I am reporting a safety concern involving a delayed transmission gear engagement and delayed rearview camera activation. Intermittently, when shifting the vehicle from Park into Reverse or Drive, there is a noticeable delay before the transmission actually engages the selected gear. During this delay, the rearview camera also fails to activate immediately. Because there is no immediate indication that the requested gear has engaged, I have experienced situations where I believed the vehicle was in Reverse or Drive, but it remained in the previous state for several seconds. In some instances, the transmission engages unexpectedly after the delay, causing the vehicle to move in a direction different from what I anticipated. For example, after selecting Reverse, the vehicle may hesitate before moving, or after changing between Reverse and Drive, the delayed engagement can result in the vehicle moving in the previously engaged direction before finally shifting into the selected gear. This creates a significant safety hazard, particularly when parking, backing out of driveways, or maneuvering in areas with pedestrians or other vehicles. The delayed rearview camera activation further increases the risk because it prevents the driver from immediately seeing behind the vehicle while attempting to reverse. The issue occurs intermittently and without warning, making it difficult to predict. I am concerned that this condition could lead to a crash, property damage, or injury if the vehicle moves unexpectedly due to the delayed gear engagement. I request that NHTSA investigate this issue to determine whether it is related to a transmission, electronic shift control, or software defect affecting the safe operation of the vehicle.

The contact owns a 2021 Ford Explorer. The contact stated that while driving at approximately 35 MPH down a hill, the engine and powertrain were resistant when the brakes were applied. No warning lights illuminated. A mobile mechanic was sent by a local dealer to inspect the vehicle. The mechanic determined that the failure was due to the transfer case but was not related to NHTSA Campaign Number: 26V402000 (Power Train). After test driving the vehicle, the mobile mechanic determined that the vehicle was unsafe to drive. The vehicle was not repaired. The manufacturer was contacted and referred the contact to the local dealer. The failure mileage was approximately 89,200.

While applying brakes coming to a stop light, the vehicle gave a message stating service brakes. The brakes still felt normal. Then after a few minutes, the brakes felt like they suddenly stopped working and then the rear tires started locking up and releasing. The frong brakes felt like they were not working at all. Stopping distance approximately tripled and not really any way to adjust braking pressure. Releasing the pedal would give a 2ish second delay before the brakes would reapply upon hitting the pedal again, and withing 1 second would lock up the rear brakes again, starting the cycle over again. Took approximately 3 applications to fully stop in a safe manner. Luckily I was already driving slow due to the warning and was able stop before hitting the car in front. Was driving approximately 30 mph when this happened. Under normal circumstances and driving distances this would have not been possible. The car was taken to the Ford. They determined there was internal failure in the Electronic Brake Booster. The fix was to replace the full assembly (EBB, ABS, etc), and flush the brake fluid. Along with diagnostics of the issue, the cost was $4,100.87 to fix. Since replacing the EBB assembly, the problem has gone away. There were no other indictions of an issue before this happened. I know other Explorer years, and other models that use this same component have been recalled for similar issues as well, and think there is a issue with this module as well.

Hi, i am concerned about the safety of my vehicle. i brought it to the attention of ford motor company and an authorized dealer that i am getting a parking brake warning on my dashboard. the authorized dealer inspected it, and said that the braking system is contaminated due to liquid spilling from the damaged cupholders. i communicated to them, that 1. the cupholders are clearly not strong enough to be placed over a critical component of the vehicle such as a braking system. 2. i think that this is a design flaw as there are normally drips and spills from having liquid in your car, especially in the designated place for it (cupholders). I should not be held liable for the repair to the braking system. they insisted this should be for my account, and if i decided not to fix it, it could lead to the entire braking system malfunctioning while the car is in motion or at a stand still. I am noting this with the department of transportation for investigation hopefully prior to an incident/accident taking place and the risk of a fatality, i have communicated to the dealer but they feel they have no accountability for this. while this may seem to be a cosmetic issue "just a cupholder", it is directly over a critical system of the car, and based on research i have done, i am not the only one with a broken cupholder in my ford explorer.

Part 573 Safety Recall Report 23V-675 The recall that is has been applied causes safety issues with the car when driving forward or in reverse. When putting the car into park the parking break auto applies. Now that is great however.. When putting the car into reverse or drive it does not release until the accelerator is applied. Now the problem is that the electric parking brake is delayed vs the acceleration causing the car to jerk forward or in reverse when the accelerator is applied. It acts like your trying to do a burn out holding down the break and it releases. This is not safe. A worker at the dealer almost rear ended a power pole in the parking lot because of this. My suggestion is to have the electric parking brake disengaged automatically when the car is put into drive or reverse. This will keep the protection of the electronic parking break being applied while in park and allowing smooth transition back to drive and reverse. Why has this issue been brushed under the rug..

When I showed up to the dealership they stated they can only order certain amount of parts and I wasn’t on the list yet was told over the phone I was placed on the list on May 22 2024.

Concerned about this situation, I took my vehicle to the service department at your dealership, where I was informed that there was a recall to replace a part related to this issue. I appreciate that you have identified and committed to addressing this problem through the recall. However, I am concerned that despite more than one or two months having passed since my initial visit, I have still not received any notification regarding the availability of the necessary part for the repair. Ford Motor Company (Ford) is recalling certain 2020-2027 Aviator and 2020-2026 Explorer vehicles. The switch for the easy-entry second-row outer seats may bind or stick, resulting in the seats unlatching, tipping, or sliding unexpectedly.

Consequence & remedy

Consequence: A seat that moves unexpectedly may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect and replace the bezel around the switch as necessary, free of charge. This will be a phased campaign, with the remedy becoming available in different phases based on model and model years. Interim letters explaining the safety risk were mailed August 19, 2026. Additional letters will be sent once the remedy is available, anticipated in January 2027. Lincoln Aviator owner notification letters were mailed August 21, 2026. The engine block heater may crack and develop a coolant leak, causing it to short circuit when the block heater is plugged in.

Consequence & remedy

Consequence: An electrical short circuit can increase the risk of a fire.

Remedy: Owners are advised not to plug in their block heater until the vehicle is remedied. Dealers will replace the block heater, free of charge. Owners will also have a alternative option to replace engine block heater element with a threaded blanking plug, and remove the block heater electrical cord. A software error may cause the rearview camera to display a blank image, or the image may remain on the display after the backing event has ended.

Consequence & remedy

Consequence: A rearview camera that displays a blank image can reduce the driver's view behind the vehicle. A rearview image that continues to be displayed after the vehicle is shifted out of reverse may distract the driver. Both of these situations can increase the risk of a crash.

Consequence & remedy

Consequence: A disconnected driveshaft can result in vehicle rollaway when the vehicle is placed in park without the parking brake applied. A disconnected driveshaft can also result in a loss of drive power. Either of these scenarios can increase the risk of a crash.

Ford Motor Company (Ford) is recalling certain 2020-2021 Explorer and Aviator vehicles. The seat belt buckle anchor bolts at one or more seating positions may be improperly secured. Additionally, vehicles may have an improperly secured seat belt retractor anchor bolt and/or seat belt anchor bolt at the second-row center seating position, if equipped.

Consequence & remedy

Consequence: A loose seat belt or seat belt buckle may not properly restrain an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will inspect seat belt buckle anchor bolts in all seating positions. Additionally, dealers will inspect the seat belt retractor anchor bolt and seat belt anchor bolt at the second-row center seating position, if equipped. If loose anchor bolts are found, the affected seat components will be replaced. Repairs will be performed free of charge. Owner notification letters were mailed on June 5, 2025. The engine intake valves may break while driving, which can result in engine failure and a loss of drive power.

Ford Motor Company (Ford) is recalling certain 2020-2022 Explorer vehicles equipped with the Police Interceptor Utility Package. In the event of an engine failure, engine oil and fuel vapor may be released into the engine compartment and accumulate near ignition sources such as hot engine or exhaust components, possibly resulting in an engine compartment fire.

Consequence & remedy

Consequence: An engine compartment fire increases the risk of injury.

Remedy: Owners are advised to park and shut off the engine as promptly as possible upon hearing unexpected engine noises, or after experiencing an unexpected engine power reduction, or seeing smoke from the engine compartment. Dealers will update the Powertrain Control Module (PCM) software. In addition, dealers will inspect for connecting rod bearing failure and if necessary, replace the engine long block. On July 22, 2022, the Office of Defects Investigation (ODI) granted a Defect Petition (DP22001) and opened a Preliminary Evaluation (PE22007) to investigate allegations of loss of motive power in model year (MY) 2021 Ford Bronco vehicles equipped with 2.7L EcoBoost engines. Complainants were reporting sudden engine failure while driving resulting in a loss of motive power, often without the ability to restart. During PE22007, the cause of engine failure and loss of motive power in the subject vehicles was determined to be intake valve fracture, and the full scope of vehicles affected by the subject defect was identified as all MY 2021-2022 Ford and Lincoln models equipped with either the 2.7L or 3.0L EcoBoost engines, referred to as the “Nano” engine family.  If an intake valve fractures in the subject engines, it may drop into the cylinder and contact the piston, often resulting in catastrophic engine damage. On September 29, 2023, PE22007 was upgraded to an Engineering Analysis (EA23002) to further investigate the subject defect across the full scope of affected vehicles. During EA23002, ODI visited Ford facilities to review technical information related to intake valve fracture in “Nano” engines, coordinated with NHTSA’s Vehicle Research and Testing Center (VRTC), and analyzed updated field data relating the subject defect. Forensic analysis of fractured intake valves demonstrated that the defective components exhibited “grinding burn” or out of specification hardness in the area of the keeper grooves. The presence of grinding burn is evidence that during the groove grinding phase of production, the temperature of the valve became sufficiently high to alter the microstructure of the material and is indicative that the valve supplier’s manufacturing processes were not within control specifications. Grinding burn results in a hard, brittle microstructure and high residual stresses toward the surface of the valve. Through normal engine loading, a valve with grinding burn will likely fracture at the area of highest vulnerability, which is the third keeper groove. In its response to the EA23002 Information Request (IR) letter, Ford provided evidence of 396 customer complaints (including field reports), 825 warranty claims, and 936 engine exchanges. When considering both ODI and manufacturer failure report data, there have been reports representing 1066 unique vehicles within the subject vehicle population. Analysis of the failure report data demonstrates that vehicles equipped with potentially defective intake valves were built within a production “spike period” of May 2021 – October 2021. The beginning of the spike period corresponds to a ramp up in production at the valve supplier manufacturing facility, and the end of the spike period aligns with the implementation of several mid-production process improvements and the decision to change the metal alloy used to produce intake valves from “Silchrome Lite” to “Silchrome 1”. Ford began investigating the subject defect in July 2021, determined the root cause, and took corrective measures for the new production vehicles. Analysis of the failure report data demonstrates that these corrective actions were successful in eliminating the risk of intake valve fracture for subject vehicles produced after October 2021. However, Ford did not take any field action to address vehicles produced during the spike period, and in the several months following the implementation of the production changes, a significant number of field failures continued to occur. It is Ford’s assessment that not all valves produced during the spike period are defective, and that failures associated with the subject defect occur at low time in service. Analysis of the failure report data demonstrates that the vast majority of failures have occurred before 20,000 miles with over half of all reported failures occurring before 5,000 miles. The rate of reported failures related to the subject defect has steadily decreased since November 2021. ODI coordinated with NHTSA’s National Center for Statistics and Analysis (NCSA) to conduct statistical analysis and predict the number of future failures associated with the subject defect. The results of this analysis, and the trends identified through the failure report data are generally consistent with Ford’s assessment that defective intake valves will likely fail at a low time in service and indicate that the majority of subject vehicles equipped with defective valves have already experienced a failure. On August 23, 2024, Ford submitted recall 24V635 for subject vehicles equipped with “Nano” engines and built within the production spike period identified above (total population 90,736 vehicles). The remedy includes an inspection at a Ford/Lincoln dealership where the lifetime engine cycles of the vehicle will be determined. If a vehicle does not meet a threshold for lifetime engine cycles, the dealership will conduct a high RPM engine cycle accumulation procedure, which aims to identify if the vehicle is equipped with defective intake valves. Any vehicle that fails the accumulation procedure will receive an engine replacement under the recall. In addition to recall 24V635, Ford has stated that it will launch customer satisfaction campaign 24N12, which will provide extended warranty coverage to the subject vehicles through the earlier of 10 years or 150,000 miles. The RQ was opened to focus on the effectiveness of the remedies of Recalls 22V-255 and 23V-199 related to the alleged problem. Recall 22V-255 involved replacing the bushing and axle cover in the following subject vehicles: 3.3L FHEV Police built before October 14, 2020, and 3.3L Gas Police vehicle built before January 31, 2021. For 2.3L RWD, 3.0L PHEV, 3.3LFHEV, and 3.0LST retail (non-police) vehicles, the recall provided an PCM software update to engage the Electronic Park Brake (EPB) when Park is commanded. The recall addressed a breakage of the rear axle bolt that can cause the driveshaft/half shafts to disconnect, resulting in a loss of transmission torque to the rear wheels necessary to hold park. If the parking brake is not applied, the loss of the primary park torque will allow the vehicle to roll in park increasing the risk of crash and injury. The affected vehicles had received a previous Powertrain Control Module (PCM) update which did not include an automatic EPB engagement when vehicle is shifted to park. On October 17, 2023, Ford filed a new safety recall (Recall 23V-675) covering 238,364 MY 2020-2022 Explorer vehicles with the following powertrains: 3.3L FHEV Police, 3.3L Gas Police, 3.0L Gas Police, 2.3L RWD, 3.0L PHEV, 3.3L FHEV, 3.0L RWD and 3.0L AWD, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. Ford stated that the cause of the bolt fracture is due to joint design not robust to peak axle input torques and manufacturing variability. The primary contributor is insufficient bearings for maximum joint loads. This can result in bearing deformation, increasing bending stress on the bolt, which may lead to fatigue failure. On January 19, 2023, Ford amended Recall 23V-675 to add 1,942 MY 2020-2022 Explorer vehicles with the following powertrain: 3.0L RWD Platinum and King Ranch variant, produced from October 22, 2018 to July 6, 2022, equipped with a 3-point mounted axle design. In addition to the safety recall, Ford will conduct a customer satisfaction program for the following MY 2020-2022 Explorer vehicles with the following powertrains: 2.3L AWD, 3.3L Gas Retail, 3.0L Gas AWD Platinum and King Ranch, produced from October 22, 2018 to July 6, 2022. Ford stated that the Explorer powerpack variants not covered under Recalled 23V-675 do not present an unreasonable risk to motor vehicle safety due to the combination of lower peak torque levels and torque management strategies to share inputs between front and rear axles, reducing occurrence of higher torque.  According to Ford, this is also evidenced by the lower current and projected failure rates. In view of the recall action being taken by Ford, ODI is closing this RQ. NHTSA will continue to monitor complaints from the customer satisfaction program vehicles and reopen this issue, if necessary. The agency reserves the right to take additional action if warranted by future circumstances.
