I have had the previous recall for my rear camera/display screen repaired in 08/2022. However, recently I have had my display screen start to glitch and turn off intermittently. Sometimes this happens while I am driving. Other times is happens while I am in reverse and the reverse camera does not work. When the reverse camera is not functioning there is concern for unseen dangers. I have videos of this but I cannot upload them into the form.

Numerous times over the recent 2 months, I have experienced different issues with my 2021 Ford Explorer. The vehicle was purchased used in 2022 with less than 10,000 miles on it. It currently has 36k miles. When the vehicle is stopped but in drive and then accelerated to merge into traffic, the vehicle stalls and will not engage. The accelerator has to be pressed repeatedly to get the vehicle to begin moving again. This can take anywhere from 2-3 presses and 10-20 seconds. This creates a hazard for moving traffic and a safety issue. This occurs repeatedly with no warning beforehand. The vehicle has no lights to reflect an issue. Additionally, while the vehicle has been parked and shut-off, the electronics will turn on. On two separate occasions, the lights have turned on on their own and the radio has done the same; there was nobody in the car either. Over the past weekend, while driving down the road, no rain or water on windshield, and the wipes started. The vehicle persistently has a blue screen during backup. There is a concern for the electronics as a whole with all of the issues occurring. When accelerating on the highway/interstate, often times, an odd smell can be sensed, as if the EcoBoost is not functioning appropriately. The recall for rear axle bolt was repaired by Ford in Yukon, OK, and was informed that is the only recall currently with a repair available. The vehicle is available for inspection, as necessary.

I was backing up at low speed (less than 1 mph) to park by gently letting my foot off the brake to back. I was using the rear backup camera to gauge how far I could go back without striking the vehicle behind me. The screen showed video that I was still several feet away from the vehicle behind me when I felt a sudden bump. I stepped on the brake hard to ensure I was no longer moving but the camera still showed several feet between my rear bumper and the vehicle behind me. I put the vehicle in park and got out to verify that I had struck the vehicle behind me. I had. I returned to the driver’s seat, put the vehicle back in reverse to see what the camera showed and now it appeared on the screen that my bumper was touching the vehicle behind me. The video on the screen had absolutely froze while I was backing, which contributed to me making an error in judgement while backing, causing me to strike the vehicle behind me. The DuPage County Sheriff’s Office (Illinois) completed a crash report. SOPC23-08519

When the temperature outside is higher than an estimated 80 degrees the Battery indicator appears on the dash and the Ford Pass app tells me that there is a Charge System Fault. "The Battery light indicates that your vehicle has a concern with the 12-volt charging system." I have had a Ford Dealer check into this when I was under lease and they said they were unable to duplicate the warning. I also had an independent service center look into this and we decided to replace the battery and the battery terminals. However the alert still appears in the higher temperatures after making the change. I have seen this alert more than a dozen times having leased the vehicle from Ford from April 2019-April 2022 then purchasing the lease in April 2022. The date below indicates the last date the alert occur (5/12/23)

THIS INVESTIGATION REVEALED THAT CONSOLE FIRES IN THE SUBJECT VEHICLES MOST OFTEN OCCURRED WHILE VEHICLES EQUIPPED WITH THE LAMP OUTAGE MODULE (LOM), WHICH IS PART OF THE MESSAGE CENTER, WERE TOWING A TRAILER AND WHICH HAD BEEN MODIFIED BY ADDING AN AFTERMARKET TRAILER WIRING HARNESS INTO THE VEHICLE'S TAILLIGHT CIRCUITS.THE OWNER'S MANUAL STATES NEVER AND MORE THAN OTHER TRAILER LIGHT BULB TO ONE CIRCUIT HOWEVER TRAILERS FREQUENTLY CONTAIN SEVERAL BULBS.THE RESULTING EXCESS CURRENT PASSING THROUGH THE LOM COULD CAUSE IT TO OVERHEAT RESULTING IN A FIRE.PROPER AFTERMARKET INSTALLATION OF TRAILER WIRING REQUIRES THE USE OF A BYPASS MODULE, WHICH OBTAINS POWER FOR THE TRAILER LIGHTS DIRECTLY FROM THE BATTERY RATHER THAN FROM THE VEHICLE'S TAIL LIGHT AND STOP LAMP CIRCUITS, OR THE USE OF THE MANUFACTURER'S WIRING HARNESS.IN A LETTER DATED SEPTEMBER 2, 1997, FORD NOTIFIED ODI THAT IT INTENDS TO TAKE ACTION TO PRECLUDE FURTHER INCIDENTS INCLUDING ISSUING A TECHNICAL SERVICE BULLETIN TO ITS DEALERS, NOTIFYING TRAILER RENTAL ASSOCIATIONS AND AGENCIES WHERE KNOWN, AND ADVISING OWNERS BY LETTER OF THE PROPER METHOD FOR CONNECTION OF TRAILER TOW WIRING ON THE SUBJECT VEHICLES.THERE WAS NO DEFECT TREND IN THE SUBJECT VEHICLES IDENTIFIED AS A RESULT OF THIS INVESTIGATION, AND FURTHER EXPENDITURE OF AGENCY RESOURCES IS NOT WARRANTED TO CONTINUE THE INVESTIGATION.

THE CONTACT ASSEMBLY FOR THE REMOTE POWER MIRROR CONTROL SWITCH MAY DEVELOP A SHORT CIRCUIT BETWEEN THE SWITCH POSITIVE PAD AND CHASSIS GROUND PAD AS A RESULT OF WATER INTRUSION.EXCESS CURRENT FLOW RESULTING FROM THE SHORT CIRCUIT CONDITION MAY OVERHEAT THE REMOTE POWER MIRROR CONTROL SWITCH CIRCUIT BOARD AND CAUSE A FIRE IN THE DRIVER'S DOOR.FORD HAS INDICATED THAT CURRENT IN EXCESS OF 15 AMPS CAN OVERHEAT THE CIRCUIT BOARD ENOUGH TO CAUSE A FIRE.CIRCUIT PROTECTION IS PROVIDED BY A 30 AMP FUSE.FORD IS RECALLING ALL 1993-94 4-DOOR EXPLORER VEHICLES EQUIPPED WITH THE SUBJECT ELECTRONIC OPTION COMBINATION TO INSTALL AN IN-LINE 10 AMP FUSE IN THE POSITIVE BATTERY FEED TO THE REMOTE POWER MIRROR CONTROL SWITCH ASSEMBLY.NO 2-DOOR EXPLORER VEHICLES WERE BUILT WITH THE SUBJECT COMPONENTS.ACCORDING TO FORD, VEHICLES BUILT PRIOR TO THE RECALL PRODUCTION RANGE WERE EQUIPPED WITH LOWER CAPACITY CIRCUIT BOARDS FOR THE SUBJECT SWITCH ASSEMBLY THAT WOULD FAIL FROM AN OPEN CIRCUIT IF A SIMILAR SHORT OCCURRED.A NEW SWITCH ASSEMBLY WITH 10 AMP FUSE PROTECTION WAS INTRODUCED BEGINNING WITH THE 1995 MODEL YEAR EXPLORER.MAZDA WILL CONDUCT A PARALLEL COMPAIGN (97V-083.002) TO REMEDY THE SUBJECT CONDITION IN APPROXIMATELY 14,000 NAVAJO VEHICLES.THE NAVAJO VEHICLES WERE AVAILABLE IN ONLY THE 2-DOOR VERSION WHICH, UNLIKE THE EXPLORER VEHICLES, DID USE THE SUBJECT OPTION COMBINATION IN THAT BODY STYLE.
