Toyota issued a recall due to suspected second row seat welds failing in a crash on October 7, 2025, with Safety Recall Notice mailed beginning of December 2025, and has not offered a remedy. The suggestion of not having anyone sit in the second row for over four months now is absurd and warrants investigation. We purchased the van with needing the space to occupy 5+ passengers and are not able to have all passengers sit in the third row. Using third row is inconvenient for car seats and parents in their mid [XXX] . This is complete negligence by Toyota. Keeping the van on a dealer lot for months on end will be bad for batteries, brakes and rotors. With no timeline for a remedy and by doing a welding repair it may cause future issues with the van from rust prevention, electronics and etc. We noticed unusual vibrations from 2nd row seats both with car seats in place and with just grocery bags within the 1st week of driving the van back in July 2025. We purchased a 2025 Toyota Sienna Woodland Edition in July 2025. On October 7, 2025 we were notified that vehicle is subject to a safety recall [NHSTA Recall No. 25V-668, Toyota Recall No. 25TA12 (Interim Notice 25TB12)] due to improperly welded seat rails. The recall states: "The subject vehicles contain second-row seats that are mounted on seat rails that may have been improperly welded. If these seats are occupied during certain high-speed collisions, the seat may lose structural integrity, increasing the risk of injury. All known owners of the subject vehicles will be notified to return their vehicles to a Toyota dealer. For all involved vehicles, the dealer will replace the second-row seat rails with rails that have proper welding, free of charge. For all involved vehicles, Toyota will notify customers by early December 2025." Following the recall notice, we returned our Toyota Sienna to the dealer due to the safety risk of putting our children (or any passenger) in the second row seats and we are currently driving a rental car. Toyota has not yet indicated a solution for this recall and has not provided a remedy as originally stated by December 2026. Given that it is now February 20, 2026 and our vehicle has been out of service for it's intended purpose for over 120 days without a remedy, we would like to officially log a complaint with the NHSTA.

We were notified by Toyota that it was unsafe to transport our children in the second row seats in November 2025 and the vehicle was under a recall. This puts our children at risk. A letter from NHTSA stated Toyota would have a remedy by 12/6. The manufacturer (Toyota) confirmed this recall. The seat always shook but we assumed it was the carseats shaking, but, upon removing them to take the car to the dealer, we were still hearing the shaking even without the carseats. Given that Toyota has not indicated a solution date for this and the problem was discovered in October 2025 I believe the manufacturer has failed or is unable to remedy this safety recall for our vehicle in a timely manner and would like to officially log a complaint with NHTSA.

Rollx Vans (Rollx) is recalling certain GMC Yukon, Sierra, Chevrolet Express 2500, Ford Transit, E-250, Toyota Sienna, Sienna Hybrid, Honda Odyssey, Chrysler Town and Country, Voyager, Pacifica, Dodge Grand Caravan, and Ram Promaster vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. Please refer to Rollx's recall report for specific model year information. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Rollx will work with Q'Straint to inspect and replace the retractors as necessary, free of charge. Owner notification letters were mailed February 5, 2026. Owners may contact Rollx's customer service at 1-800-956-6668.

Vantage Mobility, LLC (VM) is recalling certain 2021-2026 Honda Odyssey, 2021-2025 Chrysler Pacifica, and Toyota Sienna vehicles equipped with QRT-Deluxe and QRT-Max wheelchair restraints. The retractors may not lock, preventing the wheelchair from being properly secured.

Consequence & remedy

Consequence: An unsecured wheelchair can move during transit, increasing the risk of injury.

Remedy: Dealers will inspect and replace the retractors, free of charge. Owner notification letters was mailed January 27, 2026. Owners may contact Vantage Mobility customer service at 1-800-488-9082.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2022-2026 Toyota, Lexus, and Subaru Solterra vehicles equipped with a Panoramic View Monitor (PVM) system. Please see the recall report for a complete list of models. A software error may cause the rearview camera to freeze or display a blank screen when the vehicle is in reverse. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 111, "Rear Visibility."

Consequence & remedy

Consequence: A rearview camera that fails to display an image can reduce the driver's view behind the vehicle, increasing the risk of a crash.

Remedy: Dealers will update the parking assist software, free of charge. Owner letters were mailed January 2, 2026. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB13 and 25LB06. Subaru's number for this recall is WRE25.

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2025 Sienna Hybrid vehicles. The second-row seat rails may be welded improperly.

Consequence & remedy

Consequence: Improperly welded seat rails can prevent the seat from adequately restraining an occupant during a crash, increasing the risk of injury.

Remedy: Dealers will replace the second-row seat rails, free of charge. Owner letters were mailed May 15, 2026. This is a phased recall. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's numbers for this recall are 25TB12 and 25TA12.

As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) numbers 202, "Head Restraints," 207, "Seating Systems," and 225, "Child Restraint Anchorage Systems."

Consequence & remedy

Consequence: An improperly tightened seatback may not adequately restrain the occupant during a crash, increasing their risk of injury.

Remedy: Dealers will tighten the seat assembly bolts, free of charge. Owner notification letters were mailed April 1, 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Toyota's number for this recall is 25TA05.
