IT TO AN INDEPENDENT ASA MASTER CERTIFIED TECHNICIAN WITH 38 YEARS EXPERIENCE. HE INFORMED US THAT FORD HAS HAD THIS PROBLEM FOR YEARS AND WILL NOT ADMIT THERE IS A PROBLEM. THROTTLE SYSTEM WAS CLEANED AND WE WILL SEE IF THIS CORRECTS ISSUE. MY PROBLEM IS WITH FORD AND DEALERSHIP THAT DOES NOT EXPLAIN THIS TO THE CUSTOMER. THIS IS AN EXTREMELY DANGEROUS SITUATION AS THE TRUCK JUST STOPS WITH NO WARNING AT 70 MILES AN HOUR! FORD SHOULD TELL THE CUSTOMERS WHAT IS WRONG SO IT CAN BE FIXED INSTEAD OF HIDING THE PROBLEM. I WENT ON THE INTERNET AND READ ABOUT THIS HAPPENING TO OTHER PEOPLE (A BIG ORANGE WRENCH APPEARS IN DISPLAY WHEN IT HAPPENS). THEY KNOW WHAT THE PROBLEM IS!! SOMEONE IS GOING TO DIE AND MAY HAVE ALREADY DUE TO THE LACK OF TRANSPARENCY. IT'S AS IF THEY ACKNOWLEDGE THE PROBLEM THEY ARE ADMITTING THERE IS ONE AND THEN BECOME LIABLE TO CORRECT IT. THIS IS A SAFETY ISSUE AND PEOPLE NEED TO BE MADE AWARE OF IT.

I ALSO SINCE THEN INSTALLED A VAPOR CATCH CAN FOR OVER 400$. IT'S A SHAME YOU HAVE TO DO THAT. ALSO I RECENTLY HAD MY REAR DEFROST ON THE STOP WORKING ON THE MODEL WITH THE REAR SLIDING WINDOW. WHICH AGAIN HAS MANY COMPLAINTS. FORDS FIX IS TO REPLACE THE WHOLE REAR WINDOW AT A HEAVY PRICE. BOX BEING ABLE TO DEFROST IS A MAJOR VISIBILITY ISSUE ESPECIALLY IN THE WINTER. BOTH ISSUES HAVE MANY COMPLAINT ONLINE AND FORD HASN'T FIXED EITHER. THE PICTURE OF THE COKE BOTTLE IS WHAT DRAINED OUT OF THE CATCH CAN BY NPR. IT WAS 1 MONTH OF COMMUTING AND A 8 HOUR ROAD TRIP.

RQ24005 · Unintended Transmission Downshift

Opened Mar 27, 2024 · Closed Jul 29, 2024

Status: closed (inferred from source dates) · Power Train:automatic Transmission:control Module (tcm/pcm/tecm)

In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24-005). The agency reserves the right to take additional action if warranted by future circumstances.
