FIRESTONE, LT225/75R16, MS STEEL TEX RADIAL R4. THERE WAS A LOUD POP, AND THEN THERE WAS SOMETHING FLAPPING AGAINST TRAILER. TOOK VEHICLE TO FIRESTONE, AND DEALER STATED THAT DRIVER'S SIDE REAR OUTSIDE DUAL TIRES HAD TREAD SEPARATION. *AKTHERE WAS DAMAGE TO THE MOTOR HOME. THE TIRE WAS DESTROYED BY THE DEALER. VEHICLE DESCRIPTION: CLASS A AND C MOTOR HOMES EQUIPPED WITH SAFETY BELT ASSEMBLIES BUILT BY AM-SAFE. ON SOME SEAT BELTS, THE BUCKLE AND CONNECTOR MAY UNLATCH DURING A COLLISION.

Consequence & remedy

Consequence: SERIOUS INJURY OR DEATH COULD OCCUR IF THE VEHICLE OCCUPANT IS NOT PROPERLY RESTRAINED.

Remedy: AM-SAFE IN NOTIFYING CUSTOMERS THAT THEY WILL INSPECT AND REPLACE THE DEFECTIVE BUCKLE ASSEMBLIES FREE OF CHARGE.
