I was driving my Pacifica with my special needs children to thier therapy appointments. The power steering light came on all of a sudden going down the highway at 50-55mph. Steering became extremely difficult. I was barely able to take the the turn to pull into the parking lot. This is even more dangerous as their is a drop-off on the other side if i had not managed to turn aggressively enough. It scared both of my children and myself. The dealership wanted $206 diagnostic fee which we cannot afford. The light will go off and we regain steering once it has been off for a few minutes then light kicks on and we lose steering again.

The contact owned a 2022 Chrysler Pacifica. The contact stated that while driving approximately 55 MPH, a very loud snapping noise had suddenly occurred, and the message "Stability System Activated" displayed before the steering wheel suddenly turned to the right, causing the driver to lose control of the vehicle to crash and roll over multiple times. The police and first responders were called to the scene, and the occupants were transported to the hospital. During the crash, the driver sustained multiple neck and back fractures and a right leg injury from the seat belt strap. The front passenger sustained multiple rib fractures. The vehicle was destroyed and later towed away. The cause of the failure was not determined. The manufacturer and local dealer were not notified of the failure. The contact also indicated that due to the injuries sustained in the crash, his right hand is unusable. The VIN was not available. The failure mileage was 5,000.

On three separate occasions (December 2023-February 2024), while driving in temperatures below freezing, our 2022 Chrysler Pacifica Hybrid dashboard gave the following warning, along side the illumination of a red turtle warning light: "Stop Safely. Vehicle will shut off soon." The vehicle then lost the ability to accelerate, and the speed dropped significantly and immediately. Two of the three times, my one and three year old were in the vehicle and there was no shoulder on the 50mph road we were traveling on after sunset. Fortunately, the three times this happened, we were less than a half mile from our home and we coasted downhill to the bottom of our driveway. Our vehicle has now been at a certified Chrysler dealership for three full business days, and the technician "has no idea what is wrong," "is still working on it," "has no idea how long it will take to fix," and "has no rental or loaner cars available" for our family while we wait for our family's mode of Transportation to be returned. They claim they are unable to recreate the issue, so they cannot fix it. We showed them a picture of the dashboard, but that is not enough for them. It is currently 72 degrees in Dayton, OH, and the issue has never occurred in temperatures above 30 degrees. Thus, I doubt they will be able to "recreate" the issue until next winter. They've offered no other options or resolution until they can "recreate the issue."

Traveling down a country road on a clear afternoon, the vehicle traveled approximately 35 mph while cresting a small hill. On the other side of the hill was a "T" shaped intersection where another vehicle stopped in the intersection from a stop sign several feet behind. The other driver failed to complete the turn when they saw oncoming traffic. The driver noticed the intersection had an obstructed path and attempted to move into the oncoming traffic lane since no oncoming traffic was visible. Just then, the onboard computer installed in the control panel of the drivers seat had acted upon either the obstruction or the switch in lanes and assumed complete control of the braking and steering systems. The driver was unable to retrieve control from the computer systems artificial intelligence before a direct crash occured with a telephone pole. Rendering the vehicle dangerous as live wires draped the saturated earth below the tires and feet of passengers exiting the vehicle. The manufacturer was contacted, but no follow up has been made. Neither the police or insurance representatives have been helpful or any investigation made into this account. There were several lights, sensors, noises, and messages that were flashing and beeping but none were able to be understood before the sudden impact.

Power steering went completely out. Vehicle has only 6300 miles on it. Luckily we were driving slow and very close to home. Vehicle is almost impossible to turn. This could have been extremely catastrophic if wife or daughter were driving on highway.

Consequence & remedy
Consequence: A vehicle fire increases the risk of injury.
Remedy: Owners are advised not to charge their vehicles, and to park outside and away from structures, until their vehicle is repaired. Dealers will update the high voltage battery pack control module software. In addition, dealers will inspect and replace the battery pack assembly, as necessary. On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV). On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown. An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash. In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008. The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode. The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010. These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010. The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down. When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced. Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. The Office of Defects Investigation is opening this Recall Query (RQ) to assess the adequacy, frequency, and safety consequence of the remedy for recall 23V-010. On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy.
