I PULLED UP TO ABOUT TWO FEET FROM THEIR GRASSY AREA, GOT INTO POSITION TO BACK UP INTO A PARKING SPACE, STOPPED, AND PUT MY 2011 SIENNA INTO REVERSE, AND AS SOON AS I GENTLY TOUCHED THE EXCELERATOR TO BEGIN BACKING UP THE VAN SPED FORWARD AT A HIGH RATE OF SPEED CRASHING INTO AND OVER AN 8 INCH RAILROAD TIE THAT HAD A STEELBEAM ON TOP THAT SEPARATED THE DRIVEWAY FROM THEIR YARD. I HAD TO SLAM BOTH FEET ONTO THE BRAKE TO GET IT TO STOP WHICH IT FINALLY DID. I THOUGHT I HAD NOT PUT IT IN REVERSE BUT I LOOKED AT THE GEARSHIFT AND IT WAS INDEED IN REVERSE. I JIGGLED THE GEARSHIFT TO MAKE SURE IT WAS SECURED IN THE SLOT AND IT WAS. I THEN PUT IT IN PARK AND SAT THERE FOR A MINUTE OR TWO TRYING TO GAIN COMPOSURE TRYING TO FIGURE OUT WHAT JUST HAPPENED, I WAS IN SHOCK AS NOTHING LIKE THIS HAS EVER HAPPENED TO ME BEFORE. AFTER THAT MINUTE OR TWO I DECIDED I COULDN'T LET THE VEHICLE SIT THERE AND NEEDED TO MOVE IT. SCARED, BUT I PUT IT IN REVERSE AGAIN, READY TO POUNCE ON THE BRAKES WITH BOTH FEET AND BARELY TOUCHING THE EXCELERATOR I STARTED MOVING IN REVERSE. IT WAS WORKING FINE THEN. I THEN GOT OUT TO CHECK FOR DAMAGE AND THE BUMPER ON THE DRIVER'S SIDE WAS DAMAGED FROM THE IMPACT. THE AIRBAG DID NOT DEPLOY, I WAS NOT HURT HOWEVER HAD ANY OF MY RELATIVES BEEN IN THAT AREA AT THAT MOMENT IT COULD'VE RESULTED IN AN INJURY OR FATALITY. I REPORTED WHAT HAPPENED TO THE LOCAL DEALERSHIP WHERE I BOUGHT IT AND THEN TO TOYOTA USA AS INSTRUCTED IN THE HANDBOOK. NOW I'M REPORTING IT HERE. AM AWAITING CONTACT SUPPOSEDLY WITHIN THE NEXT DAY OR SO TO SEE WHAT TOYOTA INTENDS TO DO WITH THE SIENNA. I WILL NOT DRIVE AGAIN IT UNTIL THE REASON FOR THIS DEFECT IS FOUND AND FIXED AND DAMAGES REPAIRED. THE OUTCOME WILL DECIDE WHETHER I WILL EVER BE A TOYOTA CUSTOMER AGAIN.

HELLO, I OWN A 2011 SIENNA LE V6. AFTER WARMING UP, THE PROBLEM DOES TEND TO FADE AWAY. BUT THE WARMUP PERIOD, ESPECIALLY ON COLD NORTHEAST DAYS, COULD TAKE AS LONG AS 20 MINS. AND 8 MILES OF CITY DRIVING TO OCCUR (AND THIS IS AFTER A 5 MIN. PARKED IDLE WARMUP PERIOD), AND EVEN THEN, THE VAN STILL EXHIBITS THIS REVVING ISSUE SOMEWHAT, JUST NOT AS SIGNIFICANT AS WHEN IT'S IN THE WARMUP STAGE. ON MY FIRST COMPLAINT LAST WINTER, THE DEALER CHANGED OUT THE THERMOSTAT AND CONSIDERED THE PROBLEM FIXED. HOWEVER, THE VAN CONTINUES TO SHOW THE SAME PROBLEM THIS WINTER. THE CAR BEGAN ACCELERATE BACKWARDS SO RAPIDLY THAT I THOUGHT I MUST INADVERTENTLY PUT MY FOOT ON THE ACCELERATOR. I PULLED MY FOOT AWAY FROM THE PEDALS BUT THE CAR CONTINUED TO ACCELERATE. I SHIFTED INTO PARK, BUT BEFORE THERE WAS ANY CHANGE IN SPEED THE CAR CRASHED INTO MY NEIGHBOR'S CAR ACROSS THE STREET, CAUSING OVER $5,000 IN DAMAGE. IF THERE HAD BEEN SOMEONE STANDING ACROSS THE STREET, IT WOULD LIKELY HAVE BEEN A FATALITY. I CONTACTED TOYOTA AND THEY SENT OUT A CORPORATE REPRESENTATIVE TO EXAMINE THE CAR. HE ACKNOWLEDGED THERE WAS RAPID ACCELERATION IN REVERSE, AND STATED THAT WAS "NORMAL" OPERATION FOR THE CAR AFTER A COLD START, AND THAT IT WOULD BE EVEN FASTER IN COLD WEATHER (WE LIVE IN SOUTHERN CALIFORNIA). HE STATED THEY HAD INCREASED THE CAR IDLE SPEED AFTER COLD STARTS FOR POLLUTION CONTROL PURPOSES, AND THIS WAS THE RESULT. I CAREFULLY REVIEWED THE OWNER'S MANUAL AND INSIDE OF THE CABIN FOR ANY WARNING ABOUT RAPID ACCELERATION IN REVERSE AFTER A COLD START, AND THERE WAS NONE. I HAVE ALWAYS BEEN A CAREFUL DRIVER WHO HAS MAINTAINED A GOOD DRIVER DISCOUNT, AND NOTHING REMOTELY LIKE THIS HAS EVER HAPPENED TO ME BEFORE. THE CONTACT DID NOT INFORM THE MANUFACTURER NOR WAS THE VEHICLE REPAIRED. THE CURRENT MILEAGE WAS APPROXIMATELY 2,400. THE FAILURE MILEAGE WAS APPROXIMATELY 300.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2004-2011 Sienna vehicles originally sold in, or currently registered in, Connecticut, Delaware, Illinois, Indiana, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, New Hampshire, New Jersey, New York, Ohio, Pennsylvania, Rhode Island, Vermont, Virginia, West Virginia, Wisconsin and the District of Columbia. The affected vehicles have a spare tire carrier assembly mounted under the vehicle whose attachment cable may corrode due to high concentrations of road salt splashing onto the spare tire carrier.

Consequence & remedy

Consequence: If the carrier cable is corroded due to road salt, the spare tire may separate from the vehicle, increasing the risk of a crash.

Remedy: Toyota sent owners in August 2014 an interim notification to advise owners of the recall and that dealers will perform an interim remedy to remove the spare tire and relocate it to the luggage compartment, free of charge. Owners are being mailed a second letter as parts become available and dealers will replace the spare tire carrier assembly with an improved one, free of charge. The remedy campaign is being performed in phases. The first phase of the recall began July 7, 2015. Owners may contact Toyota customer service at 1-800-331-4331. NOTE: 2004-2010 Siennas subject to this recall were subject to recall campaign 10V-160 initiated in 2010. DUE TO ITS PROXIMITY TO THE PARKING BRAKE PEDAL, IN LIMITED CIRCUMSTANCES THE STOP LAMP SWITCH BRACKET COULD BE DAMAGED BY THE OPERATOR'S FOOT DURING PARKING BRAKE APPLICATION.

Consequence & remedy

Consequence: THIS COULD RESULT IN BRAKE DRAG WITH ASSOCIATED BRAKE NOISE, VIBRATION, AND/OR ILLUMINATION OF THE BRAKE WARNING LIGHT. BRAKING EFFECTIVENESS COULD BE REDUCED INCREASING THE RISK OF A CRASH.

SOUTHEAST TOYOTA IS RECALLING CERTAIN MODEL YEAR 2005-2011 PASSENGER VEHICLES FOR FAILING TO COMPLY WITH THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 110, "TIRE SELECTION AND RIMS." THESE VEHICLES WERE SOLD WITHOUT THE REQUISITE LOAD CARRYING CAPACITY MODIFICATION LABELS.

Consequence & remedy

Consequence: A DRIVER MAY OVERLOAD A VEHICLE WHICH MAY INCREASE THE RISK OF A CRASH.

EA21002 · Desiccated Air Bag Inflator Rupture

From 2000 through 2017, Takata produced millions of air bag inflators using two types of phase-stabilized ammonium nitrate ("PSAN") propellant -- propellant 2004 and propellant 2004L. After prolonged exposure to high temperature cycles and humidity, inflators using propellant 2004 can degrade, causing the propellant to burn too quickly when ignited. The rapid burning can cause the inflator to rupture during deployment, potentially causing serious or even fatal injury to vehicle occupants. See 2016 Blomquist Report at www.nhtsa.gov/sites/nhtsa.gov/files/documents/expert_report-hrblomquist.pdf.Consequently, all frontal inflators using propellant 2004 that do not contain a "desiccant" (a substance that traps and holds moisture) in US vehicles are under recall. These "non-desiccated" inflators either have been or are required to be replaced.In some cases, the remedy part for these recalled inflators was, or will be, an inflator using either propellant 2004 or 2004L that does contain a desiccant. None of these "desiccated" remedy parts (which were installed in older model year vehicles) are currently under recall for a degradation concern. Certain subsets of desiccated PSAN inflators using propellant 2004 for use as original equipment, however, have been recalled for a degradation concern. All Takata inflators produced with propellant 2004L contain desiccant, and none of these desiccated inflators using propellant 2004L are under recall for a degradation concern. There have been no reported field ruptures in any non-recalled desiccated PSAN inflators.It is understood that desiccants fully saturate at some threshold, at which point any additional moisture will not be captured. This means the degradation process observed in non-desiccated inflators using propellant 2004 may also occur in non-recalled desiccated inflators using propellant 2004, assuming additional moisture enters the inflator and high temperature cycling occurs. Based on available information, desiccant saturation can occur within the first five years in the worst environments, and the time required for full saturation is affected by multiple factors. While no present safety risk has been identified, further work is needed to evaluate the future risk of non-recalled desiccated inflators using propellant 2004.Three entities -- Takata (now known as TK Global), the Independent Testing Coalition, and Exponent -- have been studying the long-term behavior of Takata desiccated PSAN inflators using propellant 2004L (as well as 2004) in the presence of moisture and temperature cycling. The research efforts, which include development of predictive modeling techniques and field sample analysis, are ongoing. To date, none of the researchers have identified field evidence showing that propellant 2004L is undergoing a degradation process that leads to aggressive deployment and potential rupture. However, the time in service of such inflators remains short compared to that of the inflators using propellant 2004. Further study is needed to assess the long-term safety of desiccated inflators using propellant 2004L.The Office of Defects Investigation is opening this investigation to examine whether a safety defect related to propellant degradation exists in non-recalled desiccated PSAN frontal inflators manufactured by Takata. This investigation will require extensive information on Takata production processes and surveys of inflators in the field.
