The contact owns a 2013 Ford F-150. The contact stated that while driving 35 MPH and 55 MPH, the transmission inadvertently downshifted to first gear, causing the vehicle to abruptly slow down. The contact almost lost control of the vehicle. The vehicle was taken to the local dealer who was unable to duplicate the failure. There were no failure codes retrieved and no service was performed. The manufacturer was contacted but no assistance was offered. The contact indicated that the vehicle was previously serviced under the Manufacturer Extended Service Campaign Number: 19N01 and NHTSA Campaign Number: 16V248000 (Power Train) however, the failure occurred. The failure mileage was 102,000.

The contact owns a 2013 Ford F-150. The contact stated while driving 5 MPH from a complete stop and making a left turn in a parking lot, the steering wheel seized inadvertently, and the vehicle veered into a handicap sign pole. The messages "Service Power Steering", "Power Steering Assist Fault", and "Service Advancetrac" were displayed. The air bags had not deployed. The contact had not sustained any injuries. The Police patrol that was parked nearby the contact verified that he had not sustained any injuries. There was no police report filed. The vehicle was towed to an independent mechanic where it was diagnosed that the rack and pinion had seized while driving. Additionally, it was diagnosed that the electric steering motor had failed. The vehicle was not repaired. The dealer was notified of the failure and an appointment was scheduled for a diagnostic test. The vehicle was not repaired. The contact related the failure to NHTSA Campaign Number: 14V316000 (Steering) however, the VIN was not included. The manufacturer was notified of the failure but provided no assistance and referred the contact to the NHTSA hotline. The failure mileage was approximately 130,000. The contact stated that they are waiting for the repair for the dent in the fender caused by the accident.

The contact owns a 2013 Ford F-150. The contact stated that while her son was driving approximately 40 MPH in 5th gear, the vehicle downshifted to 1st gear. There were no warning lights illuminated. The driver cruised to the side of roadway due to the engine revving and the vehicle skidding. The driver turned off and restarted the vehicle; however, the vehicle failed to shift into gear immediately. The contact stated that after the vehicle shifted into gear the vehicle lunged forward however, the driver was able to keep driving the vehicle. The failure recurred twice. The vehicle had not been taken to the dealer. The vehicle had not been diagnosed or repaired. The manufacturer was not made aware of the failure. The approximate failure mileage was 168,000.

After a couple hours and a warmer temp., this issue resolved itself. Next morning battery was dead. replaced battery. Next cold morning. the vehicle cranked fine, but the steering was failing again. Called dealership, they bring in for a checkup, a week down the road, Bought this truck brand new and only have approx., 22,000 miles on it. Have saw many complaints with this problem on the computer. This is not isolated incidents for this paricular year model. No recalls for this year model has not been issued, I understand Ford did a recall on 2014 year model.. This is a very serious situation and needs rectification. Also a very expensive out of pocket for me. Please get ford to issue a recall on this mode, be fore I have a very serious accident. Thanxs David.

When attempting to coast to a stop at slow speeds the throttle stays up and even causes the truck to accelerate at times. The truck will shift hard randomly. The truck appears to get stuck in a high gear and loses power to accelerate, at times. Driving at highway speeds the truck loses power. A mechanic shop pulled codes p0720 and P0722. The mechanic shop referred me to the dealer because the codes were under an extended warranty for a recall 16s19-supplement #1. The dealer reported that they did not pull any codes off the truck except finding erratic misfires. The dealer reported that the truck engine light has to come on for them to read a code. They reported they can not order the lead frame part until they pull a code off the truck. They also report the part is on back order with no ETA. The trucks wrench light continues to come on but not engine light. An auto body shop was able to pull P0720 code off my truck without the engine light coming on. Ford has not returned my calls.

This is a duplicate report. Not sure prior report was transmitted. Average traffic and a clear day with a dry road (Interstate highway), while driving at the posted speed limit of 70mph with cruise-control engaged, the 6-speed automatic transmission suddenly downshifted to a lower gear (See NHTSA Campaign ID: 19V075000). The downshift caused the vehicle to slow with a rough and sudden jerk which could have resulted in loss of vehicle control. All dashboard instrumentation, except for the tachometer, stopped functioning. There were no 'warnings' previous to this failure. After shutting the engine down then restarting, the engine and transmission seem to operate normal. Have taken vehicle to local Ford dealer where the failure was confirmed, but the dealer has yet to perform any fix though the problem is covered by Ford Recall #19S07. The problem has not reappeared as of this date. No inspections have been conducted since the vehicle was observed at local Ford dealer on October 13th. Today's date is November 18th, week number six since the Ford dealer first had knowledge of the problem.

The contact owns a 2013 Ford F-150. The contact stated that while driving at an undisclosed speed, the transmission unexpectedly downshifted into first gear with the transmission warning light illuminated. Additionally, the transmission seized and failed to shift into the intended gear, and the speedometer was inoperable. The vehicle was taken to the dealer, and the powertrain control module was reprogrammed; however, the failure recurred. The dealer was made aware of the failure and determined that the transmission molded leadframe needed to be replaced; however, the replacement part was on backorder. The vehicle was not repaired. The manufacturer was made aware of the failure and informed the contact to take the vehicle in for diagnostic testing. The contact related the failure to NHTSA Campaign Number: 19V075000 (Power Train) however, the VIN was not included in the recall. The failure mileage was approximately 75,000. The contact stated that the vehicle was taken in twice to be looked at. First for a reprogramming and then for a diagnosis that resulted in the replacement of the frame.

The contact owns a 2013 Ford F-150. The contact stated that while driving at 10 MPH, the vehicle shifted from first to fifth gear unintendedly. There was no warning light illuminated. The contact veered to the side of the road. The contact stated that upon turning off and restarting the vehicle, the vehicle operated as needed. The contact stated that the failure had been reoccurring while driving. Additionally, the contact stated while driving at 55 MPH, the vehicle downshifted from fifth to first gear unintendedly. The contact stated that the vehicle then shifted from first to fifth gear unintendedly while driving later that evening. The contact stated that she turned off and restarted the vehicle; however, the transmission remained stuck in first gear. The vehicle was taken to the dealer where the transmission was recalibrated; however, the failure persisted. The vehicle was not repaired. The manufacturer was notified of the failure and a case was opened. The failure mileage was approximately 155,000.

The software used to reprogram the PCM did not have the necessary updates to prevent the transmission from unexpectedly downshifting into first gear, regardless of vehicle speed.

Consequence: Unexpectedly downshifting into first gear may result in a loss of vehicle control, increasing the risk of a crash.

16V345000 · Service Brakes, Hydraulic:fluid

Ford Motor Company (Ford) is recalling certain model year 2013-2014 F-150 vehicles manufactured August 1, 2013, to August 31, 2014 equipped with 3.5L engines. The master cylinder rear cup seal may roll, resulting in brake fluid leakage from the primary reservoir into the brake booster.

Consequence & remedy

Consequence: Loss of brake fluid from the primary reservoir results in a complete loss of front brake function, increasing the risk of a crash.

Remedy: Ford will notify owners, and dealers will replace the brake master cylinder and, if necessary, the brake booster, free of charge. The recall began on October 28, 2016. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 16S24.

The Office of Defects Investigation (ODI) opened an investigation based on consumer allegations of transmissions unexpectedly downshifting to a lower gear without input from the driver on model year (MY) 2011-2013 Ford F-150, Expedition and Navigator vehicles. ODI has received 455 unique reports related to the alleged problem that were not covered under a previous recall for this issue (16V-248).Expedition and Navigator complaint rates are lower than F-150 complaints. Of those 455 reports, 34.7% claimed that the rear wheels locked up when the transmission downshifted. There were 4 accident claims with no injuries. There are many instances where drivers report leaving their lane or the road because of this issue.In response to ODI's Information Request (IR) letter, Ford Motor Company (Ford) responded with 783 unique reports related to the alleged problem. There were 6 accident claims with 3 injuries. Ford also submitted 1206 warranty claims related to transmission downshifting failures. The failure can occur at any time, but typically at higher speed, and include symptoms such as sudden transmission downshifting, loss of speedometer, erratic shifting of transmission, and transmission stuck in one gear. The failures typically occur without warning and may result in the rear wheels momentarily locking up. The driver often had to fully stop the vehicle and restart it to get the vehicle to shift properly.On February 2nd, 2019 Ford notified ODI that it would conduct a safety recall (19V-075) to remedy transmission downshifting failures on all 2011-2013 F-150 vehicles. Ford will also offer extended warranties to 2011-2013 Expedition and Navigator vehicles with the same issue. Per Ford, some vehicles may experience an intermittent loss of Transmission Output Speed Sensor (OSS) signal to the Powertrain Control Module (PCM), potentially resulting in a temporary unintended downshift into first gear. On May 22, 2013, the Office of Defects Investigation (ODI) opened PE13-018 to investigate 95 complaints alleging incidents of reduced engine power during hard accelerations in model year (MY) 2011 through 2013 Ford F-150 trucks.ODI was aware of three technical service bulletins (TSBs) issued by Ford for a condition that was related to moisture accumulation in the Charge Air Cooler (CAC) for the turbochargers.A Preliminary Evaluation was opened to gather information to assess whether the subject vehicles contain a safety related defect.The subject vehicles are powered by Ford?s 3.5L EcoBoost engine which uses twin turbochargers to obtain a balance of fuel economy and engine power based on the driver?s demand.When the driver requests more power from the engine, as in accelerating to pass, merge, ascend hills or haul a load the two turbochargers will spin up compressing the air which is then used to increase the power created by the combustion in the engine.The compressed air, which gains heat by the compression process, is passed through a Charge Air Cooler (CAC) which is designed to lower the temperature of the air in order to make the combustion process even more efficient (Figures 1 and 2).In response to ODI?s Information Request letter, Ford indicated that a misfire condition could occur after steady-speed operation under significantly humid and rainy conditions.Ford?s analysis found that under those conditions, condensation could form and accumulate on the inside of the CAC tubes which could then be ingested into the engine during particularly hard acceleration near wide-open throttle.An engine misfire of up to three cylinders could occur if the amount of condensed water released from the CAC exceeded the engine?s operating threshold for water ingestion.Ford noted that due to regulations set by the EPA, the powertrain software would disable up to two of the misfiring cylinders for no more than 30 seconds and until the driver tips out the throttle in order to protect the catalytic converter from damage. Ford?s testing of this condition on the subject vehicles showed that a vehicle experiencing such a condition would continue to maintain its speed as well as accelerate, albeit at a lower rate than expected by the driver.Similar results were obtained in testing done by NHTSA?s Vehicle Research and Testing Center in East Liberty, Ohio (Figure 6).In an ongoing process to address the problem Ford issued a series of TSBs where a deflector shield was installed onto the CAC (Figures 3, 4 and 5).In January 2014 Ford indicated that its data shows that the TSBs have been effective in resolving the problem, that for the MY 2013 vehicles their remedy is 100% effective and the latest TSBs for MY 2011 ? 2012 are 95% effective.The 525 complaints to ODI and the total 4,120 unique VIN complaints listed in the Failure Report are incidents of alleged reduced power or driveability symptoms and not all are necessarily related to the condition created by water ingestion.Reduced power conditions can occur for a variety of reasons including faults with ignition coils, spark plugs, a catalytic converter, the throttle body, turbocharger, fuel pump/filter, powertrain control module (hardware or software) or transmission shifting problems.Ford also noted that in a large number of warranty claims on the subject vehicles, the CAC was replaced or a TSB procedure was performed when the symptoms exhibited were inconsistent with a CAC water ingestion related issue.Given these circumstances, further use of the agency resources in this matter does not appear to be warranted.Accordingly, this investigation is closed.The closing of
