“I rented vehicle VIN [XXX] from Budget Rental Car in Panama City Beach, FL (Reservation #XXX, Rental Agreement XXX). I later discovered the vehicle had an unrepaired federal safety recall (NHTSA Recall 25V543 / Manufacturer Recall 25C42) involving the Body Control Module. Budget never informed me of the recall at any time. On November 11, 2025, when I attempted to extend my rental, the representative told me the vehicle had been ‘sold’ and must be returned within two days. When I attempted to exchange it on November 12–13 at three different Budget locations in Lansing, MI, they refused to take the vehicle, stating it had a safety recall and should not have been rented out. Only after this did I check the VIN myself and learn of the safety recall. The recall affects multiple electrical and visibility-related components: trailer lights, interior lights, mirrors, windows, rear defroster, USB/12V ports, and HVAC systems. These issues can reduce driver visibility and increase the risk of a crash. While driving the vehicle, I experienced visibility problems. The windows fogged up severely, and the defroster did not operate properly. As a result, I was unable to see an animal that entered the roadway and unfortunately struck it. There is now damage to the front driver’s side of the vehicle. This visibility impairment aligns with the Body Control Module recall defects described by NHTSA. This vehicle should not have been rented to any customer, especially with the remedy unavailable until late 2025. I am reporting this because an unrepaired safety recall vehicle placed into active rental service directly resulted in a visibility hazard and an unavoidable collision while I was driving.” I did not make it back in time to return the vehicle yesterday 11/14/2025 I am finishing up this report and on my way to Budget in Panama City Beach Florida to return it. We purchased our 2022 Ford Explorer brand new on April 12, 2022. The backup camera was nonfunctional that same evening. We brought the car to Porter Ford, where the issue was confirmed via diagnostic code U0264:00 ("Lost Communication With Camera Module Rear"). The dealership informed us that this was tied to a known recall, but that the necessary parts were unavailable. We waited more than two years for a resolution, during which time the camera system remained inoperable or intermittently failed. This defect directly impacts rear visibility, which is required under FMVSS 111. In August 2024, Willis Ford performed Recall 23S23, replacing the rear camera and wiring harness. The issue continued. In May 2024, they opened Recall 23S232, again noting a need for parts. In September 2024, the camera was replaced again. The issue persisted. In June 2025, the issue was finally diagnosed as a failed IPMB module and possibly a harness fault. Ford has since quoted us $2,937.90 to repair the issue—despite written confirmation from the service advisor to Ford that: "If the vehicle was still under warranty, this repair would be covered." We have not paid anything, and are refusing partial coverage, as the defect was: Reported within 24 hours of vehicle delivery Tied to multiple recalls Never resolved after years of authorized service center attempts This is a clear and ongoing safety defect affecting rear visibility. Ford has failed to comply with safety standards by not resolving the issue in a timely or effective manner. We request NHTSA investigation into Ford’s handling of this camera recall and similar issues in other 2022 Explorers.

Both driver and passenger low beam headlights started strobe, than completely stopped working while driving at night. This was extremely stressful and dangerous as I was no longer able to see the roads very well, my options were get a tow truck to pick up and bring my vehicle from Michigan back to Minnesota or sit on the side of the road until the morning also a safety risk. Upon inspection the following day, it was only the low beam that was not working and all other LED's were working. This is when I tried to replace both headlight modules, that did not fix the issue. I then bought new headlights thinking the low beam LED stopped working after going through 3 sets of headlights the issue was never resolved.

What component or system failed or malfunctioned, and is it available for inspection upon request? Sunroof headliner assembly failed. The headliner fabric detached and came off the track without an prior misuse or manual operation, it simply fell loose on its own. The material is now sagging and when trying to use the sunroof the fabric hangs in the line of vision causing a safety hazard. How was your safety or the safety of others put at risk? While the issue did not cause an immediate accident, the loose headliner obstructs visibility while trying to use could become a distraction to the driver.IT blocks the driver's line of sight or interferes with airbags in an emergency. Has the problem been reproduced or confirmed by a dealer or independent service center?The failure occurred spontaneously and I have a appointment to take the dealership for further inspection. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? No formal inspection has been completed at this time. The issue will be presented to a Ford service center for further evaluation. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warning lights or messages appeared prior to the failure. The issue occurred suddenly and without any prior indication. The fabric and track system separated unexpectedly during normal stationary conditions.

Consequence & remedy

Consequence: Battery failure can cause a loss of drive power, increasing the risk of a crash. Battery failure can also increase the risk of a fire.

Remedy: Dealers will perform a Battery Energy Control Module (BECM) diagnostic test and replace the high voltage battery pack, as necessary, free of charge. Ford Motor Company (Ford) is recalling certain 2020 Ford Explorer vehicles equipped with 2.3L or 3.3L engines. The wiring harness may not have been properly secured and may contact the air conditioner compressor (A/C) pulley, potentially damaging the harness or the A/C belt.

Consequence & remedy

Consequence: The A/C pulley may rub through the wiring harness insulation and contact the unfused battery positive wire, resulting in a short circuit, increasing the risk of a fire.

Remedy: Ford will notify owners, and dealers will install a tie strap to the wiring harness at the hand shock tower clip, free of charge. The recall began October 21, 2019. Owners may contact Ford customer service at 1-866-436-7332. Ford's number for this recall is 19S32.

Consequence: Without the cover, the MPR lever may be unintentionally moved, allowing the vehicle to roll away. Being in Factory Mode, the vehicle will not display warnings and/or gear selection on the instrument cluster. Both conditions increase the risk of a crash.

Remedy: Ford will notify owners, and dealers will inspect and, if necessary, install a MPR cover. THE GENERIC ELECTRONIC MODULE (GEM) COULD EXPERIENCE A CONDITION REFERRED TO AS "LOCK-UP" IN WHICH THE GEM CONTROLLED ELECTRONIC FUNCTIONS (E.G., FRONT WINDSHIELD WIPERS, INTERIOR LIGHTS, 4X4 SYSTEM, ETC.) COULD NOT BE TURNED ON OR, IN SOME CASES IF THE FUNCTION IS ON, COULD NOT BE TURNED OFF.

Consequence & remedy

Consequence: AN INOPERATIVE WINDSHIELD WIPER SYSTEM COULD AFFECT DRIVER VISIBILITY, INCREASING THE RISK OF A CRASH.

Remedy: DEALERS WILL INSTALL A RESISTOR IN THE GEM CIRCUIT.

Consequence & remedy

Consequence: THE STANDARD SPECIFIED THAT A WARNING TO THE DRIVER SHALL BE ACTUATED WHENEVER THE KEY HAS BEEN LEFT IN THE LOCKING SYSTEM AND THE DRIVER'S DOOR IS OPENED.

Remedy: DEALERS WILL INSTALL A JUMPER WIRE TO PROVIDE THE NECESSARY GROUND CIRCUIT IN THESE VEHICLES.

THIS INVESTIGATION REVEALED THAT CONSOLE FIRES IN THE SUBJECT VEHICLES MOST OFTEN OCCURRED WHILE VEHICLES EQUIPPED WITH THE LAMP OUTAGE MODULE (LOM), WHICH IS PART OF THE MESSAGE CENTER, WERE TOWING A TRAILER AND WHICH HAD BEEN MODIFIED BY ADDING AN AFTERMARKET TRAILER WIRING HARNESS INTO THE VEHICLE'S TAILLIGHT CIRCUITS.THE OWNER'S MANUAL STATES NEVER AND MORE THAN OTHER TRAILER LIGHT BULB TO ONE CIRCUIT HOWEVER TRAILERS FREQUENTLY CONTAIN SEVERAL BULBS.THE RESULTING EXCESS CURRENT PASSING THROUGH THE LOM COULD CAUSE IT TO OVERHEAT RESULTING IN A FIRE.PROPER AFTERMARKET INSTALLATION OF TRAILER WIRING REQUIRES THE USE OF A BYPASS MODULE, WHICH OBTAINS POWER FOR THE TRAILER LIGHTS DIRECTLY FROM THE BATTERY RATHER THAN FROM THE VEHICLE'S TAIL LIGHT AND STOP LAMP CIRCUITS, OR THE USE OF THE MANUFACTURER'S WIRING HARNESS.IN A LETTER DATED SEPTEMBER 2, 1997, FORD NOTIFIED ODI THAT IT INTENDS TO TAKE ACTION TO PRECLUDE FURTHER INCIDENTS INCLUDING ISSUING A TECHNICAL SERVICE BULLETIN TO ITS DEALERS, NOTIFYING TRAILER RENTAL ASSOCIATIONS AND AGENCIES WHERE KNOWN, AND ADVISING OWNERS BY LETTER OF THE PROPER METHOD FOR CONNECTION OF TRAILER TOW WIRING ON THE SUBJECT VEHICLES.THERE WAS NO DEFECT TREND IN THE SUBJECT VEHICLES IDENTIFIED AS A RESULT OF THIS INVESTIGATION, AND FURTHER EXPENDITURE OF AGENCY RESOURCES IS NOT WARRANTED TO CONTINUE THE INVESTIGATION.

THE CONTACT ASSEMBLY FOR THE REMOTE POWER MIRROR CONTROL SWITCH MAY DEVELOP A SHORT CIRCUIT BETWEEN THE SWITCH POSITIVE PAD AND CHASSIS GROUND PAD AS A RESULT OF WATER INTRUSION.EXCESS CURRENT FLOW RESULTING FROM THE SHORT CIRCUIT CONDITION MAY OVERHEAT THE REMOTE POWER MIRROR CONTROL SWITCH CIRCUIT BOARD AND CAUSE A FIRE IN THE DRIVER'S DOOR.FORD HAS INDICATED THAT CURRENT IN EXCESS OF 15 AMPS CAN OVERHEAT THE CIRCUIT BOARD ENOUGH TO CAUSE A FIRE.CIRCUIT PROTECTION IS PROVIDED BY A 30 AMP FUSE.FORD IS RECALLING ALL 1993-94 4-DOOR EXPLORER VEHICLES EQUIPPED WITH THE SUBJECT ELECTRONIC OPTION COMBINATION TO INSTALL AN IN-LINE 10 AMP FUSE IN THE POSITIVE BATTERY FEED TO THE REMOTE POWER MIRROR CONTROL SWITCH ASSEMBLY.NO 2-DOOR EXPLORER VEHICLES WERE BUILT WITH THE SUBJECT COMPONENTS.ACCORDING TO FORD, VEHICLES BUILT PRIOR TO THE RECALL PRODUCTION RANGE WERE EQUIPPED WITH LOWER CAPACITY CIRCUIT BOARDS FOR THE SUBJECT SWITCH ASSEMBLY THAT WOULD FAIL FROM AN OPEN CIRCUIT IF A SIMILAR SHORT OCCURRED.A NEW SWITCH ASSEMBLY WITH 10 AMP FUSE PROTECTION WAS INTRODUCED BEGINNING WITH THE 1995 MODEL YEAR EXPLORER.MAZDA WILL CONDUCT A PARALLEL COMPAIGN (97V-083.002) TO REMEDY THE SUBJECT CONDITION IN APPROXIMATELY 14,000 NAVAJO VEHICLES.THE NAVAJO VEHICLES WERE AVAILABLE IN ONLY THE 2-DOOR VERSION WHICH, UNLIKE THE EXPLORER VEHICLES, DID USE THE SUBJECT OPTION COMBINATION IN THAT BODY STYLE.
