WHILE BACKING OUT OF A PARKING SPOT, I APPLIED THE SERVICE BRAKES AND THE BRAKE PEDAL WENT TO THE FLOOR. THE TRUCK CONTINUED IN REVERSE, STRIKING A CONCRETE WALL. I SECURED THE VEHICLE AND INSPECTED THE DAMAGE. UNDER THE HOOD, I LOOKED AT THE BRAKE FLUID RESERVOIR. THERE WAS A VERY SMALL AMOUNT OF FLUID IN THE BOTTOM OF THE RESERVOIR. I LOOKED BACK AT THE DASHBOARD TO SEE IF THE BRAKE LIGHT WAS ILLUMINATED, IT WAS NOT. I HAD THE TRUCK TOWED TO THE DEALERSHIP WHERE I PURCHASED (NEW) IT TWO YEARS PRIOR, GOSCH FORD IN HEMET, CA. THE SERVICE TECHNICIAN SAID THE BRAKE MASTER CYLINDER AND BRAKE BOOSTER WERE BAD AND THAT THE FLUID HAD LEAKED BACK INTO THE BOOSTER. SINCE THE TRUCK WAS OUT OF WARRANTY, THE ESTIMATE FOR REPAIR WOULD BE $800. THE TRUCK HAS 36,443 MILES (443 MILES OVER THE BASIC WARRANTY FOR BRAKES). AS A SMALL BUSINESS OWNER, THE TRUCK IS USED TO TOW MY EQUIPMENT TRAILER NECESSARY FOR WORK. WITHOUT THE TRUCK, I DONT WORK AND MY BUSINESS SUFFERS. THE DEALERSHIP REFUSED TO SUBMIT THIS TO FORD FOR WARRANTY WORK STATING FORD IS VERY FIRM ON THIS ISSUE; EVEN ONE MILE OVER 36,000 WOULD BE REFUSED. I APPROVED THE REPAIRS SINCE I NEED THIS VEHICLE FOR MY LIVELIHOOD. AN HOUR LATER, THE SERVICE DEPARTMENT CALLED SAYING THERE IS A NATIONWIDE BACKORDER ON THE MASTER CYLINDER AND BRAKE BOOSTER (GO FIGURE), AND THE PARTS WOULDNT BE AVAILABLE FOR 5 WEEKS! SO, MY TRUCK (THE TRUCK I PURCHASED NEW TO ENSURE DEPENDABILITY, SAFETY AND RELIABILITY) SITS AT THE DEALERSHIP WAITING FOR THE REPLACEMENT PARTS FOR AN APPARENT COMMON BRAKE FAILURE PROBLEM. SINCE THEY ARE REPLACING THE DEFECTIVE PARTS WITH NEW (HOPEFULLY NOT DEFECTIVE) PARTS, I WILL CRINGE EACH TIME I STEP ON THE BRAKE PEDAL. AN INTERNET CHECK OF THIS PROBLEM REVEALED OTHERS WITH THE EXACT SAME SITUATION. WHY IS THERE NO RECALL? THIS IS A HUGE BRAKE FAILURE ISSUE.

MY FAMILY AND I WERE PREPARING TO PULL OUT OF A CRODED PARKING LOT AS WE WERE LEAVING A BASEBALL TOURNAMENT.MY HUSBAND ATTEMPTED TO PUT THE TRUCK INTO DRIVE AND AS HE SHIFTED, THE TRUCK LUNGED BACKWARD AS HE HAD NO BRAKES TO STOP THE VEHICLE. HE HAD TO THROW IT INTO P TO STOP.A MOTHER AND HER SON WERE WALKING NEAR THE VEHICLE AT THIS TIME, MY HUSBAND WAS QUITE UNNERVED.HE TURNED THE VEHICLE OFF AND WENT TO A LOCAL STORE FOR BRAKE FLUID IN ORDER TO DRIVE HOME.HE TOOK THE TRUCK TO THE NEAREST DEALER WHERE THEY HAD DISCOVERED THAT THE MASTER CYLINDER AND ENTIRE BRAKE SYSTEM FAILED.THERE WAS NO WARNING TO THIS ISSUE.OUR TRUCK ONLY HAS 44,000MILES AND IS 2 YEARS OLD.THIS IS OUR FAMILY VEHICLE USED TO TRANSPORT OUR CHILDREN, NOT TO HAUL OR TOW THINGS.I RESEARCHED THIS ISSUE ONLINE AND CONTACTED FORD AFTER REALIZING THAT OUR TRUCK WAS NOT THE ONLY TRUCK WITH THIS ISSUE. I SPOKE WITH REGIONAL MANAGER RANDY COPELAND REGARDING THE ISSUE AND EXPRESSED MY CONCERNS THAT THIS IS A HUGE SAFETY ISSUE.AN ENTIRE BRAKE SYSTEM SHOULD NEVER FAIL UNLESS IT WAS FAULTY, ESPECIALLY ON A NEWER VEHICLE.I EXPLAINED TO HIM THAT THIS IS A SERIOUS ISSUE AND COULD RESULT IN LOSS OF LIVES IF IT IS NOT ADDRESSED.I WAS TOLD TO WORRY ABOUT MY OWN VEHICLE.I WAS TOLD THAT I NEED TO KEEP CONVERSATION PROFESSIONAL, I WAS UPSET AT THE FACT THAT HE DISMISSED THE SERIOUSNESS OF NO BRAKES.MY CONCERNS FELL ON DEAF EARS.I WANT FORD TO REALIZE THE SERIOUSNESS OF THIS SAFETY ISSUE.MR. COPELAND ALSO COULD NOT ANSWER MY QUESTION- WHAT WOULD FORD HAVE DONE IF MY HUSBAND WOULD HAVE BACKED OVER THIS MOTHER AND HER SON.I WANT FORD TO RECOGNIZE THIS PROBLEM AND TAKE ACTION BEFORE SOMEONE IS SERIOUSLY INJURED OR LIVES ARE LOST.

WHILE DRIVING ON FREEWAY A "LOW BRAKE FLUID" WARNING LIGHT CAME ON THE DASH. I TESTED BRAKES AND THEY FELT OK AT THAT POINT. AFTER DRIVING SEVERAL MORE MILES THE BRAKE PEDAL TOOK MORE PRESSURE TO SLOW DOWN THE VEHICLE AND BY THE TIME I STOPPED THE BRAKE PEDAL WENT TO THE FLOOR. I CHECKED THE BRAKE FLUID RESERVOIR AND IT WAS EMPTY. I FOUND NO BRAKE FLUID LEAKING UNDER THE VEHICLE. I FILLED THE RESERVOIR WITH NEW BRAKE FLUID AND AFTER DRIVING 5 MILES THE BRAKE WARNING LIGHT CAME ON AGAIN. I STOPPED AND CHECKED THE RESERVOIR AND IT WAS EMPTY AGAIN. AFTER FILLING THE RESERVOIR AGAIN I TOOK THE VEHICLE INTO A FORD DEALER. THEY TOLD ME THE MASTER CYLINDER HAD FAILED AND LEAKED THE BRAKE FLUID INTO THE BRAKE BOOSTER UNIT AND CAUSED IT TO FAIL AS WELL. THE SERVICE ADVISER SAID THEY HAD 3 OTHER F-150 TRUCKS COME IN WITH THIS SAME ISSUE. THE FIX IS TO REPLACE THE MASTER CYLINDER AND THE BRAKE BOOSTER UNIT WITH NEW PARTS. HE CLAIMS THE NEW PARTS HAVE BEEN RE-ENGINEERED TO FIX THE ISSUE. I CHECKED THE INTERNET FOR THIS PROBLEM AND FOUND MANY OTHERS HAVE HAD THE SAME ISSUE WITH AT LEAST 2011, 2012, 2013 FORD F-150 TRUCKS, SOME NEVER RECEIVED THE "LOW BRAKE FLUID" LIGHT BEFORE THE BRAKES FAILED. THIS IS AN EXTREMELY DANGEROUS SAFETY ISSUE AS THE BRAKES BECOME UNUSABLE WHEN THIS OCCURS. I TOW A LARGE TRAILER ON SOME STEEP GRADES AND THIS COULD BE A DISASTROUS ISSUE AS YOU CAN IMAGINE. I BELIEVE THIS SHOULD DEFINITELY TRIGGER AN IMMEDIATE RECALL TO FIX THIS VERY DANGEROUS ISSUE.

I REPLACED MY BREAKS (ALL OF THEM FRONT AND REAR) IN AUGUST OF 2014. NOT EVEN ONE YEAR OF OWNING THE VEHICLE AND ONLY ABOUT 13,000 MILES. THEY WOULD SHUDDER AND SHAKE WHEN I APPLIED THE BRAKES. FORD TOLD ME IT WAS NOT COVERED UNDER WARRANTY AND IT AS "NORMAL WEAR AND TEAR." FAST FORWARD, JULY OF 2015. THE BREAKS ARE DOING THE SAME THING AGAIN. SHUDDERING AND SHAKING. I AGAIN TAKE IT INTO FORD, AND THEY ARE TELLING ME THE ROTORS ARE CRACKED AND I AM GOING TO HAVE TO REPLACE THEM. WITHIN A YEAR AND NOW AT ONLY 27,000 MILES. THEY ASKED ME IF I TOW A LOT OR HEAVY ITEMS. I TOLD THEM I HAVE NEVER TOWED ANYTHING AND ONLY DRIVE THE VEHICLE 3 DAYS PER WEEK. FORD HAD NO EXPLANATION OF HOW IT COULD HAVE HAPPENED; HOWEVER, AGAIN IT IS NOT COVERED UNDER WARRANTY. BREAKS/ROTERS ARE NOT COVERED! AT 13,000 AND AT 27,000 THERE IS NO REASON ANY BREAKS OR ROTERS NEED TO BE REPLACED. I OWNED A 2002 FORD F-150 AND DID NOT NEED BREAKS UNTIL 70,000 MILES. THERE IS A PROBLEM!

VEHICLE WAS SLOWLY MOVING BACKWARDS AND WOULD NOT STOP. TRIED PUMPING THE PEDDLE TO STOP THE TRUCK. USED PARKING BRAKE TO STOP VEHICLE TO AVOID BACKING INTO OTHER CARS BEHIND. CHECKED BRAKE FLUID LEVEL AND FOUND IT WAS ALMOST OUT, LESS THAN A 1/4 INCH IN THE RESERVOIR. CHECKED ENTIRE VEHICLE AND NO LEAKS COULD BE FOUND CALLED CROSSROADS FORD IN CARY, NC AND THEY INDICATED I SHOULD FILL IT WITH DOT3 FLUID AND BRING IT IN. FILLED RESERVOIR TO MAX LINE AND PUMPED BRAKED A FEW TIMES UNTIL THEY WERE FIRM. DROVE TO THE DEALERSHIP, ABOUT 25 MILES. NO ISSUES GETTING TO THE DEALERSHIP OTHER THAN THE BRAKES WERE QUITE SPONGY AND NOT TOO RESPONSIVE. UPON INSPECTION AT THE DEALERSHIP, THEY REMOVED THE MASTER CYLINDER AND FOUND THE FLUID HAD FILLED THE BRAKE BOOSTER AND THE MASTER CYLINDER HAD FAILED.. THIS SEEMS TO BE A VERY SERIOUS PROBLEM, I'M VERY LUCKY I WASN'T KILLED OR KILLED SOMEONE IN AN ACCIDENT. VEHICLE WAS JUST A LITTLE OVER 500 OUT OF WARRANTY.

The Office of Defects Investigation (ODI) opened an investigation based on consumer allegations of transmissions unexpectedly downshifting to a lower gear without input from the driver on model year (MY) 2011-2013 Ford F-150, Expedition and Navigator vehicles. ODI has received 455 unique reports related to the alleged problem that were not covered under a previous recall for this issue (16V-248).Expedition and Navigator complaint rates are lower than F-150 complaints. Of those 455 reports, 34.7% claimed that the rear wheels locked up when the transmission downshifted. There were 4 accident claims with no injuries. There are many instances where drivers report leaving their lane or the road because of this issue. In response to ODI's Information Request (IR) letter, Ford Motor Company (Ford) responded with 783 unique reports related to the alleged problem. There were 6 accident claims with 3 injuries. Ford also submitted 1206 warranty claims related to transmission downshifting failures. The failure can occur at any time, but typically at higher speed, and include symptoms such as sudden transmission downshifting, loss of speedometer, erratic shifting of transmission, and transmission stuck in one gear. The failures typically occur without warning and may result in the rear wheels momentarily locking up. The driver often had to fully stop the vehicle and restart it to get the vehicle to shift properly. On February 2nd, 2019 Ford notified ODI that it would conduct a safety recall (19V-075) to remedy transmission downshifting failures on all 2011-2013 F-150 vehicles. Ford will also offer extended warranties to 2011-2013 Expedition and Navigator vehicles with the same issue. Per Ford, some vehicles may experience an intermittent loss of Transmission Output Speed Sensor (OSS) signal to the Powertrain Control Module (PCM), potentially resulting in a temporary unintended downshift into first gear. Ford will reprogram the PCM with software that will prevent an unintended downshift into first gear and illuminate the Malfunction Indicator Light in the event of an intermittent OSS signal loss. The recall action initiated by the manufacturer appears to address the safety risks of concern to ODI.Accordingly, the investigation is closed. The ODI reports cited above can be reviewed at www-odi.nhtsa.dot.gov/owners/SearchNHTSAID under the identification (ODI) numbers found in the attached list.

PE13018 · Reduced Power During Hard Acceleration

Opened May 22, 2013 · Closed Apr 7, 2014

Status: closed (inferred from source dates) · Engine; Engine And Engine Cooling

On May 22, 2013, the Office of Defects Investigation (ODI) opened PE13-018 to investigate 95 complaints alleging incidents of reduced engine power during hard accelerations in model year (MY) 2011 through 2013 Ford F-150 trucks.ODI was aware of three technical service bulletins (TSBs) issued by Ford for a condition that was related to moisture accumulation in the Charge Air Cooler (CAC) for the turbochargers.A Preliminary Evaluation was opened to gather information to assess whether the subject vehicles contain a safety related defect. The subject vehicles are powered by Ford?s 3.5L EcoBoost engine which uses twin turbochargers to obtain a balance of fuel economy and engine power based on the driver?s demand. When the driver requests more power from the engine, as in accelerating to pass, merge, ascend hills or haul a load the two turbochargers will spin up compressing the air which is then used to increase the power created by the combustion in the engine. The compressed air, which gains heat by the compression process, is passed through a Charge Air Cooler (CAC) which is designed to lower the temperature of the air in order to make the combustion process even more efficient (Figures 1 and 2).In response to ODI?s Information Request letter, Ford indicated that a misfire condition could occur after steady-speed operation under significantly humid and rainy conditions.Ford?s analysis found that under those conditions, condensation could form and accumulate on the inside of the CAC tubes which could then be ingested into the engine during particularly hard acceleration near wide-open throttle. An engine misfire of up to three cylinders could occur if the amount of condensed water released from the CAC exceeded the engine?s operating threshold for water ingestion.Ford noted that due to regulations set by the EPA, the powertrain software would disable up to two of the misfiring cylinders for no more than 30 seconds and until the driver tips out the throttle in order to protect the catalytic converter from damage. Ford?s testing of this condition on the subject vehicles showed that a vehicle experiencing such a condition would continue to maintain its speed as well as accelerate, albeit at a lower rate than expected by the driver.Similar results were obtained in testing done by NHTSA?s Vehicle Research and Testing Center in East Liberty, Ohio (Figure 6).In an ongoing process to address the problem Ford issued a series of TSBs where a deflector shield was installed onto the CAC (Figures 3, 4 and 5).In January 2014 Ford indicated that its data shows that the TSBs have been effective in resolving the problem, that for the MY 2013 vehicles their remedy is 100% effective and the latest TSBs for MY 2011 ? 2012 are 95% effective. The 525 complaints to ODI and the total 4,120 unique VIN complaints listed in the Failure Report are incidents of alleged reduced power or driveability symptoms and not all are necessarily related to the condition created by water ingestion.Reduced power conditions can occur for a variety of reasons including faults with ignition coils, spark plugs, a catalytic converter, the throttle body, turbocharger, fuel pump/filter, powertrain control module (hardware or software) or transmission shifting problems.Ford also noted that in a large number of warranty claims on the subject vehicles, the CAC was replaced or a TSB procedure was performed when the symptoms exhibited were inconsistent with a CAC water ingestion related issue.Given these circumstances, further use of the agency resources in this matter does not appear to be warranted.Accordingly, this investigation is closed. The closing of
