I experienced a sudden loss of power and steering while driving my vehicle. The screen advised me to pull over because the engine would shut off soon, but I already had lost steering and power. I turned the car off and was able to restart it and drive it 1 mile home, and the check engine light was on. I had it towed to the dealer. After 2 weeks of testing, the dealer reported, “When your vehicle was initially checked in, the following trouble codes were present: 5 BPCM FAULT CODES P1A21-00, P1D43-00, P1C01-00, P1A2A-00, and P1A29-00. We have completed Recall Y40.” The repair tech said they expected the engine to lose power again at some point and I would need to have it towed in again to be repaired then. The repairs made so far are only software updates, no hardware has been repaired because the dealer said the errors didn’t recur after they reset the system. I have elevated the issue with the dealer and am awaiting a response. I am concerned based on the error codes and technician information that my vehicle is still at risk of catching fire at any time or experiencing another catastrophic loss of power if the faulty hardware is not replaced.

When driving on highway, electrical system started beeping and showing various signals, could no longer keep speed or accelerate. Had to put on warning lights and coast to side of road with four small kids in car. Was able to pull off to side and slowly get to safe location by resting vehicle in between events. When fully charged, didn’t seem to have issue so was able to drive to dealer. Thought was related to engine switching from electric to gas power. Dealer checked and performed two software updates. On way home from dealer, same problem occurred on highway. Super dangerous to loose all acceleration or same speed on highway! I got a video of second event as I was a passenger and my husband was driving. But as can see in video the car has a charge so the incident is not related to the switch from electric to gas power. Now we are very concerned whether Chrysler has the means to fix this at all and or whether the dealership will be actually able to identify the issue given the car was literally being driven home from the dealer. The hybrid propulsion system on my 2020 Chrysler Pacifica Plug-In Hybrid has repeatedly failed, resulting in sudden and complete loss of power while driving at highway speeds. The vehicle enters full limp mode, displays a red turtle warning light, and the motor cuts out entirely, creating a serious collision risk. The failure occurs without warning and has happened on multiple occasions. The root cause has been traced through escalating component failures: first the Power Inverter Module (PIM) was found to have an internal failure disabling regenerative braking and causing the motor to cut out; subsequently the battery charging module was found to be failing, causing the high-voltage battery to drain to 0% and stall the vehicle. Both components were replaced by an authorized Stellantis dealership under warranty. Despite replacement of the PIM, the charging module, and ultimately the transmission, the defect has not been resolved. The vehicle has been confirmed defective by the authorized dealer on each visit and has been subject to a STAR (engineering escalation) case with Stellantis. Warning symptoms include the red turtle warning light, check engine light, and sudden power loss. The vehicle has been out of service for cumulative warranty repairs exceeding 130 days, including a continuous period exceeding 90 days. The vehicle remains unrepaired and in dealer custody as of the date of this filing. The dealer has confirmed the defect on each visit and has conducted multiple rounds of diagnostic testing and component replacement, none of which have restored safe and reliable operation. The component failures are available for inspection upon request.

The hybrid battery overheated and started burning the interior of the vehicle while it sat turned off charging in the garage. Smoke filled the house which had to be evacuated. Emergency services were called and the fire department brought the incident under control. An incident report was completed by the fire department and the vehicle is currently in the possession of insurance who is doing their own evaluation. There were no warning indicators or other problems prior to this happening.

The issue is the Pentastar V6 engine defect. It has caused engine misfires resulting in bucking and surging, decreased engine performance, hesitation, loss of power, premature wear on internal components. The potentially dangerous issues on the “defective design” of the engine’s needle bearings within the rocker arms, the use of sub-standard materials and procedures during manufacture, “defective or miscalibrated” ECM software, and poor quality controls.  The Defect causes unsafe driving conditions because the Class Vehicles, that includes 2020 chrysler pacifica, have a significant chance of failing while being driven. Further, even the lesser symptoms of the Defect affect vehicle performance and safety, making it harder for a driver to control the vehicle as it loses power, hesitates, or misfires. the vehicle has only been inspected by the mechanic at the dealership we leased the vehicle from.

While driving at highway speeds a warning flashed on the dashboard "Stop Safely Vehicle Will Shut Off Soon". I was able to coast to safe stop off the highway and drive slowly on battery powered acceleration for a few miles until the vehicle completely died. The gas engine did not engage during this period. I shut the engine off with emergency lights on while on a two lane road. After restarting the engine, I drove for about 30 seconds before it completely failed again, and pulled off in a residential area. I waited about 5 minutes before restarting the engine and attempted to drive the vehicle again. The gas engine did not engage and the same warnings were displayed as we attempted to reach a charging station to get to safe place to stop. The vehicle lost all power again after about 20 seconds and we pulled over. I waiting about 10 minutes until restarting the vehicle and the gas engine engaged. We were able to drive the vehicle to a safe location to park and unload 2 kids and an infant. My family was absolutely at risk during this incident. I'm bringing the vehicle to a dealership immediately and considering a complete replacement. This is unacceptable. A family should not have to fear for their lives while driving a American made vehicle on American roads.

My car has been going thru the refresh for gas and oil for the last few days. the motor has been running to empty out the "old" gas. so today, i took it thru a car wash on a conveyor belt system, with cars behind me. at the end of this i went to put it in gear so i hit the brake from the neutral position and put it in drive. i hit the gas and it went nowhere! there was a note on the dash that said i had to put the car in park, hit the brake and then put it in gear. i did this and it finally started to move. the car and five others behind could have been in serious trouble if i didnt get this figured out quickly, damn close to causing a wreck inside the car wash. now you tell me why a safety feature almost killed me?????

THE DEALERSHIP TECHNICIANS SAID (THEY HAVE NEVER SEEN THIS CONDITION) IT CAME FROM THE CHRYSLER OEM PLANT THIS WAY, SAID THAT ALL THE BOLTS TO THE COOLANT TEMPERATURE HOUSING THAT FITS ONTO THE ENGINE BLOCK WITH BOLTS WERE COMPLETELY LOOSE AND THE ONLY WAY FOR THIS TO HAPPEN IS THAT THE OPERATOR AT THE MANUFACTURING PLANT THAT ATTACHES THESE UNITS TO THE ENGINE DID NOT OR FORGOT TO BOLT DOWN COMPLETELY THE COOLING UNIT TO THE ENGINE. Chrysler (FCA US, LLC) is recalling certain 2020-2022 Pacifica Plug-In Hybrid Electric (PHEV) vehicles. The battery pack may cause a vehicle fire, even when the vehicle is parked with the ignition in the "Off" position.

Consequence & remedy

Consequence: A vehicle fire increases the risk of injury.

Remedy: Owners are advised not to charge their vehicles, and to park outside and away from structures, until their vehicle is repaired. Dealers will update the high voltage battery pack control module software. In addition, dealers will inspect and replace the battery pack assembly, as necessary. All repairs will be performed free of charge. Owner notification letters were mailed beginning June 23, 2026. Owners may contact FCA US LLC customer service at 1-800-853-1403. FCA US LLC's number for this recall is 52D. The Vehicle Identification Numbers (VINs) involved in this recall will become searchable on NHTSA.gov on June 11, 2026.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Chrysler (FCA US, LLC) is recalling certain 2020-2023 Pacifica and 2020-2021, 2023 Voyager vehicles. An improperly secured connector for the side curtain air bags may prevent the air bags from deploying. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard numbers 214, "Side Impact Protection" and 226, "Ejection Mitigation."

Consequence & remedy

Consequence: An air bag that does not deploy during a crash increases the risk of injury.

Remedy: Dealers will secure the connector, free of charge. All vehicles have been remedied. Owner notification letters were mailed October 22, 2024. Owners may contact FCA customer service at 1-800-853-1403. FCA's number for this recall is A8B.

Chrysler (FCA US LLC) is recalling certain 2020 Jeep Gladiator and Jeep Cherokee, 2019-2020 Ram 1500 Pickup, Ram 2500 Pickup, Ram 3500 Pickup, Chrysler Pacifica, Dodge Durango, Jeep Grand Cherokee, Jeep Wrangler, and Jeep Renegade and 2019 Dodge Challenger vehicles equipped with 8.4" or 12" radio displays. A software error can cause the rearview camera image to remain displayed after the vehicle has been shifted out of reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rearview Mirrors."

Consequence & remedy

Consequence: The lingering rearview image can distract the driver, increasing the risk of a crash.

Remedy: Chrysler has notified owners, and dealers will update the radio display software, free of charge. Optionally, owners can choose to remotely update their software via an Over-The-Air (OTA) update available as of May 1, 2020. The recall began April 27, 2020. RQ24004 · Loss Of Motive Power Due To An Internal Wiring Connector Short.

On July 22, 2022, the Office of Defects Investigation (ODI) opened investigation PE22-008 regarding complaints alleging a loss of motive power condition potentially due to a transmission malfunction in Model Years 2019-2021 Chrysler Pacifica Plug-In Hybrid Electric Vehicles (PHEV).  On January 17, 2023, Fiat Chrysler Automobiles (FCA) issued safety recall (NHTSA Recall 23V-010) involving 2017-2023 Model Years (MY) Pacifica Plug-In Hybrid Electric Vehicles (PHEV) produced between August 12, 2016, through January 9, 2023. The recall states that the loss of motive power condition in these vehicles is caused by an internal transmission wiring connector that may short circuit, resulting in an unexpected engine shutdown.  An unexpected engine shutdown can cause a loss of drive power, increasing the risk of a crash.  In discussions with FCA on Recall 23V-010, ODI confirmed the recall was related to investigation PE22-008.  The remedy for 23V-010 is to update the Power Inverter module (PIM) and/or Instrument Panel Cluster (IPC) software to provide drivers a message stating “Stop Safely - Vehicle Will Shut Off Soon” followed by a chime and MIL, PRNDL blinking (on cluster and shifter knob), low SOC (State of Charge) warning and reduced power mode.  The recall remedy does not replace the defective hardware that was described in Recall 23V-010, and the wiring connector short still exists on these vehicles. To date, ODI has received 132 reports for the population of vehicles subject to recall 23V-010.  These reports allege a loss of motive power condition that reduces the vehicles speed, followed by dashboard warning lamps displaying messages associated with recall 23V-010.  The presence of these warning lamps and messages indicates the vehicle likely had the recall remedy applied. Follow up discussions with FCA confirm their remedy does not prevent a loss of motive power, but instead limits torque to the engine and after an unspecified number of miles or period of time, the engine will shut down.  When this occurs, FCA relies on the owner to transport the vehicle to a dealership after they experience a loss of motive power condition, at which time the defective hardware is replaced.  Furthermore, the dashboard messages introduced in the recall remedy may cause the driver to pull over in an unsafe location. Pulling over in an unsafe location may expose the occupants to undue harm. PE22008 · Transmission Fault Leading To Loss Of Motive Power

On July 22, 2022, the Office of Defects Investigation (ODI) opened PE22008 to investigate allegation of loss of motive power, potentially due to transmission malfunction, in certain Chrysler Pacifica vehicles. In a January 17, 2023 letter to NHTSA, Fiat Chrysler Automobiles (FCA) submitted recall notice 23V-010 involving a transmission motor wiring defect in 67,118 model year (MY) 2017-2023 Chrysler Pacifica plug-in hybrid electric vehicles (PHEVs) manufactured from August 12, 2016 through January 9, 2023. FCA determined that an internal transmission connector short could result in an unexpected engine shutdown under certain conditions. Follow up correspondence with FCA confirmed this defect results in an electric motor position trouble code that was the basis for opening PE22008, therefore NHTSA is closing this investigation. The 23V-010 recall remedy is a software update to address the hardware failure associated with PE22008. When a hardware failure is detected, the vehicle limits motive power while instructing the driver to pull over to a safe location. NHTSA is concerned that drivers will pull over to an unsafe location when the warning message associated with the recall remedy displays on the dashboard. Due to the nature of the recall remedy and related reports, NHTSA has opened Recall Query RQ24004 to assess the recall remedy. The reports and failure report summary values included with this closing action are related to allegations of Recall 23V-010 remedy inadequacy. The Agency reserves the right to take additional action if warranted by future circumstances.
