TOOK IT INTO FORD, THEY REPLACED THE BATTERY. DIDN'T HAPPEN FOR A FEW MORE DAYS. SAME THING. CRANK, CRANK, CRANK, BUT WOULDN'T START. DID SOME SEARCHING ONLINE, AND FOUND SIMILAR SITUATION REGARDING A BURNT/MELTED FUSE 27 (THE FUEL PUMP FUSE). FORD KNOWS ABOUT THIS ISSUE SO MUCH THAT THEY EVEN CREATED A PART TO MOVE THE FUEL PUMP TO A LARGER FUSE. I'M GLAD THIS ISSUE ONLY HAPPENED WHILE SITTING STILL AND I WAS ABLE TO GET THE FUSED REPLACED WITHOUT INCIDENT. HOWEVER, I WOULD IMAGINE THAT SOON MY VEHICLE WOULD HAVE STOPPED WHILE TRAVELING DUE TO THE FUEL PUMP FUSE COMPLETELY DYING. ALSO SEEMS LIKE A FIRE HAZARD, OR IT COULD DO FURTHER DAMAGE TO THE FUSE BOX OR TO SURROUNDING FUSES. FORD SHOULD ISSUE A RECALL FOR THIS IMMEDIATELY.

ALSO, I CANNOT LOCK MY ENTIRE TRUCK WITH THE KEY FOB BECAUSE OF THIS ISSUE, AND I CAN ONLY DO IT MANUALLY. I FEEL THAT THIS IS A SAFETY ISSUE. ALSO, MY FRONT MAP LIGHTS AND THE REAR WINDOW DEFROST DOES NOT WORK. I LIVE IN A COLD STATE (INDIANA) SO IT WOULD BE NICE TO HAVE THESE ELECTRICAL ISSUES TAKEN CARE OF. JUST LIKE MOST PEOPLE, I HAVE CHILDREN THAT ARE FREQUENTLY IN MY TRUCK. NOTHING HAS BEEN HIT OR DAMAGED IN ANY WAY. THIS JUST STARTED HAPPENING FOR THE FIRST TIME IN JUNE 2019. I'VE TOLD MY LOCAL FORD DEALER, BUT THEY TELL ME THAT SINCE IT IS NOT ON RECALL I WILL HAVE TO PAY FOR IT OUT OF POCKET. I AM A DISABLED VETERAN, AND THAT'S NOT NECESSARILY THE EASIEST THING FOR ME TO DO, ESPECIALLY SINCE IT IS OUT OF WARRANTY. I'VE THOUGH ABOUT GETTING AN EXTENDED WARRANTY, BUT HOW CAN I BE SURE THAT WILL COVER THE COST OF THE REPAIRS? THANK YOU

IT STRANDED MYSELF AND CHILDREN, AND WE HAD A HARROWING EXPERIENCE THAT WAS PRETTY DICEY. FORD HAD JUST PUT IN A NEW ENGINE, AT MY COST, +14,000.00, AFTER THE TIMING CHAIN FAILED. THEY WOULD NOT REPAIR IT AT THEIR COST, EVEN THOUGH I HAVE COPIES OF THE FAULT CODES FROM BEFORE THE FAILURE OF THE TIMING CHAIN THAT DESTROYED THE OLD ENGINE. THE NEW ONE HAD UNDER 10,000 MILES ON THE NEW ENGINE, PROBABLY ABOUT 5,000 MILES ON IT. WE HAD DRIVEN ON THE RELATIVELY FLAT GRADE MAINLINE ROAD, AND DROVE UP ABOUT 0.8 MILES, CLIMBING MAYBE 400 FT ELEVATION OVER THAT DISTANCE. I WAS IN THE PROCESS OF TURNING THE VEHICLE AROUND WHEN THE ENGINE BURST INTO FLAMES. IT WAS ON A GRAVEL ROAD. WE HAD TRAVELED FROM HOME, 35.2 MILES.

I GOT A LEFT REAR DOOR AJAR WARNING ON THE DASH OF MY 2014 FORD F150 CREW CAB PICK UP TRUCK. I WOULD CHECK THE DOOR AND IT WAS CLOSED PROPERLY. EVERY TIME I STARTED THE TRUCK I WOULD GET THE WARNING. I MADE AN APPOINTMENT WITH FORD TO FIND OUT WHY THIS WAS HAPPENING. ON THE DAY OF THE APPOINTMENT WITH FORD, I STARTED THE TRUCK AND HEARD A BUZZING TYPE NOICE FROM THE REAR SEAT AREA. I GOT OUT TO HE TRUCK WITH IT RUNNING AND OPENED THE LEFT REAR DOOR TO SEARCH FOR THE BUZZING NOISE. AS I WAS LOOKING FOR THE NOISE I STARTED TO SMELL PLASTIC BURNING, THEN OUT OF THE LEFT PILLAR BETWEEN THE FRONT AND BACK DOOR SMOKE STARTED POURING OUT OF WHERE THE SEAT BELT GOES BEHIND THE COVER WHERE THE SEAT BELT RETRACTS TO. SMOKE AND THE SMELL OF THE PLASTIC STARTED TO FILL THE CABIN AREA OF THE TRUCK. I SHUT THE TRUCK OFF. WITHIN ABOUT TEN MINUTES THE SMOKE CLEARED BUT THE SMELL OF BURNED PLASTIC WAS STRONG. I THEN HAD THE TRUCK TOWED TO MILLER FORD IN LUMBERTON, NEW JERSEY. A PRELIMINARY REPORT FROM FORD IS THAT THE ELECTRICAL HARNESS IN THE DOOR POST AND UNDER THE CARPET AREA IS MELTED. I AM AWAITING TO HEAR FROM FORD THE FINAL STORY AT THIS TIME.

AUTO STARTED MY TRUCK AND THE DEFROST AUTOMATICALLY TURNS ON AS IT'S WARMING UP THE TRUCK. 5 MIN LATER AFTER GETTING IN THE TRUCK DROVE THE TRUCK FOR 10 SEC OR SO AND I HEARD A LOUD POP! MY GLASS HAD SHATTERED INTO THE PIECES. DID RESEARCH AND SEEN I'M NOT THE ONLY ONE WHO HAS HAD THIS HAPPEN TO THEM. THE DEFROST SENSOR HAS BEEN CAUSING THIS TO THE 09-14 F150 AND FORD IS NOT DOING ANYTHING ABOUT THIS.. I CALLED A COUPLE DEALERSHIPS AND ASKED THEM ABOUT IT AND THEY ALSO TOLD ME THEY HAVE HAD A LOT OF OTHER PEOPLE CALL THEM ABOUT THIS ISSUE. WHAT IS FORD GOING TO DO ABOUT THIS? I DON'T REALLY WANT TO PAY 1K OUT OF POCKET TO FIX SOMETHING THAT'S AN ISSUE WITH THE MANUFACTURER. IF THIS HAS HAPPENED TO ANYONE ELSE, PLEASE PUT IN A COMPLAINT SO THIS CAN GET RESOLVED SOONER THAN LATER! I'M JUST GLAD NO ONE WAS IN THE BACK SEAT WHEN THIS SHATTERED AT IT WAS EXTREMELY LOUD AND COULD'VE SHATTERED ON THEM! THANK YOU !

VEHICLE FIRE. I STARTED THE VEHICLE UP IN MY DRIVEWAY AND HEARD A SMALL POP/EXPLOSION AND SMOKE CAME OUT FROM UNDER THE HOOD. I IMMEDIATELY SHUT OFF THE TRUCK, POPPED THE HOOD AND COULD NOT IDENTIFY THE SOURCE OF THE ISSUE. I STARTED THE TRUCK AGAIN AND NOTICED WHERE THE FIE/SMOKE WAS, SHUT OFF THE TRUCK, AND PROCEEDED TO RUSH INTO THE HOUSE TO GRAB THE KITCHEN FIRE EXTINGUISHER AND PUT THE FIRE OUT. THANKFULLY I WAS NOT DRIVING AND MY HOUSE DIDN'T CATCH FIRE. I CALLED MY MECHANIC AND HAD THE TOW TRUCK DELIVER THE VEHICLE TO HIM AND THEY CONFIRMED THAT IT WAS THE OEM ALTERNATOR THAT WAS THE CAUSE OF THE FIRE. I CONTACTED THE DEALERSHIP WANTING TO REPORT THE ISSUE AND THE DEALERSHIP IN THEIR WORDS SAID "NOT SURE WHAT YOU WANT US TO DO?" NOT ARE YOU OK, THIS IS SERIOUS LET US HELP, ETC. JUST A SIMPLE PASSING THE BUCK TO A 1800 NUMBER TO FORD. I FILED A CASE WITH FORD MOTOR COMPANY, BUT THAT TOO WAS A JOKE. THEY DIDN'T CARE EITHER. I WAS TOLD THEY'LL FILE IT AS NEGATIVE FEEDBACK AND SINCE I WAS SO UPSET, I WAS DIRECTED TO CREATE A COMPLAINT WITH NHTSA. I FOUND THIS VERY DISTURBING AS FORD APPEARS TO NOT HAVE CARED ONE BIT ABOUT THEIR MANUFACTURED VEHICLE CATCHING FIRE OR THE POSSIBILITY OF SOMEONE GETTING HURT OR KILLED. I HOPE THIS COMPLAINT FINDS THE RIGHT PEOPLE AND PREVENTS A REAL ACCIDENT OR TRAGEDY FROM HAPPENING. AT THIS TIME I DON'T HAVE THE REPAIR INVOICES AS THE FIRE DID A DECENT AMOUNT OF DAMAGE EVEN THOUGH I DID PUT IT OUT WITHIN 60 SECONDS OF IT STARTING. THE MECHANICS ARE STILL ASSESSING ALL THE DAMAGE DONE. SINCE THE FIRE WAS IN THE ENGINE BAY AREA ON THE BOTTOM, THERE ISN'T MUCH VISIBLE BURN DAMAGE (AND IT IS CURRENTLY COVERED IN DRY CHEM FROM THE FIRE EXTINGUISHER). I DID FILE A CASE WITH FORD AS MENTIONED AND IF YOU NEED THE CASE NUMBER I'D BE GLAD TO PROVIDE IT.

ON 2 SEPARATE OCCASIONS WITH ME AND 1 OCCASION WITH A FORD SERVICE TECH. (ALL OF WHICH OCCURRED BETWEEN THE DATES OF SEPT 13 AND 18, 2019) MY F150 ENGINE SHUTDOWN WHILE DRIVING. MY FIRST OCCURRENCE WAS WHILE ATTEMPTING TO ACCELERATE ONTO INTERSTATE 24 IN CHATTANOOGA,TN. SECOND OCCURRENCE WAS WHEN I WAS DRIVING IN HEAVY AFTERNOON TRAFFIC ON US HWY 27 IN ROSSVILLE, GA. BOTH TIMES I WAS ABLE TO COAST THE VEHICLE TO SIDE OF THE ROAD OR PARKING LOT TO RE START VEHICLE. AFTER EACH OCCURENCE I TOOK VEHICLE TO A MECHANIC TO CHECK VEHICLE. THE FIRST TIME WAS A LOCAL SHOP DUE TO KNOW DEALERSHIPS CLOSE BY, SECOND TIME I WAS CLOSE TO MTN VIEW FORD IN CHATTANOOGA, TN. AFTER TROUBLE SHOOTING AND VEHICLE SHUTTING OFF ON THE TECHNICIAN, THEY WERE ABLE TO TRACE THE USE DOWN THAT IS PICTURED. THE TRUCK DID NOT LEAVE ANY ERROR CODES OR INDICATIONS TO ISSUE. WHEN THE VEHICLE WAS NOT CAUSING THE IMMEDIATE ISSUE IT WAS RUNNING FINE WITH NO PROBLEMS INDICATED. THE TECHNICIAN DID INDICATE TO ME THAT THIS HAS HAPPENED SEVERAL TIMES THAT HE WAS AWARE OF. WITH THAT BEING SAID, I FEEL THAT THIS ISSUE SHOULD HAVE BEEN PLACED INTO A RECALL DUE TO THE DANGER THAT I ENCOUNTERED WITH THE VEHICLE SHUTTING OFF WHILE ON INTERSTATE AND MAJOR HIGHWAY WITH NO INDICATIONS OF ISSUES. I HAVE ATTEMPTED TO CONTACT FORD ABOUT THIS ISSUE BUT THEIR AUTOMATED SYSTEM DOES NOT OFFER A PATH TO REPORT ISSUES.I WAS TOLD THAT THIS FUSE IS THE ONE FOR THE FUEL PUMP. MDF

RQ24005 · Unintended Transmission Downshift

Opened Mar 27, 2024 · Closed Jul 29, 2024

Status: closed (inferred from source dates) · Power Train:automatic Transmission:control Module (tcm/pcm/tecm)

In September 2016, Ford Motor Company (Ford) issued safety recall 16V-248 which included certain model year (MY) 2011-2012 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions. This recall was in response to allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, increasing the risk of a crash and/or injury, and included approximately 153,581 vehicles. Ford identified the root cause of the unintended, unexpected vehicle transmission downshifting to a lower gear to be intermittent signal loss from the transmission’s Output Shaft Speed (OSS) sensor due to solder joint cracks on the OSS circuit board as a result of manufacturing process issues. The OSS sensor is mounted onto the molded lead frame and transmits changes in transmission shaft speed to the Powertrain Control Module (PCM). The remedy program described in safety recall 16V-248 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In December 2017, the Office of Defects Investigation (ODI) opened a Recall Query (RQ17-010) to assess the scope and remedy effectiveness of safety recall 16V-248. As a result of this ODI investigation, Ford issued safety recall 19V-075 to expand the scope of 16V-248 to include MY 2011-2013 Ford F-150 vehicles equipped with 6-speed (6R80) automatic transmissions and included approximately 1,263,051 vehicles. The remedy program described in safety recall 19V-075 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring. In June 2019, Ford issued safety recall 19V-433 as the remedy program used to service MY 2013 Ford F-150 5.0L and 6.2L vehicles in accordance with safety recall 19V-075 did not contain the necessary updates to prevent unintended vehicle transmission downshifting from occurring and included approximately 107,857 vehicles. On March 27, 2024, ODI opened this Recall Query (RQ24-005) to investigate the scope of the above-described safety recalls. At the time that this investigation was opened, ODI had received 86 Vehicle Owner Questionnaires (VOQs) reporting allegations of unintended, unexpected vehicle transmission downshifting to a lower gear without warning, often accompanied by temporary rear wheel lock-up, in MY 2014 Ford F-150 vehicles. As a result of this Recall Query, Ford filled safety recall 24V-444 on June 14, 2024, to include MY 2014 Ford F-150 vehicles and expand the scope of safety recalls 16V-248, 19V-075 and 19V-433. The remedy program described in safety recall 24V-444 involved reprogramming of the PCM to prevent unintended vehicle transmission downshifting from occurring and included approximately 552,188 vehicles. In its May 2024 response to ODI’s information request, Ford concluded that a predominant root cause for the intermittent OSS signal loss has not been identified outside of the original safety recall (16V-248). Ford’s investigation also concluded that the primary causes of the defect in MY 2014 F-150 vehicles were related to the molded lead frame and the control assembly transmission main. In the May 2024 response, Ford identified 2,184 total reports that described unintended vehicle transmission downshifting and rear wheel lock-up events; 1,441 warranty claims and 743 reports from consumer complaints, field reports, consumer surveys, and lawsuits/legal claims. Note that the values presented here and in the table above represent the total number of reports which can include a single consumer with multiple reports of the same event as well as a consumer with reports of multiple distinct events. Of the 1,441 warranty claims, 551 warranty claims involved repair of the molded lead frame (38%) and 509 warranty claims involved repair of the control assembly transmission main (35%). Ford has also stated that the PCM programming used to prevent unintended vehicle transmission downshifting was implemented starting with MY 2015 vehicle #1 production and onward. In view of the safety recall action being taken by Ford, ODI is closing this Recall Query (RQ24-005). The agency reserves the right to take additional action if warranted by future circumstances.
