The contact owns a 2017 Ford Explorer. The contact stated while driving approximately 55 MPH, the front driver’s side the keypad on the door detached from the vehicle and was hanging on by a wire. The contact stated that the liftgate ajar warning light was illuminated however, she was unable to open the liftgate. The contact took the vehicle to the local dealer and the dealer was made aware of the failure. The contact also requested that the vehicle be serviced and the recalls associated with the VIN be repaired. The vehicle was not diagnosed or repaired. The manufacturer had been informed of the failure. The failure mileage was approximately 112,000.

The contact owned a 2021 Ford Explorer which was used to haul a 2002 Opus OP. The contact stated that while the vehicle was at the RV dealer for installations to be performed the vehicle caught on fire and was destroyed. The contact was informed after the installation of an electronic brake controller was completed the vehicle was parked and approximately 20 minutes later flames were present coming from the engine compartment. The fire department was called to the scene and extinguished the flames. The vehicle was destroyed. Prior to the incident an electric brake controller was installed in the vehicle and wired DC to DC in order to send a charge to the tow vehicle. The manufacturer was made aware of the failure and a case was opened. The fire department indicated that the fire originated in the engine compartment. A fire report was taken at the scene. The failure mileage was 3,807.

My rack and pinion steering column and power steering pump went out causing a hefty cost to replace and it is starting to fail again creating hazardous conditions while driving. I do believe there is a steering issue in these models. Also my power lift gate in the back closes without stopping automatically while I’m loading the rear or unloading. There is NO STOPPING SAFETY SENSOR, I HAD THE BACK CLOSE ON ME MULTIPLE TIMES AND ALMOST CRUSH ME. there is no safety sensor to stop the power lift gate from closing on you, I have had to crawl in my rear end to avoid being crushed by my liftgate! This is not safe!

The manufacturer was informed of the failure and informed the contact that the VIN was not included in a recall. Upon investigation, the contact associated the failure with NHTSA Campaign Number: 14V286000 (Steering) however, the VIN was not included. The failure mileage was approximately 108,000.

1. The vehicle HORN unknowingly and unexpectedly stopped working, became unavailable, and subsequently failed to alert other of hazardous conditions, when approperiately initiated by the driver/operator. Without any indication or prior warning to the driver that the HORN was NOT FUNCTIONAL. This vehicle can be made available for NHTSA inspection upon request. For all intensive purposes, the only unique purpose for having a vehicle HORN is for SAFETY. 2. SAFETY was compromised and reslted in highly eleveted risks for the Driver, Passangers, Pedestrians, Traffic Officers, By Standers, Other Vehicles and Congestion, following a Night Time Egress of an NFL Football Game. 3. The problem is repeatable and the HORN is currently in a FAILED state. The HORN has not been diagnosed or repaired. It has not been confirmed, by the Dealer, nor Independent Service Center. WHY? For them to bill me some ungodly amount, just to tell me, my the HORN DOES NOT WORK.. ON a vehicle I financed/purchased 4 months ago? and that I paid $200 last month for a FAILED BATTERY last month. Good LORD, and on a vehicle with more automated driver assist sensors, than a MARTIAN LAND EXPLORER? and the HORN FAILS? For SAFETY, HORNS are intentionally "enabled" and maintained in fuctional state. Note: AUTO OWNER has reported this SAFETY DEFECT to NHTSA. Owner requests all costs of diagnosis, parts, replacement, labor, repairs, wiring, computer, software, and the transparent identification of flaws in automatic updates via internet and wireless, connectivity, software updates, digital processing of vehicle operating systems, and digital monitoring of driving conditions, location, and environment. 4. No. It has not been inspected by manufacturer, police, insurance reps, or others. Yet. 5. No warning lights, messages, or indicators prior to non-functioning and unexpected HORN failure. The two problems I am having seem to have been programmed into the onboard computer to fail after a certain time or mileage. I will address each problem individually starting with the most hazardous one first. The onboard navigation system utilizes a SD card program that becomes outdated as time passes. I have received numerous mailing from Ford notifying me that the original navigation system is outdated and that I should update to a current one at a cost to me of $149.00. The original system works fine for me and I felt no need to upgrade until the system began getting interrupted by prompts to update (see FIGURE 1 below). When the prompt appears the navigation system is no longer operational. The most recent of these interruptions became a serious and hazardous situation. I was driving on the interstate nearing an interchange to switch to a different highway. I was relying on the navigation system to direct me to the correct exit and this is when the prompt appeared. The navigation system was now useless and will only resume normal operation if the vehicle is shut off and restarted. You don’t do this at 70+ mph! My only option was to use my phone which meant I had to program it and while driving this is dangerous. I feel Ford was being irresponsible by programming the prompts into the onboard computer that disable the navigation system in return for the sale of an updated SD card. Our radio/navigational/climate control screen will go completely black while driving the vehicle. This has happened approximately 15 times since the purchase of the brand new car in August of 2020. Therefore, I lose all of my controls and my GPS. This will not come back on until the car is turned off and is given time to reset. This reset time varies from 30 minutes to 1 hour. The first time this occurred was one month after purchasing the vehicle. So, after three different visits to Ford dealerships, Hopkins Ford, Manning Ford and Dundee Ford, not one technician can duplicate this problem so it is still happening. Recently, I was in Columbus, Ohio and the screen went black while I was driving in an unfamiliar area. I'm aware that Ford had a massive recall on this very issue but somehow our VIN number is not included in this. I frequently travel across the Midwest because my fifteen year old daughter is in travel volleyball. I cannot afford for safety reasons for this to happen to the two of us when we are driving in unfamiliar places. This is a huge safety concern for my daughter and I.

The battery continues to drain even when parked next to my house. After a few days some systems shut down beginning with the passenger door handle sensor not unlocking the doors when touched. A few days later the car would not start. It has been to several dealers in the nearly three years I have owned it and had numerous batteries replaced. Dealer always states everything checks out fine I have been stranded with a no start several times. The 12 volt accessories socket also stops working.

Ford Motor Company (Ford) is recalling certain 2020-2022 Lincoln Aviator and Ford Explorer vehicles equipped with 3.0L PHEV engines. A manufacturing defect in one or more of the high voltage battery cells may result in an internal short circuit and battery failure.

Consequence & remedy

Consequence: Battery failure can cause a loss of drive power, increasing the risk of a crash. Battery failure can also increase the risk of a fire.

Remedy: Dealers will perform a Battery Energy Control Module (BECM) diagnostic test and replace the high voltage battery pack, as necessary, free of charge. Ford Motor Company (Ford) is recalling certain 2020 Ford Explorer vehicles equipped with 2.3L or 3.3L engines. The wiring harness may not have been properly secured and may contact the air conditioner compressor (A/C) pulley, potentially damaging the harness or the A/C belt.

Consequence & remedy

Consequence: The A/C pulley may rub through the wiring harness insulation and contact the unfused battery positive wire, resulting in a short circuit, increasing the risk of a fire.

Remedy: Ford will notify owners, and dealers will install a tie strap to the wiring harness at the hand shock tower clip, free of charge. The recall began October 21, 2019. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 19S32.

In addition, the vehicles may be in Factory Mode, which disables warning alerts and does not display the transmission gear selected. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 114, "Theft Prevention" and 102, "Trans Shift Lever Seq/Starter Interlock/Trans Braking Effect."

Consequence & remedy

Consequence: Without the cover, the MPR lever may be unintentionally moved, allowing the vehicle to roll away. Being in Factory Mode, the vehicle will not display warnings and/or gear selection on the instrument cluster. Both conditions increase the risk of a crash.

Remedy: Ford will notify owners, and dealers will inspect and, if necessary, install a MPR cover. Dealers will verify the instrument cluster is not in Factory Mode and clear any fault codes, free of charge. The recall began August 23, 2019. Power Train:automatic Transmission:gear Position Indication (prndl)

Ford Motor Company (Ford) is recalling certain 2020 Explorer and Lincoln Aviator vehicles. The Manual Park Release (MPR) cover may not be installed. In addition, the vehicles may be in Factory Mode, which disables warning alerts and does not display the transmission gear selected. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 114, "Theft Prevention" and 102, "Trans Shift Lever Seq/Starter Interlock/Trans Braking Effect."

Consequence: Without the cover, the MPR lever may be unintentionally moved, allowing the vehicle to roll away. Being in Factory Mode, the vehicle will not display warnings and/or gear selection on the instrument cluster. Both conditions increase the risk of a crash.

Remedy: Ford will notify owners, and dealers will inspect and, if necessary, install a MPR cover. Dealers will verify the instrument cluster is not in Factory Mode and clear any fault codes, free of charge. The recall began August 23, 2019. THE GENERIC ELECTRONIC MODULE (GEM) COULD EXPERIENCE A CONDITION REFERRED TO AS "LOCK-UP" IN WHICH THE GEM CONTROLLED ELECTRONIC FUNCTIONS (E.G., FRONT WINDSHIELD WIPERS, INTERIOR LIGHTS, 4X4 SYSTEM, ETC.) COULD NOT BE TURNED ON OR, IN SOME CASES IF THE FUNCTION IS ON, COULD NOT BE TURNED OFF.

Consequence & remedy

Consequence: AN INOPERATIVE WINDSHIELD WIPER SYSTEM COULD AFFECT DRIVER VISIBILITY, INCREASING THE RISK OF A CRASH.

Remedy: DEALERS WILL INSTALL A RESISTOR IN THE GEM CIRCUIT.

Consequence & remedy

Consequence: THE STANDARD SPECIFIED THAT A WARNING TO THE DRIVER SHALL BE ACTUATED WHENEVER THE KEY HAS BEEN LEFT IN THE LOCKING SYSTEM AND THE DRIVER'S DOOR IS OPENED.

Remedy: DEALERS WILL INSTALL A JUMPER WIRE TO PROVIDE THE NECESSARY GROUND CIRCUIT IN THESE VEHICLES.

THIS INVESTIGATION REVEALED THAT CONSOLE FIRES IN THE SUBJECT VEHICLES MOST OFTEN OCCURRED WHILE VEHICLES EQUIPPED WITH THE LAMP OUTAGE MODULE (LOM), WHICH IS PART OF THE MESSAGE CENTER, WERE TOWING A TRAILER AND WHICH HAD BEEN MODIFIED BY ADDING AN AFTERMARKET TRAILER WIRING HARNESS INTO THE VEHICLE'S TAILLIGHT CIRCUITS.THE OWNER'S MANUAL STATES NEVER AND MORE THAN OTHER TRAILER LIGHT BULB TO ONE CIRCUIT HOWEVER TRAILERS FREQUENTLY CONTAIN SEVERAL BULBS.THE RESULTING EXCESS CURRENT PASSING THROUGH THE LOM COULD CAUSE IT TO OVERHEAT RESULTING IN A FIRE.PROPER AFTERMARKET INSTALLATION OF TRAILER WIRING REQUIRES THE USE OF A BYPASS MODULE, WHICH OBTAINS POWER FOR THE TRAILER LIGHTS DIRECTLY FROM THE BATTERY RATHER THAN FROM THE VEHICLE'S TAIL LIGHT AND STOP LAMP CIRCUITS, OR THE USE OF THE MANUFACTURER'S WIRING HARNESS.IN A LETTER DATED SEPTEMBER 2, 1997, FORD NOTIFIED ODI THAT IT INTENDS TO TAKE ACTION TO PRECLUDE FURTHER INCIDENTS INCLUDING ISSUING A TECHNICAL SERVICE BULLETIN TO ITS DEALERS, NOTIFYING TRAILER RENTAL ASSOCIATIONS AND AGENCIES WHERE KNOWN, AND ADVISING OWNERS BY LETTER OF THE PROPER METHOD FOR CONNECTION OF TRAILER TOW WIRING ON THE SUBJECT VEHICLES.THERE WAS NO DEFECT TREND IN THE SUBJECT VEHICLES IDENTIFIED AS A RESULT OF THIS INVESTIGATION, AND FURTHER EXPENDITURE OF AGENCY RESOURCES IS NOT WARRANTED TO CONTINUE THE INVESTIGATION.

THE CONTACT ASSEMBLY FOR THE REMOTE POWER MIRROR CONTROL SWITCH MAY DEVELOP A SHORT CIRCUIT BETWEEN THE SWITCH POSITIVE PAD AND CHASSIS GROUND PAD AS A RESULT OF WATER INTRUSION.EXCESS CURRENT FLOW RESULTING FROM THE SHORT CIRCUIT CONDITION MAY OVERHEAT THE REMOTE POWER MIRROR CONTROL SWITCH CIRCUIT BOARD AND CAUSE A FIRE IN THE DRIVER'S DOOR.FORD HAS INDICATED THAT CURRENT IN EXCESS OF 15 AMPS CAN OVERHEAT THE CIRCUIT BOARD ENOUGH TO CAUSE A FIRE.CIRCUIT PROTECTION IS PROVIDED BY A 30 AMP FUSE.FORD IS RECALLING ALL 1993-94 4-DOOR EXPLORER VEHICLES EQUIPPED WITH THE SUBJECT ELECTRONIC OPTION COMBINATION TO INSTALL AN IN-LINE 10 AMP FUSE IN THE POSITIVE BATTERY FEED TO THE REMOTE POWER MIRROR CONTROL SWITCH ASSEMBLY.NO 2-DOOR EXPLORER VEHICLES WERE BUILT WITH THE SUBJECT COMPONENTS.ACCORDING TO FORD, VEHICLES BUILT PRIOR TO THE RECALL PRODUCTION RANGE WERE EQUIPPED WITH LOWER CAPACITY CIRCUIT BOARDS FOR THE SUBJECT SWITCH ASSEMBLY THAT WOULD FAIL FROM AN OPEN CIRCUIT IF A SIMILAR SHORT OCCURRED.A NEW SWITCH ASSEMBLY WITH 10 AMP FUSE PROTECTION WAS INTRODUCED BEGINNING WITH THE 1995 MODEL YEAR EXPLORER.MAZDA WILL CONDUCT A PARALLEL COMPAIGN (97V-083.002) TO REMEDY THE SUBJECT CONDITION IN APPROXIMATELY 14,000 NAVAJO VEHICLES.THE NAVAJO VEHICLES WERE AVAILABLE IN ONLY THE 2-DOOR VERSION WHICH, UNLIKE THE EXPLORER VEHICLES, DID USE THE SUBJECT OPTION COMBINATION IN THAT BODY STYLE.
