On XXX, I was involved in a collision with my 2025 Cadillac Lyriq with vin # [XXX]. in which my vehicle struck a parked car, then a tree, then a curb, in sequence, at approximately 30 mph. The vehicle continued onto the curb after the final impact. All passengers sustained injuries and required X-rays and medical treatment. There were no warning lights or dashboard indicators prior to the collision suggesting any system malfunction. During the collision, the front airbags did not deploy despite multiple significant impacts. Additionally, Automatic emergency braking didn't engage as it should Following the collision, inspection showed extensive front-end damage including damage to the tires, suspension, and cross member, hood, grill, fender etc. requiring full front-end replacement. I am requesting that NHTSA investigate the non-deployment of the airbag system and the brake failure in relation to this incident. The contact owns a 2025 Cadillac Lyriq. The contact stated that while driving approximately 45 MPH on a four-lane highway with the cruise control deactivated, the vehicle unintendedly accelerated. The contact stated that the vehicle accelerated above 100 MPH. During the failure, the cruise control erroneously activated but the speed was not displayed. The contact attempted to stop the vehicle by depressing the brake pedal but it accelerated instead. OnStar was contacted for assistance and moments later the vehicle decelerated and was driven to the shoulder. The contact stated that the vehicle travelled approximately 3 miles before the vehicle decelerated. The contact exited the vehicle, and there was smoke coming from the wheel wells and the brake pads. A tow truck was called to the scene, and the vehicle was towed to a local dealer. The cause of the failure was not yet determined. The manufacturer was notified of the failure. The failure mileage was 10,000.

Forward Collision Avoidance

I was driving on the freeway and was between two cars. The car on my right was trying to merge into me. I immediately was breaking while trying to control steering. I attempted to honk three times and my honk horn would not work. In a case of an emergency, that is absolutely unacceptable a car like this should be able to honk.

I did some online research and am concerned that the accident may have been caused by a safety issue reported on our 2025 model Cadillac Lyriq involving its One Pedal Driving (OPD) feature. We had tried using OPD when we first got the car and didn't like it and have not engaged it again over the past year. However, after an Over The Air update last week, the feature seems have been engaged by itself. I noticed it the evening before and needed to research how to disengage. In the mean time my wife had to go out before I was able to disengage it, and not realizing the issue was anything but undesirable she went out to an appointment and had an accident pulling into a parking spot at Walgreens. The car surged forward into some small bushes and a small causing damage to the front bumper, grill cover, and several other pieces. No one was hurt, but the risk is obvious.

OnStar system on the vehicle is busted. Because of this the accident detection no longer works and the GPS is stuck in no mans land which makes it impossible to use their app to figure out charging percentage, how far and route to take to the nearest charging station.

My car is equipped with a forward-facing camera, mounted on the forward area of the interior rearview mirror. Additional cameras are mounted on the front bumper, trunk door, and on the rearview mirrors on each side of the car. Various sensors mounted throughout both bumpers face the front, rear, and each side of the car. At the time of this incident all sensors and cameras were clean and free of obstructions, and software was up to date. I was driving uphill, negotiating a tight left curve at approximately 30 MPH in a rural, forested area in the Santa Cruz Mountains (California.) There were no other conveyances, objects, or obstruction on the roadway in front or behind me. The weather was clear and the daytime visibility was excellent. The road was dry and I estimate its gradient at 30%. As I was about to enter the curve, the vertical dirt hillside appeared ahead and directly in front of the forward-facing cameras and sensors, which I supose, detected this topography. With minimal pressure, I had my right foot on the throttle to keep the aforementioned speed while turning. The cruise control was not in use. As I began to turn, and while still facing the hillside, the throttle stopped responding and the car decelerated. Simultaneously, a warning appeared on my dashboard screen, indicating the forward collision warning system was unavailable. I fully depressed the throtle but instead of gaining speed, the car slowed down to 12 MPH as I completed the turn. In quick succession, I lifted my right foot of the throtle and depressed it all the way to the floorboard but the car did not accelerate. The car eventually regained forward acceleration when I had the throttle pedal all the way against the floorboard but despite this severe throttle application, the car only accelerated very slowly. Thereafter, the throtle resumed normal operation and I continued my trip over the hills and back. I believe I could have been rear ended during this scary incident.

When I applied the brakes to stop in the space, the car did not respond, it continued to drive forward, up the curb, crashed through the store front, and eventually came to rest mid-way in the store, amidst the furniture and store structure.

General Motors (GM) is recalling certain 2026 Cadillac VISTIQ vehicles and 2025 Cadillac LYRIQ vehicles. The bolts attaching the high voltage battery to the interior floor may be missing or loose.

Consequence & remedy

Consequence: An improperly secured battery may become damaged in a crash, increasing the risk of a fire.

Remedy: Dealers will inspect the high-voltage battery bolts and replace or re-tighten the bolts, as necessary, free of charge. Owner notification letters are scheduled to be mailed September 8, 2025. Owners may contact Cadillac customer service at 1-800-333-4223. GM's number for this recall is N252511300.

General Motors LLC (GM) is recalling certain 2023-2025 Cadillac LYRIQ vehicles. The left or right front stabilizer bar bracket bolts may have been improperly tightened, which can allow the bolts to loosen and separate from the vehicle frame. A detached stabilizer bar or bracket can damage the vehicle's high voltage cables or EV battery coolant lines.

Consequence & remedy

Consequence: Damaged cables or coolant lines can result in a vehicle shutdown, causing a loss of drive power and increasing the risk of a crash.

Remedy: Dealers will inspect and tighten the left and/or right stabilizer bar bracket bolts, as necessary, free of charge. Owner notification letters were mailed May 8, 2025. Owners may contact Cadilac customer service at 1-800-333-4223. GM's number for this recall is N252494190.

Consequence: Incorrect head restraint information may lead to improperly adjusted head restraints, increasing the risk of injury in a crash.

Remedy: GM will mail owner's manual inserts with the correct illustrations and descriptions for rear outboard head restraints, free of charge. Owner notification letters were mailed April 30, 2025. EA26006 · Electronic Brake Module Component Failure

On April 11, 2024, the Office of Defects Investigation (ODI) opened Preliminary Evaluation PE24011 after receiving complaints concerning a loss of brake assist in 2023 MY Cadillac Lyriq vehicles. These complaints allege a hard brake pedal, followed by a “Brake System Failure” message at start up or while driving. GM shared records regarding several internal investigations for loss of brake assist due to alleged spindle fractures in the eBoost system. GM has stated that in the event of a spindle fracture, anti-lock brakes (ABS), stability control, and traction control remain functional until the vehicle comes to a complete stop. At this point, ABS, brake assist, stability control, and traction control functions are lost. The loss of these functions causes the vehicle to set one of three Diagnostic Trouble Codes (DTCs) based on failed performance criteria for the Electronic Brake Control Module (EBCM). These DTCs, in turn, activate several driver alerts: an audible chime, a visual “Service Brake System” message in the display, and multiple illuminated Malfunction Indicator Lights (MILs) for the ABS, Traction Control, and Brakes informing the driver of loss of functionality. The vehicle speed will also be limited to 70 KPH (43 MPH). Since opening PE24011, ODI has continued to receive reports of loss of brake assist. Some of these reports describe symptoms inconsistent with GM’s description of a spindle failure. These reports allege an immediate loss of brake assist functionality as opposed to the loss occurring after the vehicle comes to a complete stop. The sudden loss of brake assist during a braking event could result in extended braking distance, which increases the risk of a crash or injury. ODI believes further analysis is necessary to assess the potential for failures in GM’s eBoost system to resulting in safety-related outcomes. At this time, ODI is aware of the following vehicles that are equipped with this eBoost system: 2023-2026 MY Cadillac Lyriq, Chevrolet Colorado, and GMC Canyon; 2024-2026 MY Buick Enclave, Envision; Chevrolet Blazer EV, Equinox EV, Traverse; Cruise Origin; and GMC Acadia; 2025-2026 MY Cadillac Celestiq and Optiq. ODI is also aware of 2024-2026 MY Acura ZDX and 2024-2026 MY Honda Prologue vehicles that were built with this system. The Acura and Honda vehicles were manufactured by GM in a joint venture with Honda, and will be included in the Engineering Analysis as peer vehicles. ODI is opening this Engineering Analysis to further assess the safety-related consequences of a failed eBoost system. In addition, ODI is expanding the scope of the investigation to encompass all GM vehicles that share this eBoost system.
