I WAS DRIVING SOUTH FROM AUSTIN TO NEW BRAUNFELS, TEXAS ON I-35 AT APPROXIMATELY 7:15 A.M. I HAD THE SPEED CONTROL SET FOR 55 M.P.H. AS I WAS LEAVING SAN MARCOS, AN ALARM TONE SOUNDED, AND THE DASH BOARD DISPLAY SHOWED "STOP SAFELY NOW" AND THE ENGINE SHUT OFF. I WAS ABLE TO MOVE TO THE SHOULDER USING BATTERY POWER ONLY AT LESS THAN 20 MILES AN HOUR, CAUSING ISSUES WITH OTHER TRAFFIC ON THE INTERSTATE, WITH MANY OF THE OTHER DRIVERS ANXIOUS TO GET TO WORK. I STOPPED THE CAR, AND WHEN IT WAS SAFE TO DO SO, EXITED THE VEHICLE AND DID A PERIMETER SCAN FOR ISSUES. NONE WERE DETECTED. I OPENED THE HOOD AND CHECKED FOR UNTOWARD ISSUES. THERE WAS NO SMOKE. THE ENGINE WASN'T OVERLY HOT. OIL WAS FULL TO THE APPROPRIATE MARK. THE VEHICLE HAD BEEN SERVICED BY THE DEALERSHIP WHO SOLD THE VEHICLE TO ME ABOUT 1790 MILES PRIOR TO THIS INCIDENT, AND A FULL DIAGNOSTIC WAS PERFORMED WITHOUT ANY ISSUES DETECTED. AFTER COMPLETING THIS CHECK, I RESTARTED THE CAR AND DROVE FOR ANOTHER 20 OR SO MILES WHEN THE ALARM AND DISPLAY REPEATED AS ABOVE. AGAIN THERE WERE NO ISSUES. I RESTARTED THE VEHICLE AFTER ABOUT THREE MINUTES AND WAS ABLE TO COMPLETE THE 75 MILE COMMUTE. THE RETURN TRIP HAD NO ISSUES. SINCE THE VEHICLE SHUT DOWN COMPLETELY WITHOUT WARNING IN FAST-MOVING TRAFFIC, THIS IS A CATASTROPHIC FAILURE. INFORMATION FROM THE INTERNET INDICATES THAT FORD HAS HAD THIS ISSUE SINCE THE FIRST ESCAPE HYBRIDS WERE ON THE ROAD, AND THERE APPEARS TO BE NO DEFINITIVE PROBLEM OR CORRECTION. I REQUEST THAT THE NHTSA EXAMINE THIS TO DETERMINE IF A RECALL SHOULD BE PERFORMED BEFORE SOMEONE IS KILLED BECAUSE OF A FAULTY ALARM THAT SHUTS THE VEHICLE DOWN COMPLETELY WITHOUT WARNING. *TR

FAILURE OF THE ELECTRONIC WATER PUMP FOR THE HYBRID SYSTEM CAUSES THE IMMEDIATE SHUTDOWN OF THE VEHICLE LEAVING DRIVER NO POWER TO MOVE TO A SAFE AREA. THERE IS NO WARNING BUT JUST AN IMMEDIATE ELECTRICAL SHUTOFF UNTIL DRIVER CAN MOVE TO THE SHOULDER AND RESTART THE VEHICLE. THIS OCCURRED TWICE AND LEFT US IN VERY DANGEROUS SITUATIONS BEFORE I HAD THE CAR REPAIRED. FORD WOULD NOT REPLACE THE PART CLAIMING IT IS NOT COVERED UNDER THEIR HYBRID WARRANTY ALTHOUGH THE PART IS INTEGRAL TO THE HYBRID SYSTEM. PART SHOULD BE RECALLED BEFORE FATALITIES OCCUR. *TR

I PULLED THE CAR TO THE SIDE OF THE ROAD, TURNED IT OFF, AND WAS ABLE TO RESUME DRIVING AFTER A FEW MINUTES. ON 5 SEPARATE TRIPS SUBSEQUENTLY, THIS SUDDEN LOSS OF ENGINE POWER HAS HAPPENED, OFTEN IN HEAVY TRAFFIC WHERE PULLING TO THE SIDE OF THE ROAD WAS VERY DIFFICULT AND DANGEROUS. THE FAILURE OFTEN HAPPENS MULTIPLE TIMES ON THE SAME TRIP. EACH TIME THE FAILURE HAS HAPPENED HAS BEEN IN THE SUMMER AND IN THE LATE AFTERNOON OR EVENING. EACH TIME THE CAR CAN BE RESTARTED ONCE PULLED TO THE SIDE OF THE ROAD AND TURNED OFF. *TR

A RED TRIANGLE WARNING LIGHT APPEARED ON THE DASHBOARD WITH THE FOLLOWING WORDS: "PULL OVER SAFELY NOW AND TURN OFF ENGINE". ALL 4 TIMES I WAS DRIVING AROUND 65 MPH ON AN INTERSTATE. ALL 4 TIMES THE CAR STARTED AGAIN NORMALLY AFTER A FEW MINUTES. AFTER THE 3RD INCIDENT, I STARTED THE CAR AGAIN SOON, AND IT STALLED AGAIN AFTER 10 MORE MINUTES OF HIGHWAY DRIVING. FIRST INCIDENT LATE JULY 2012, THEN LATE AUGUST 2012, AND TWICE ON JUNE 19, 2013. THIS IS A TERRIFYING EXPERIENCE, AND I WAS EXTREMELY LUCKY NOT TO BE REAR ENDED. THE DEALER CLAIMS IT CANNOT DUPLICATE PROBLEM OR SEE ANY CODES IN DIAGNOSTIC COMPUTER, REFUSES TO PURSUE. HELP!!!! *TR

FORD ESCAPE HYBRID (FEH) "STOP VEHICLE SAFELY" RED TRIANGLE WARNING AND IMMEDIATE ENGINE SHUTDOWN DUE TO THE WELL-KNOWN DEFECT IN THE BOSCH OEM MOTOR ELECTRONICS COOLING SYSTEM (MECS) PUMP IN THE 2005-2008 MODEL YEARS. SEE FORD TSB 08-24-5. IF THE NHTSA DID EVEN A CURSORY SEARCH ON THE INTERNET THEY WOULD SEE MANY, MANY HUNDREDS OF POSTINGS AND RELATED ACCIDENTS OR NEAR-MISSES. THIS IS A VERY SERIOUS SAFETY PROBLEM BECAUSE FAILURE IS ONLY TRIGGERED BY DRIVING AT HIGHWAY SPEEDS OR DRIVING UP STEEP HILLS AND THE SHUTDOWN IS IMMEDIATE (NO PRIOR WARNING), CAUSING SUDDEN AND SIGNIFICANT DROP IN SPEED AND FRANTIC ACTIONS BY THE DRIVER TO ABRUPTLY STEER THE UNPOWERED CAR TO THE SHOULDER. THERE ARE NO EXTERNAL INDICATORS TO WARN OTHER VEHICLES OF THE SUDDEN SLOWDOWN AND LANE CHANGE AND THE DRIVER DARE NOT USE THE BRAKES TO SIGNAL OTHERS BECAUSE TO HIT THE BRAKES WITH THE ENGINE SHUT DOWN AND THE VEHICLE ALREADY QUICKLY SLOWING TO A STOP WOULD CAUSE THE FEH TO ABRUPTLY STOP IN THE MIDDLE OF HIGHWAY. ALTHOUGH THIS PART IS UNIQUE TO THE HYBRID VERSIONS OF ESCAPES, FORD REFUSES TO CLASSIFY IT AS A UNIQUE HYBRID COMPONENT (WHICH WOULD BE COVERED BY THE 8 YR 100,000 MI WARRANTY) AND ROUTINELY CHARGES $800 TO REPLACE IT. IN MY CASE FAILURES HAPPENED FOUR TIMES IN THE LAST YEAR, ALWAYS ON INTERSTATE HIGHWAYS. TWO OF THE FAILURES NEARLY CAUSED MULTIPLE CAR PILEUPS BECAUSE THE INTERSTATE WAS FULL OF CARS AND SEMIS WHEN THE ENGINE CUT OUT. I AM FRANKLY SHOCKED THAT THE NHTSA DIDN'T REQUIRE A RECALL YEARS AGO FOR A WELL-KNOWN AND COMMON DEFECT THAT WITHOUT WARNING CAUSES FEH'S TO ABRUPTLY AND SIGNIFICANTLY SLOW DOWN SPECIFICALLY AT HIGH SPEEDS OR STEEP HILL CLIMBS, FORCING THE DRIVER TO ABRUPTLY CHANGE LANES TO GET TO THE SHOULDER. SEVERAL CLASS ACTION LAWSUITS ARE BEING PREPARED. ISN'T IT TIME THE NHTSA ADDRESSED THIS VERY SERIOUS DEFECT? *TR

MY 17 YEAR OLD DAUGHTER WAS DRIVING HOME APPROX 5:30 PM ON A BUSY FOUR LANE ROAD FROM AN ERRAND AND THE "STOP SAFELY NOW" LIGHT CAME ON. AT THAT POINT SHE COULD NOT GET OVER DUE TO TRAFFIC AND THE ENGINE SHUT OFF IN THE MIDDLE OF THE ROAD. SHE WAS ABLE TO COAST DOWN A HILL INTO A TURN LANE NEARLY HITTING AN ONCOMING CAR AND STOP THE CAR. WE ARRIVED ON THE SCENE AND WAS ABLE TO RESTART THE CAR AND BRING IT HOME. WE TOOK IT TO THE DEALER WHERE THEY WANTED TO REPLACE THE COOLANT MOTOR TO THE BATTERY WHICH WAS DEFECTIVE AND CAUSING THE HYBRID BATTERY TO OVERHEAT THEREBY SHUTTING DOWN THE ENGINE COMPLETELY. THIS IS TO PREVENT THE HYBRID BATTERY FROM OVERHEATING. I DO UNDERSTAND THE NEED FOR THIS TO ELIMINATE THE POSSIBILITY OF A FIRE, BUT THESE SHOULD BE REPLACED EVERY 50,000 MILES TO STOP THE ENGINE FROM SHUTTING OFF . WE WERE GETTING READY TO DRIVE ON A 600 MILE TRIP IN THIS CAR AND WOULD HAVE BEEN STRANDED ON THE HIGHWAY. THE WAY THE WARRANTY IS WRITTEN, IT IS NOT COVERED. AFTER RESEARCHING THE PROBLEM, THIS HAPPENS MORE FREQUENTLY ESPECIALLY AFTER 50,000 MILES. PLEASE INVESTIGATE AND PROVIDE A RECALL AND/OR FIX TO THIS PROBLEM ON THESE CARS. *TR

DP22005 · Pedestrian Alert Sounds

Opened Jan 27, 2023 · Closed Aug 7, 2023

Status: closed (inferred from source dates) · Electrical System:propulsion System

NHTSA received a petition on or about July 18, 2022, requesting that Federal Motor Vehicle Safety Standard (FMVSS) 141 be applied to all electric and hybrid vehicles operating in the United States. The petition can be reviewed at NHTSA.gov under ODI Number 11486072. FMVSS 141 establishes performance requirements for pedestrian alert sounds for motor vehicles. The standard applies to hybrid and electric vehicles that have a gross vehicle weight rating of 4,536 KG or less or are defined as low-speed vehicles. The standard became fully applicable to all such vehicles manufactured on or after March 1, 2021.On January 27, 2023, NHTSA opened Defect Petition (DP) 22-005 to evaluate the subject matter described in the petition. On June 24, 2023 and as supplemented on June 25, 2023, the petitioner notified NHTSA he was withdrawing his petition. The petitioner indicated that, based on his review of data, there is no justification for asserting potential benefits that could be derived from actions sought by my petition. Based on the petitioner's withdrawal, DP22-005 is closed. Closure of this DP does not represent a determination by NHTSA regarding the subject matter of the petition.

DP15001 · Loss Of Power Steering While Driving

Opened Apr 1, 2015 · Closed Jun 14, 2018

Status: closed (inferred from source dates) · Steering:electric Power Assist System

On May 27, 2014, amended June 2, 2014, Ford Motor Company (Ford) submitted a Defect Information Report (DIR) to NHTSA describing a safety defect that may result in a sudden loss of power steering assist while driving in approximately 746,067 model year (MY) 2008 through 2011 Ford Escape vehicles equipped with electric power assisted steering (NHTSA 14V-284, Ford 14S05). Ford's DIR described the defect condition as a poor signal to noise ratio [SNR] in the torque sensor within the Electric Power Assisted Steering (EPAS) system [which] does not allow the PSCM to determine the driver's steering input. When the system detects this fault condition, it transitions the EPAS system to the fail-safe/manual steering mode.Ford's DIR indicates that loss of power steering assist while driving would require higher steering effort at lower vehicle speeds, which may result in an increased risk of a crash.Ford's remedy instructs dealers to check the Power Steering Control Module (PSCM) for Diagnostic Trouble Codes (DTCs) to determine the proper repair procedure. If no DTCs are present, dealers are to update the PSCM and instrument cluster module software. The updated PSCM software changes the torque sensor fault strategy so that the SNR condition does not result in a loss of power steering assist while driving. In addition, audible and visual warnings are provided for torque sensor faults. If DTCs indicating faults in the torque sensor, PSCM or Power Steering Motor (motor) are present in the initial recall inspection, the dealer performs the following repairs: 1) replaces the torque sensor for torque sensor faults (DTC B2278); or 2) replaces the steering column for faults related to the PSCM (DTC B1342) or motor (DTC B2277).Continued in attachment pages

DP12006 · Electronic Throttle Body Malfunction

Opened Oct 2, 2012 · Closed Feb 21, 2013

Status: closed (inferred from source dates) · Engine

In a letter dated August 30, 2012, The North Carolina Consumers Council (NCCC) petitioned the National Highway Traffic Safety Administration (NHTSA) to initiate a defect investigation of alleged electronic throttle body failures resulting in engine stall or surge while driving in model year (MY) 2005 through 2012 Ford Escape vehicles. On October 2, 2012 the Office of Defects Investigation (ODI) opened a Defect Petition DP12-006 to evaluate whether to grant or deny the petition. The petition is hereby granted on certain model years. The NCCC letter cites two complaints of stall while driving in MY 2009 Ford Escape vehicles that were diagnosed as failed throttle bodies with diagnostic trouble codes (DTC) P2111, Throttle Body Stuck Open, and P2112, Throttle Body Stuck Closed. The petitioner indicates that the owners of both vehicles reported experiencing repeated incidents of stalling and engine surging. According to Ford, Escape non-hybrid vehicles are equipped with Electronic Throttle Body (ETB) assemblies beginning with MY 2009. Vehicles manufactured between June 22, 2009 and October 15, 2009 may contain contaminated printed circuit boards (PCB) with plating variations. Plating variations could lead to a lack of continuity in the throttle position sensor circuit where P2111 and/or other DTCs would be generated and stored. Ford and its electronic throttle body supplier, Delphi, modified the PCB manufacturing process and issued a technical service bulletin (TSB) 09-23-5. Vehicles produced after October 15, 2009 incorporated ETBs manufactured with a PCB process that resolved this condition. ODI's analysis identified a total of 123 unique reports indicating allegations of reduced motive power or vehicle stall. Depending on the condition of throttle control system malfunction, a driver may experience varying levels of reduced engine performance associated with three limp home modes. In two limp modes, reduced engine performance may maintain vehicle speeds above 20mph while the third is a limited limp home mode with engine speeds limited to approximately 900 RPM. Allegations of stall appear to be related to the limited limp home mode. Vehicles are not likely to unexpectedly stall as a result of this condition, but drivers may characterize the reduced functionality as a stall, even though their vehicle may still has motive capability. Allegations of vehicle surge appear to be related to limp home mode operation. Complaints alleging surge are most likely related to engine RPM fluctuations at low vehicle speeds or idle as the control system engages to prevent engine stall. In limited limp mode, rough-idle conditions may exist while the control system attempts to modulate engine RPMs once the vehicle reaches a reduced speed to maintain approximately 900 RPM. Separately, ODI received 59 complaints alleging incidents of engine stall while driving in model year (MY) 2010-2011 Ford Fusion vehicles. Approximately 60 percent (36) of the incidents occurred at speeds of 40 miles per hour or more. Eighty percent of complaints (47) were received beginning March 2012 and 14 complaints reported that the throttle body was replaced or DTCs associated with limp home modes. Additionally, Ford issued TSB 10-21-6 addressing DTCs associated with idle speed drops and limited limp home mode. The petition is granted on certain model years.Preliminary Evaluation PE13-003 has been opened to assess the scope, frequency and potential safety consequences associated with the alleged defect.See full copy of the closing resume for this defect petition for list of the VOQs associated with the defect petition analysis.
