Request for Investigation: Volkswagen is intentionally ignoring a widespread GPS/SOS module failure and leaving thousands of battery fire hazards on the road, while its dealers exploit safety service visits to extort customers. I urge the NHTSA to launch a formal investigation into VW’s failure to recall the defective GPS modules and their systemic practice of offering cash settlements to suppress safety defect reporting. Thank you for your attention to this urgent public safety matter. I stand ready to provide all supporting documentation, including the dealer’s fraudulent invoice and VW’s settlement communications, upon your request. I respectfully urge the NHTSA to act swiftly and decisively to prevent another corporate cover-up like Dieselgate, and before these unaddressed safety defects result in a fatal tragedy—as we recently saw with Hyundai's delayed recall following a child's death.

My wife was involved in an accident where the car (2023 VW ID4) drove itself and crashed into six parked cars while she was attempting to apply the brakes and had no control of the steering (it drove itself). I had the insurance carrier pull down the EDR data (attached) and I have attached the Police Accident Report (attached) for your review. Thankfully, no one was injured and I have enough liability coverage to cover the property damage but would like to see this matter investigated realizing we were lucky the next person may not be.

Continuously receiving error message to have front sensors cleaned. Vehicle brought into dealership several times, for weeks at a time. On last visit sensors replaced, and I was told that “ The front sensors have a level of damage caused by sand, salt and other road debris. This has resulted in some scratches which are triggering the sensors and giving you the warnings that you are referring to.” Given this, the front sensor issues are a manufacturing flaw. I asked this if the dealer and, given the stated cause of sand, salt and road debris the likely frequency. With no answer.

On October 04/2025 The vehicle was parked and stationary when it suddenly accelerated forward on its own. The brakes were completely unresponsive. I had a passenger in the front seat, and both she and I are certain that I was pressing the brake, but the car did not slow down at all. Within a few seconds, the vehicle gained speed, and I lost control. There was no space to maneuver, and I could do nothing but brace myself against the seat and watch my rear passengers. The vehicle continued forward and crashed into a tree directly in front, causing damage to the right side of the bumper, and a large rock in front also received the impact. The airbags did not deploy. The two rear passengers struck their heads during the impact and sustained bumps, and my dog was also affected in the collision.This accident occurred despite the car having a recent annual inspection and software update. The sudden acceleration is not the only issue I have experienced with this vehicle. I have had additional safety concerns. The doors would lock, unlock and open by themselves while driving on the freeway. On May 2024, my dog was trapped inside when the doors locked automatically with my keys inside the car. I had to ask a police official in my neighborhood for a phone to call roadside assistance, and it took several hours to get my dog out of the vehicle.

A message stating forward collision assist is unavailable. The adaptive cruise control does not see the vehicle in front of you. The vehicle randomly emergency brakes. And a four Mile drive it would emergency brake three times. Dealer stated it was fixed. I return to the vehicle an hour later stating the problem persists and it is not fixed or safe to drive. I have a text from the dealer confirming the problem persists they have unable to diagnose or repair. The car is in their possession since June 7th 2025

The cruise control failed to slow down , instead it accelerated . My safety and the safety of my families was put at risk due to a sudden stop in I25 southbound cause us to slow down and when I pushed the down arrows to slow us down and pushed the brake pedal we didn’t slow down, instead we were jolted forward in accelerating speeds. Thankfully we missed the car in front of us due to us me swerving into the shoulder to assure no collision. The problem has happened on 4 different occasions. Volkswagen is aware of the issue and we have not received any kind of update or remedy for the defect. No one has inspected it , we were to have an appointment this Monday Sept 9th to have it all checked out. The gps , sos button , road side assistance and speedometer aren’t functioning properly in our car as well. No warnings lights were on and no notifications of any problems.

It is very easy to inadvertently touch the "SET" button for the cruise control. The button is capacitive so a very light touch activates the button, and it is on the outer edge of steering wheel spoke, meaning a light brush while turning the wheel activate the button. When maneuvering at slow speeds (such as in a parking lot) this results in the car accelerating, uncommanded, and without warning, up to the minimum speed of the cruise control.

Cruise control / resume cruise can inadvertently be activated due to the haptic touch pads on the steering wheel. On multiple occasions when exiting a highway after using cruise control and while turning the steering wheel, my hand inadvertently brushes against the RESUME haptic touch pad, causing the vehicle to immediately speed up to my last preset cruise speed. This is a constant issue as I use my vehicle daily for work purposes, and has caused multiple near-accidents. Per my dealer, Volkswagen does not have a fix for this. This is a non-issue in my other VW vehicle, as it has button instead of haptic touch on the steering wheel. Haptic touch should not be on a steering wheel.

Volkswagen Group of America, Inc. (Volkswagen) is recalling certain 2023-2024 ID.4 vehicles. The high voltage (HV) battery cell modules may contain misaligned electrodes that can result in a battery fire.

Consequence & remedy

Consequence: A vehicle fire while parked or driving can increase the risk of injury.

Remedy: Owners are advised to park outside immediately after charging and not leave their vehicles charging indoors overnight until the recall repair is complete. Also, owners are advised not to use Level 3 DC chargers and to limit the maximum charge of the battery to 80% until the remedy has been completed. Dealers will replace the high voltage battery cell modules as necessary, free of charge. Owner notification letters were mailed March 18, 2026. The rearview camera image may not display when the vehicle is placed in reverse or when the park function is manually activated. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Consequence & remedy

Consequence: A blank rearview camera display can reduce the driver's rear visibility, increasing the risk of a crash.

Volkswagen Group of America, Inc. (Volkswagen) is recalling certain 2021-2024 ID.4 vehicles. The door handles may allow water to enter the circuit board assembly, which can cause the doors to open unexpectedly.

Consequence & remedy

Consequence: A vehicle door that opens unexpectedly while driving increases the risk of injury.

Remedy: Volkswagen dealers will inspect and replace the door handles as necessary, and update the vehicle software, free of charge. Owner notification letters were mailed on December 27, 2024. Volkswagen Group of America, Inc. (Volkswagen) is recalling certain 2023-2024 ID.4 vehicles. The interior sunshade for the panoramic glass roof may be insufficiently fire retardant. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 302, "Flammability of Interior Materials."

Consequence & remedy

Consequence: In a fire, fabric that is not fire retardant may further fuel a fire, increasing the risk of injury.

Remedy: Dealers will apply flame retardant to the sunshade fabric, free of charge. Owner notification letters were mailed December 15, 2023. Owners may contact Volkswagen's customer service at 1-800-893-5298. Volkswagen's number for this recall is 60G3.

Volkswagen Group of America, Inc. (Volkswagen) is recalling certain 2023 ID4 rear-wheel drive vehicles. The 12-Volt battery charging cable may contact the steering column shaft and cause the cable insulation to wear, possibly resulting in a short circuit.

Consequence & remedy

Consequence: A short circuit may cause a loss of drive power, increasing the risk of a crash. A short circuit also increases the risk of a vehicle fire.

Remedy: Owners are advised to park outside and away from structures until the remedy has been completed. Dealers will inspect the 12-Volt battery charging cable and repair or replace it, as necessary, free of charge. Owner notification letters are expected to be mailed February 27, 2023. Volkswagen Group of America, Inc. (Volkswagen) is recalling certain 2023 ID4 vehicles. The electric motor sealing ring was improperly assembled, which may allow coolant to leak into the high-voltage system and interfere with the CAN communication and power supply.

Consequence: Coolant that leaks into the high-voltage system may impact safety-critical systems and warning lights, causing a loss of vehicle control or unintended air bag deployment, and increasing the risk of a crash or injury.

Remedy: Dealers will inspect and replace the electric motor, if necessary, free of charge. Owner notification letters are expected to be mailed January 6, 2023. Owners may contact Volkswagen's customer service at 1-800-893-5298. Volkswagen's number for this recall is 19Q9.

Volkswagen Group of America, Inc. (Volkswagen) is recalling certain 2022 Golf R A8, Audi S3, RS3 Sedan, Q4 E-Tron SUV, A3 Sedan, and 2022-2023 Volkswagen Golf GTI, and ID4 vehicles. The rearview camera image may be blank or unresponsive when the vehicle is placed in reverse gear or when the park function is manually activated. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 111, "Rear Visibility."

Consequence & remedy

Consequence: An inoperative rearview camera display can reduce the driver's rear visibility, increasing the risk of a crash.

Remedy: Dealers will replace the rearview camera, free of charge. Owner notification letters were mailed to Audi owners February 14, 2023. Volkswagen owners will receive an owner notification letter once remedy parts become available. Owners may contact Volkswagen's customer service at 1-800-893-5298 and Audi's customer service at 1-800-253-2834. Volkswagen's numbers for this recall are (Volkswagen) 91EK and (Audi) 91EL.

RQ24010 · Inadvertent Door Opening

On May 16, 2024, the Office of Defects Investigations (ODI) opened Recall Query (RQ) RQ24010 to investigate the remedy and scope of recalls 23V-213 and 23V-312. The affected vehicles by these recalls included model years (MY) 2021 through 2023 Volkswagen ID.4 vehicles. ODI received reports of the vehicle’s doors opening while the vehicle was in motion post recall remedy.  ODI, also, received similar allegations on vehicles that were outside the scope of the recalls. Volkswagen issued recall 23V-213 on March 29, 2023, and recall 23V-213 on May 03, 2023. These recalls addressed the defect of water ingress into the circuit board assembly of the door handle. Water ingress can cause communication issues between the door handle and the door control until. This malfunction resulted in the vehicle door inadvertently opening while the vehicle was in motion. Over the course of the investigation, ODI received 17 unique VIN VOQ complaints of the vehicle doors opening while the vehicle was in motion occurring either post recall remedy or on a vehicle that was outside the recall scope. Volkswagen submitted 190 unique VIN complaints reporting the same circumstances. On September 4, 2024, Volkswagen submitted recall 24V-651 to address the issue of inadvertent opening of the door while the vehicle is in motion. The scope of this recall expands the affected vehicles to include all MY2021-2024 Volkswagen ID.4 models, including those remedied under the subject recalls. The recall remedy consists of replacement of the door handle assembly on all four doors as well as a software update.  The software update is performed to update the door handle parameters. The new door handle will have the membrane relocated to the back of the handle to avoid standing water within the handle.  Also, a protective coating will be on the front and back of the circuit board to avoid direct moisture formation. In view of the action being taken by Volkswagen, Recall 24V-651, ODI is closing this Recall Query.  The Agency reserves the right to take additional action if warranted by future circumstances.
